Replacement of salt by a novel potassium- and magnesium-enriched salt alternative improves the cardiovascular effects of ramipril.

1. The influence of salt (sodium chloride; NaCl) (an additional 6% in the diet) and that of a novel sodium-reduced, potassium-, magnesium-, and L-lysine-enriched salt alternative on the cardiovascular effects of ramipril was studied in stroke-prone spontaneously hypertensive rats in a 6-week study. The intake of sodium chloride was adjusted to the same level by adding the salt alternative at a 1.75 times higher amount than regular salt. 2. Salt produced a marked rise in blood pressure and induced cardiac hypertrophy and significant mortality, while the salt alternative neither increased blood pressure nor caused any mortality and produced less cardiac hypertrophy than salt. 3. Ramipril treatment at a daily dose of 3 mg kg-1 normalized blood pressure and prevented the development of cardiac hypertrophy of rats on control diet. These effects of ramipril were blocked by the addition of salt but were only slightly attenuated by the addition of the salt alternative. The mortality in the salt group was prevented by ramipril. 4. Responses of mesenteric arterial rings in vitro were examined at the end of the study. Salt, but not the salt alternative, increased vascular contractile responses to noradrenaline. Ramipril treatment improved the arterial relaxation responses to acetylcholine and to sodium nitroprusside. The vascular relaxation enhancing effect of ramipril was blocked by salt but only slightly attenuated by the salt alternative. 5. Ramipril treatment did not significantly increase plasma renin activity in the presence or in the absence of salt supplementation. The salt alternative did not cause hyperkalaemia, either alone or in combination with ramipril treatment. 6. Both salt supplementations, irrespective of ramipril treatment, induced a six to eight fold increase in the urinary excretion of calcium. There was an expected 90 to 140% rise in the urinary excretion of magnesium and 200% rise in the urinary excretion of potassium in the salt alternative group. Salt also produced an approximately 50% increase in magnesuria.7. Our findings suggest that replacement of salt by the potassium-, magnesium- and L-lysine-enriched salt alternative improves the cardiovascular effects of ramipril. In the present study the beneficial effect was related to the increased intakes of potassium and/or magnesium and L-lysine from the salt alternative because the amount of sodium chloride was the same.

Gradual and rapid weight loss: effects on nutrition and performance in male athletes.

We studied seven male wrestlers and three judo athletes (weight 55-93 kg) during two weight reductions. In the "gradual" procedure (GP), a 5.0 +/- 0.4% (mean +/- SEM) weight loss was achieved in 3 weeks by energy restriction. In the "rapid" procedure (RP), 6.0 +/- 0.6% of body weight was lost in 2.4 days by fluid and diet restriction and forced sweating, and followed by a 5-h "loading" (food and drinks ad libitum). The net weight loss after GP and loading was 2.7 +/- 0.5%. Protein intakes (4-d food records) during GP and RP were 71 +/- 16 and RP 56 +/- 17 g.d-1, respectively. Carbohydrate intakes were 239 +/- 56 (GP) and 182 +/- 55 g.d-1 (RP). During GP and RP, mean thiamin, magnesium, and zinc intakes were at or below the respective recommendation. Thiamin, riboflavin, potassium, iron, and zinc status, assessed from blood chemistry, remained stable during both procedures. Changes in vitamin B6 indicator (E-ASTAC) and S-magnesium concentration were different (P < 0.01) between the procedures, suggesting negative trends during GP. Sprint (30-m run) and anaerobic (1-min Wingate test) performance was similar throughout the study. Following GP, vertical jump height with extra load increased by 6-8% (P < 0.01). Jumping results were not affected by RP. Hence, < or = 5% loss in body weight by either method did not impair experienced athletes' performance.

Beneficial effects of a potassium- and magnesium-enriched salt alternative.

