Renal vein renin ratio. Predictive value in renal and renovascular hypertension.

Renal vein renin ratios were determined in hypertensive patients, 19 with renal artery stenosis, 7 with renal artery occlusion and 13 with unilateral renal hypoplasia. Twenty-one of these patients were treated surgically. Renal vein renin ratios of greater than or equal to 1.5 were associated with high frequency of operative success, but ratios less than 1.5 by no means excluded good surgical results.

Influence of warm and cold ischemia time on initial function and one-year survival of renal allografts.

The influence of the duration of warm and cold ischemia on initial graft function on patient survival, graft survival, and graft function one year after transplantation was assessed in 234 consecutive cadaveric renal transplants. The number of grafts with delayed function was found to increase with increasing duration of the warm and cold ischemia times. On the other hand, the duration of the ischemia times and consequently the initial function, whether immediate or delayed, did not influence the survival of graft and patient nor the function of the graft one year after transplantation.

Long term survival and graft function after cadaver kidney transplantation.

During the period 1968-75 310 cadaver kidney transplantations were carried out; 261 first-time, 45 second-time and 4 third-time transplantations. Graft survival after 5 years was about 30% for both the first and the second transplant. Patient survival after 5 years was about 50%. No relationship could be found between match grade and graft survival whereas the patient survival was definitely better in the younger age groups. The major cause of graft loss was rejection, vascular and ureteral complications being less frequent. The main cause of lethal outcome was complications to the immunosuppressive therapy and to a lesser degree cardiovascular events and malignant disorders. Cadaver kidney transplantation is an acceptable treatment of chronical renal failure particularly in younger persons and in cases where dialysis treatment is difficult.

Long-term treatment of uraemic osteodystrophy with 1-alpha-hydroxycholecalciferol.

Three adolescents on regular haemodialysis were treated for more than one year with 1alphaOHD3 in doses of 0.25 - 10 mug/day orally. All the patients had hypocalcaemia, a high serum level of alkaline phosphatase and signs of uraemic osteodystrophy on X-ray. Clinical signs of bone disorder were present in two cases. During the treatment serum calcium and alkaline phosphatase normalised, and there was a marked improvement in the X-ray changes. Clinical improvement also took palce but not to the same extent as indicated by the laboratory data. It seems therefore important that treatment should start before severe bone lesions are present. Careful control of the treatment is necessary to avoid hypercalcaemia.

Delayed absorption of lithium in intoxication: a case history.

A 55-year old man with lithium intoxication showed increasing serum concentrations in spite of forced diuresis and dialysis. A high lithium content was found in gastric juice three days after the compound had been taken. The serum lithium level began to fall after gastric lavage.

Kidney transplantation and donor age.

The cumulative graft survival after 355 cadaver kidney transplantations was analyzed with special reference to the age of the donors. A significant decrease in cumulative graft survival with increasing donor age was found.
