Ten patients with primary hyperaldosteronism resulting from a functioning adrenocortical adenoma were studied. The high incidence of hypertension and/or renal disease in the families of these patients suggests that hereditary factors play a part in the genesis of aldosteronomas. In patients with hypertension and hypokalemia the diagnosis of primary hyperaldosteronism is established by measurements of serum renin activity and serum aldosterone concentrations under normal conditions and during pharmacodynamic tests. The adenoma is demonstrated by radioactive iodocholesterol scintigraphy and computerized tomography. In nearly two-thirds of the patients, unilateral adrenalectomy is followed by complete definitive recovery.
Renal uptake of 99m Tc DMSA was used to evaluate the renal function of 16 healthy subjects (controls) and 115 patients with various urinary tract diseases. Scintigraphic examination was carried out 6 hours after an intravenous injection of the product. In the 16 controls Tc DMSA uptake was 25.7 +/- 2.48% in the right kidney and 24.4 +/- 2.86% in the left kidney. In 36 patients with one single hypertrophied kidney, there was a correlation (r = 0.850) between creatinine clearance and Tc DMSA uptake, which was higher than in normal subjects (39.23 +/- 9.9%). In the group of 68 patients with unilateral (31) or bilateral (37) renal disease, a significant correlation (r = 0,725) was observed between kidney-to-kidney ratios of urea clearance and Tc DMSA uptake, so that renal impairment could be quantified. Quantitative scintigraphy did not appear to be of assistance in the remaining 11 patients with obstructive uropathy, as it overestimated renal function. The results obtained with 99 m Tc DMSA scintigraphy should be helpful in choosing between nephrectomy and conservative surgery and in assessing the degree of compensatory hypertrophy in single kidneys.
The predictive values of blood-flow monitoring using the Doppler effect during operation, to detect anatomical causes of correctable low blood flow or early thrombosis in the absence of correction, was studied in 309 cases of revascularization. When employed under precise conditions, this technique was found to be of high predictive value. Flow rates can be classified in four risk zones: very high, high, low, or nil. In practice, whenever values fall below a critical threshold of 130 ml/min at the iliofemoral level or 55 ml/min at the femoropopliteal level, arteriography should be conducted to establish the cause of the reduced flow, and to enable immediate correction of any technical fault, or improvement in distal flow. The value of this method is shown by the fact that of 44 cases with reduced flow, 22 could not be improved, and early thrombosis occurred in 70% while, in 20 cases, improvement was obtained, and early thrombosis occurred in only 10%. The use of blood-flow monitoring techniques during operation can be assumed to have reduced the incidence of early thrombosis by 27% in those cases with diminished flow, and by 4% in the overall group.
Microsurgical transversal section and repair of canine kidney was presented as a model to study the condition required for an atraumatic nephrotomy. The investigation was carried out by two methods. In a first series five kidneys were injected by coloured resins after transection and microsurgical repair. Results were evaluated by cast examination of vascular net work. In a second series, transection and repair were made by an "ex-situ" procedure on seven dogs. The repaired kidney was autotransplanted in the opposite fossa iliaca. Long term renal function was evaluated after controlateral nephrectomy, by arteriography, biologic data and DMSA Tc 99 m scintiscan. The good results showed that microsurgery is useful in atraumatic nephrotomy and now a new tool in intrarenal access.

A new whole-body unidirectional moving scanner was built using a single large bar crystal, 12 photomultipliers on its top, and a delay line system to compute the X coordinate. Motion is required in only one direction on the Y-axis, and a special collimation is adapted in order to equal the X and Y performances of the detector. The performances of this detector (C.G.R. Scanicamera) (SC) were compared under the same conditions with those of a moving rectilinear scanner (R.S.) (Ohio Nuclear F 84 dual-head scanner, 12.5 cm crystal) and with an Anger gammacamera equipped with its whole body attachment (W.B.C.) Nuclear Chicago Pho Gamma III H.P.). The general impression is that this new whole-body unidirectional moving scanner is a very convenient device for whole body studies and can also be used for large scintigraphies.

Study of 52 cases of differentiated carcinoma of the thyroid treated by anatomically total thyroidectomy with block dissection of the cervical nodes indicated three amin findings: examination by puncture for cytological examination would have missed 8 micro-carcinomas; unilateral excistion would have bee inadequate in 12 patients; lymph node excision confined to detectable lymphadenopathies would have left neoplastic tissue in place in 8 cases. In the light of these advantages from an oncological standpoint, the complications of total thyroidectomy with bilateral block dissection are few and can be avoided in most cases.

The performance characteristics of 5 scintillation cameras were compared: Searle 19, Searle 37, Ohio Nuclear, Picker 2C and Toshiba GCA 202. Inherent characteristics including spatial resolution, field uniformity and distortion, energy resolution, count-rate capability and paralyzing time were measured. Extrinsic characteristics such as spatial resolution with emission tests and sensitivity with high resolution, low energy collimators also were measured. The Ohio Nuclear camera gave the best results for most of the parameters studied, followed by Searle 37 PMT, Searle 19 PMT, Picker, and Toshiba. The size of the crystal must also be a criterion for choosing a camera.

With pneumoencephalography and pressure perfusions tests, the gamma-cisternography is a method of choice for establishing a reliable diagnosis of Normal Pressure Hydrocephalus (NPH) which concept was introduced by HAKIM. The gammacisternography allows to establish the diagnosis of NPH when a persistent ventricular filling is present and to find, very often, the etiological factor by showing the absence of radioactivity over the subarachnoïd pathways along the convexities at any stage or delayed and abnormal CSF absorption. The gammacisternography permits also to explore with 111 In DTPA the high risk patients who develop gradually over the course of months and years a NPH after a head trauma, a meningitis, a subarachnoïd hemorrhage or a neuro-surgery treatment. The NPH is present only 12 to 15% of our dementia cases with gait disturbance. Nevertheless, the search for such cases remains worthwhile as the patient may be able to return to a useful life after shunting procedure. The indication for shunting is not possible ot assess on scintigraphic results alone. It must be kept in mind that the abnormalities of CSF flow may be transient, may progress or regress. The shunting may be decided if the scintigraphic pattern remains unchanged and if the clinical symptoms are going worse. The pronostic is better in statistics after ventricular shunting when the NPH scintigraphic pattern is present. The valvo-scintigraphy can also evaluate the function of a shunt during the post-operative period.