The effects on blood pressure and the development of cardiac hypertrophy of sodium chloride (regular salt) and a novel potassium-, magnesium-, and l-lysine-enriched salt alternative, which in a previous study prolonged the life span of hypertensive rats nearly threefold as compared with the animals receiving regular salt, were compared both in spontaneously hypertensive rats and their hypertension-resistant genetic controls. In particular, the possible protective effect of increased intakes of potassium, magnesium, and l-lysine during a high intake of sodium chloride was examined. Therefore, the salt alternative was added at 1.75 times higher levels to produce the same dietary levels of sodium chloride in the regular salt and the salt alternative groups. Regular salt produced a remarkable left ventricular hypertrophy in both rat strains, but as compared with the respective control groups, it induced an increase of blood pressure only in the spontaneously hypertensive rats. The salt alternative did not induce a rise in blood pressure in either of the rat strains, nor did it produce left ventricular hypertrophy in the hypertension-resistant rats and, in the spontaneously hypertensive animals, significantly less hypertrophy than regular salt. The salt alternative appeared to prevent the sodium chloride-induced volume load since plasma levels of atrial natriuretic peptide were increased in the regular salt groups but remained normal in the salt alternative groups. Therefore, potassium, magnesium, and/or l-lysine of the salt alternative produced a powerful protection against the harmful effects of sodium chloride.

Human urinary biogenic amines and some physiological responses during situation stress.

Using a shooting competition as a model, the effect of situation stress on self-assessed mental tension, urinary metanephrine, normetanephrine, 3-methoxy-4-hydroxymandelic acid, 4-hydroxy-3-methoxyphenyl acetic acid and 5-hydroxyindole acetic acid (5-HIAA), as well as heart rate (HR), systolic and diastolic blood pressure (SBP, DBP), skin conductance level (SCL) and electromyographic (EMG) parameters were studied in 7 male and 2 female champion rifle and (rapid-fire and air-)pistol shooters. The sample collection and recordings were carried out first in the training (baseline) circumstances and then during the competition. Self-assessed mental tension was higher than suitable during the competition but good performance was the expectation. During the competition, HR, SBP, DBP, SCL and EMG were increased significantly as compared with the baseline levels. No significant correlations between the variables and between the biogenic amine metabolite excretions were observed. The competition-induced changes in self-assessed mental tension correlated positively with the corresponding changes in SCL (r = 0.63), but negatively with changes in SBP (r = -0.74). The HR of male rapid-fire pistol shooters was higher than that of male rifle shooters (P less than 0.05). All the urinary biogenic amine metabolite excretion levels were increased significantly as compared with the baseline values. Higher metabolite excretion was observed in rifle shooters than in rapid-fire shooters (P less than 0.05). The correlation between catecholamine metabolites was significant (r = 0.67-0.93), but surprisingly the correlation between 5-HIAA and catecholamine metabolites was highly significant (r = 0.80-0.91). This study indicates a marked divergence in the behaviour of various stress-responsive variables during shooting competitions.(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of interferon-gamma on biogenic amine metabolism, electroencephalographic recordings, and transient potentials.

For assessment of the underlying central mechanisms of interferon (IFN), measurements have been made of the neurotoxicity, cerebrospinal fluid (CSF) 5-hydroxyindoleacetic acid (5-HIAA) and homovanillic acid (HVA) concentrations, along with electroencephalographic (EEG) recordings and visual evoked potentials (VEP) during recombinant IFN-gamma therapy. Interferon-gamma was administered intravenously to 14 patients with non-small-cell lung cancer as 2 mg/m2 (48 X 10(6) IU/m2) three times weekly with 4 h infusion for 12 weeks. The cerebrospinal fluid (CSF) samples were taken by means of lumbar puncture prior to treatment and during the end of the second week of IFN-gamma infusions. During the course of treatment, CSF 5-HIAA diminished from 129.6 +/- 10.9 to 84.5 +/- 10.4 nmol/L (p less than 0.001), while no significant changes were observed in HVA, EEG, and VEPs. These results suggest that IFN-gamma has an effect on the central serotonergic neurotransmission.

