[Results of 1000 electrocardiographic exercise tests. Their correlation with previous ischemic cardiopathy and arteriosclerotic risk factors].

Exercise electrocardiograms were done on one thousand patients referred to the laboratory of exercise tests for: suggestive symptoms of acute heart failure, old miocardial infarction abnormal resting ECG, or evaluation of coronary reserve. The average value of cardiac rate reached for the group, was close to 80%. The maximum exercise loads managed by the men were superior to those of the women, and in general those managed in the negative test were superior in relation to the positive tests. Of the one thousand cases, 20.2% had positive exercise ECG's. There was no difference inthe percentages of positivity between the two sexes, 20.75% and 19.11% for men and women respecitvely. The percentages of positivity are greater in those subjects sent to the laboratory for suspicion of angina pectoris, old MI, or abnormal resting ECG, than in those referred for detection of ischemic heart disease. The groups of patients with diabetes mellitus, arterial hypertension, old MI, and abnormal resting ECG had the highest incidence of positive tests: 41%, 37.5%, 30.6%, and 28.2% respectively. The most frequent localization of the ST segment alterations was the anterior portion, with percentages of 85.1% similar to those mentioned in the literature. The frequency of arrithmias, of 12.4% in this group, is a little less than that described in similar groups, but it corroborates the predominance of non-lethal ventricular arrithmias. The mortality in the tests performed was null.

[Arterial hypertension in adolescents].

The clinical cases of 391 men and 609 women, adolescent all of them, are included in this study. T.A. values were obtained by the Korotkoff method and the lower figures in both measurements analized. The mean value for the men's cases were 112/72/67 and for the women's cases 105/71/67 respectively for K1, K4 and K5. The difference between the sistolic pressures is significant. Taking the mean men and 126/89/86 for the women. Such values might represent the higher values plus two standard derivations it was possible to obtain 139/92/88 for the normal limit. At higher values we found K1 at 1% for men and 10% for women; K4 at 1.2% for men and 5.7% for women; for K5 at 1.1% for men and 2.2% for women. There is a direct relation between obesity and higher figures in T.A. for K1 and K4 in both sexes (p.0.01). The use of too much salt influenced the coming up of K4 to (p.0.01).

[Values of arterial pressure in childhood and adolescence in Mexico].

Complications and mortality of blood hypertension depend on the time blood pressure levels remain above normal as well as on the values they reach along the evolution. In order to detect blood hypertension before it is well established as a disease it is necessary to identify those individuals prone to it or those with predictive data that they will develop hypertension. Distribution of blood pressure levels during the pediatric age is one of the first studies that should be done to identify those children with high blood pressure or those who will develop the disease. After reviewing the literature we found that there is disagreement on the criteria of normal and upper normal limits of blood pressure during childhood and adolescence. This explains the variability of the reports on incidence or prevalence of hypertension during this age. There are other factors of variability as methodology on blood pressure registration, geographic and time changes. Most authors agree that blood pressure levels are higher as age increases and boys have higher blood pressure level than girls. Our findings are within the those reported by others.

[Systolic intervals in disorders of intraventricular conduction].

The duration of the systolic intervals in 50 patients was studied. They were divided into the following groups of 10 each: 1) with normal intraventricular conduction; 2) with advanced right bundle branch block; 3) with advanced left bundle branch block; 4) with left anterior fascicular block, and 5) endocardial pacemaker in the right ventricle. The systolic intervals do not suffer modification in the presence of a right bundle branch block or a left anterior fasicular block. The left bundle branch block produces changes fundamentally in the preejection period at the expense of its two components. There also exists elevation of the systolic quotient. If the results obtained in various studies in the same patient are compared, it is possible to follow the evolution of ventricular alteration, but it is not valid, to make a comparison with the ideal values obtained in subjects with normal conduction. For those patients with a pacemaker in the right ventricular endocardium, the same criteria are applied as for the left bundle branch block. The shortening of the ejection period suggests a decrease in the volume of ventricular ejection.