Indole levels in human lumbar and ventricular cerebrospinal fluid and the effect of L-tryptophan administration.

Levels of tryptophan (TRP), 5-hydroxytryptophan (5-HTP), 5-hydroxytryptamine (5-HT), and 5-hydroxyindoleacetic acid (5-HIAA) in human lumbar and ventricular cerebrospinal fluid (CSF) were measured by reversed phase liquid chromatography (HPLC) with electrochemical detection. The levels of TRP ranged from 1593 to 4865 nmol/l in ventricular (VF) and from 1257 to 2557 nmol/l in lumbar CSF. The level of 5-HTP varied from 1.1 to 68.9 nmol/l in VF and from 5.3 to 10.8 nmol/l in lumbar CSF; no previous reports of 5-HTP levels in CSF exist. The serotonin level was 1.9-27.3 nmol/l in VF and 5.7-12.0 nmol/l in lumbar CSF. The levels of 5-HIAA were considerably higher in VF than in lumbar fluid with respective means of 498 +/- 52.4 nmol/l and 112 +/- 15.6 nmol/l (P less than 0.001). An oral dose of 2 g L-tryptophan significantly increased all indole levels except that of 5-HT, both in patients with progressive myoclonus epilepsy and in controls.

Psychotherapeutically oriented treatment of schizophrenia: results of 5-year-follow-up.

The authors have developed a psychotherapeutic approach to schizophrenia within the community psychiatric health care system. The orientation is psychodynamic but global in the meaning that the therapeutic plans are made on the basis of case-specific need of the patients and of their families and include both individual psychotherapy, family therapy and therapeutic communities. The material is composed of 100 successive patients of the schizophrenia group aged 16 - 44 years and for the first time admitted for treatment into the different units of the Turku Mental Health District in 1976 - 77. A multiprofessional staff was acting as therapists, supported by a continuous and intensive on-the-job -training and supervision. The results preliminary reported in this paper are based on a follow-up study 5 years after admission to treatment. 56 patients could be included in the psychotherapy group given at least some mode of psychotherapy in an adequate amount. The background variables of patients selected for various modes of therapy as well as the prognostic results are studied with the aid of logistic regression analyses. The findings indicate that the development of a global psychotherapeutic orientation is possible in the field of community psychiatry and yields results that demonstrate its meaningfulness. The implementation of psychotherapy had clearly positive explanatory connections with a favourable prognosis, and although these connections were not equally strong as the connections of some central clinical and psychosocial variables, they were obvious and manifold.

Biochemical and immunohistochemical determination of 5-hydroxytryptamine located in mast cells in the trigeminal ganglion of the rat and guinea pig.

The presence of 5-hydroxytryptamine (5-HT), as well as its precursor (5-HTP) and metabolite (5-HIAA), were biochemically determinated in the trigeminal ganglion of the guinea pig and rat. The distribution of 5-HT in the ganglion and in its posterior root was studied using both indirect immunofluorescence and the peroxidase-antiperoxidase method. In order to increase the possible 5-HT content of primary sensory neurons for subsequent immunohistochemical visualization, animals were first treated with nialamide, an inhibitor of monoamine oxidase, and then loaded with L-tryptophan. Another group of animals received colchicine to inhibit intra-axonal transport of transmitter substances. However, even combined use of loading and colchicine treatment did not reveal 5-HT immunoreactivity in ganglion cells. The only source of 5-HT immunoreactivity in the trigeminal ganglion and its posterior root was mast cells. These cells were located around the ganglion in adjacent leptomeningeal and connective tissues, as well as between the ganglion cells and nerve fibers. Only occasionally were mast cells found in the posterior root of the ganglion.

Treatment prevalence in psychiatric outpatient care in Finland. A comparative study of two areas.

Psychiatric outpatients are surveyed in two separate areas, one (the city of Turku) with clearly above-average provision of mental-health services and the other (predominantly rural Eastern Satakunta) with services corresponding to the average for the country as a whole. The results show that in Turku 3.25% and in Eastern Satakunta 1.98% of the population aged 15 years or more had visited an outpatient treatment unit during the 4-month observation period. Compared with Eastern Satakunta, the patients in turku were relatively more often young persons, men (nearly as often as women), persons in the higher social classes and patients with psychiatric disorders other then psychosis. Treatment received by the patients in Turku was more intensive and included more psychotherapeutic elements. Particularly family-type and group-type therapies were more common in Turku than in Eastern satakunta. The results are considered from the viewpoint for both epidermiologic research and health care policy.

Risk of acute coronary events and serum concentration of asymmetrical dimethylarginine.

Asymmetrical dimethylarginine (ADMA) is an endogenous nitric oxide synthase inhibitor, which has been suggested to be a novel independent risk factor for endothelial dysfunction and coronary heart disease. We investigated the association of ADMA concentration in serum with risk of acute coronary events. We did a prospective, nested, case-control study in middle-aged men from eastern Finland. In an analysis of men who did not smoke, those who were in the highest quartile for ADMA (>0.62 micromol/L) had a 3.9-fold (95% CI 1.25-12.3, p=0.02) increase in risk of acute coronary events compared with the other quartiles. Our findings suggest that ADMA is a predictor of acute coronary events.

Treatment with antioxidant and other nutrients in combination with chemotherapy and irradiation in patients with small-cell lung cancer.

Eighteen non-randomized patients with small cell lung cancer (4 women and 14 men, mean age 60.4, SD 7.8 years) received in addition to conservation small cell lung cancer treatment antioxidant treatment with vitamins, trace elements and fatty acids. All patients were out-patients who, except for one were also treated with chemotherapy and/or irradiation at regular intervals at a university of central hospital. Five patients (28%) were in an advanced stage of the disease. At the end of the follow-up period (31.7.90) the median survival time for the whole group was 505 days. Fourteen (77%) of the patients survived for more than 12 months and six patients (33%) for more than two years. One patient (5%) survived more than five years. Eight patients (44%) were still alive with a mean survival time of 32 months at the end of the study. Ten patients succumbed earlier from progression of the disease. Antioxidant treatment, in combination with chemotherapy and irradiation, prolonged the survival time of patients with small cell lung cancer compared to most published combination treatment regimens alone. We also noticed that the patients receiving antioxidants were able to tolerate chemotherapy and radiation treatment well. Surviving patients started antioxidant treatment in general earlier than those who succumbed.

Developing a global psychotherapeutic approach to schizophrenia: results of a five-year follow-up.

This is an account of a long-range action research project to determine indications for and effects of a comprehensive psychotherapeutic approach, including various treatment modalities, in the treatment of schizophrenics. Four diagnostic groups were established among the 100 patients. In the course of data analysis, the group of typical schizophrenics (56 percent) was contrasted to or compared with the entire series. A further diagnostic differentiation was established according to ego functioning; i.e., imminent, acute, regressive, and paranoid ego disintegrations, respectively. Patients and family members were interviewed upon admission, and again two and five years later, and the data recorded on a 163-item form from which 40 clinical and psychosocial variables were constructed after the baseline examinations. In addition to cross-tabulation, logistic regression analysis was employed. The conclusion that the follow-up study supports the effectiveness of our global psychotherapeutic approach to treating schizophrenia seems justified. Results so far indicate that five modes of therapy in addition to drug treatments are optimal for different patients. The five modes are long-term individual psychotherapy, couple or conjoint family therapy for married patients, family therapy with the family of origin, flexible short-term crisis intervention with a family focus, and extensive long-term treatment focused on social rehabilitation for the most ill-starred patients.
