Interactions of metal ions with two quinolone antimicrobial agents (cinoxacin and ciprofloxacin). Spectroscopic and X-ray structural characterization. Antibacterial studies.

Several novel metal-quinolone compounds have been synthesized and characterized by analytical, spectroscopic and X-ray diffraction methods. The crystal structure of the four compounds, Na(2)[(Cd(Cx)3)(Cd(Cx)3(H2O))].12H2O, [Co(Cp)2(H2O)2].9H2O, [Zn(Cp)2(H2O)2].8H2O and [Cd(HCp)2(Cl)2].4H2O, is presented and discussed: HCx=1-ethyl-1,4-dihydro-4-oxo(1,3)-dioxolo(4,5-g)cinnoline-3-carboxylic acid and HCp=1-cyclopropyl-6-fluoro-1,4-dihydro-4-oxo-7-(1-piperazinyl)-3-quinoline carboxylic acid. In all these compounds the quinolone acts as a bidentate chelate ligand that binds through one carboxylate oxygen atom and the exocyclic carbonyl oxygen atom. Complexes of ciprofloxacin were screened for their activity against several bacteria, showing activity similar to that of the ligand. In addition, the number of bacteria killed after 3 h of incubation with the ligand, [Co(Cp)2(H2O)2].9H2O, Ni(Cp)2.10H2O and Cu(Cp)2.6H2O, was determined against S. aureus ATCC25923. There is a direct relationship between the growth rate and the lethal rate. Against growing bacteria, the ligand is the most bactericidal and Cu(Cp)2.6H2O is the less bactericidal. On the contrary, against non-dividing bacteria, the complexes were more bactericidal than the ligand, with Cu(Cp)(2).6H(2)O the most bactericidal compound.

Dopamine interaction in the absence and in the presence of Cu2+ ions with macrocyclic and macrobicyclic polyamines containing pyrazole units. Crystal structures of [Cu2(L1)(H2O)2](ClO4)4 and [Cu2(H-1L3)](ClO4)3*2H2O.

The interaction with Cu2+ and dopamine of three polyazacyclophanes containing pyrazole fragments as spacers is described. Formation of mixed complexes Cu2+-macrocycle-dopamine has been studied by potentiometric methods in aqueous solution. The crystal structures of the complexes [Cu2(L1)(H2O)2](ClO4)4*2H2O (4) (L1 = 13,26-dibenzyl-3,6,9,12,13,16,19,22,25,26-decaazatricyclo[22.2.1.1(11,14)]octacosa-1(27),11,14(28),24-tetraene) and [Cu2(H-1L3)](HClO4)(ClO4)2*2H2O (6) (L3 = 1,4,7,8,11,14,17,20,21,24,29,32,33,36-tetradecaazapentacyclo[12.12.12.1(6,9).1(19,22).1(31,34)]hentetraconta-6,9(41),19(40),21,31,34(39)-hexaene) are presented. In the first one (4), each Cu2+ coordination site is made up by the three nitrogens of the polyamine bridge, a sp2 pyrazole nitrogen and one water molecule that occupies the axial position of a square pyramid. The distance between the copper ions is 6.788(2) A. In the crystal structure of 6, the coordination geometry around each Cu2+ is square pyramidal with its base being formed by two secondary nitrogens of the bridge and two nitrogen atoms of two different pyrazolate units which act as exobidentate ligands. The axial positions are occupied by the bridgehead nitrogen atoms; the elongation is more pronounced in one of the two sites [Cu(1)-N(1), 2.29(2) A; Cu(2)-N(6), 2.40(1) A]. The Cu-N distances involving the deprotonated pyrazole moieties are significantly shorter than those of the secondary nitrogens. The Cu(1)...Cu(2) distance is 3.960(3) A. The pyrazole in the noncoordinating bridge does not deprotonate and lies to one side of the macrocyclic cavity. One of the aliphatic nitrogens of this bridge is protonated and hydrogen bonded to a water molecule, which is further connected to the sp2 nitrogen of the pyrazole moiety through a hydrogen bond. The solution studies reveal a ready deprotonation of the pyrazole units induced by coordination to Cu2+. In the case of L2 (L2 = 3,6,9,12,13,16,19,22,25,26-decaazatricyclo[22.2.1.1(11,14)]octacosa-1(27),11,14(28),24-tetraene), deprotonation of both pyrazole subunits is already observed at pH ca. 4 for 2:1 Cu2+:L2 molar ratios. All three free receptors interact with dopamine in aqueous solution. L3 is a receptor particularly interesting with respect to the values of the interaction constants over five logarithmic units at neutral pH, which might suggest an encapsulation of dopamine in the macrocyclic cage. All three receptors form mixed complexes Cu2+-L-dopamine. The affinity for the formation of ternary dopamine complexes is particularly high in the case of the binuclear Cu2+ complexes of the 1-benzyl derivative L1.

Interactions of metal ions with a 2,4-diaminopyrimidine derivative (trimethoprim). Antibacterial studies.

The interaction of copper (II), zinc(II) and cadmium(II) with Trimethoprim (2,4-diamino-5-(3',4',5'-trimethoxybenzyl) pyrimidine) has been studied. The crystal structures of [Zn(Trim)2Cl2] (2) and [Cd(Trim)Cl2(CH3OH)]n (4) are reported. Compound (2) exhibits a distorted tetrahedral environment around the metal center and crystallizes in the triclinic space group P1 with a=10.2397(6), b=10.4500(6), c=16.3336(16) A, alpha=96.141(8), beta=106.085(5), gamma=96.551(5) degrees and Z=2. In complex (4), the Cd(II) centers are bridged sequentially by two chlorine ions to form infinite chains and present a six-coordinated environment; the compound crystallizes in the monoclinic P2(1)/C space group with a=13.958(5), b=7.532(2), c=18.390(2) A, alpha=90, beta=97.32(5), gamma=90 degrees and Z=4. In both structures the Trimethoprim acts as a monodentate ligand through the pyrimidinic nitrogen N(1) atom. The characterization of the Cu(Trim)2(CH3O)(ClO4) complex through EPR and magnetic measurements suggests a binuclear or polinuclear nature, with bridging methoxo groups. The complexes were screened for their activity against several bacteria, showing activity similar to that of trimethoprim.

[Fungal sinusitis. Report of four cases].

Fungal sinusitis has a nonspecific clinical presentation, is refractory to standard medical treatment and may produce expansion and erosion of the sinus wall. Various factors have been implicated in the development of fungal sinusitis: anatomical factors in the osteomeatal complex, tissular hypoxia, traumatic factors, massive exposure to fungal spores, allergy and immunosuppression. Our study of four cases diagnosed in the department included the clinical, pathological, and radiological features of fungal sinusitis. Five forms are described. Bacterial infection often occurs. The diagnosis of fungal sinusitis requires a high level of clinical and pathological suspicion. Before surgery, fungal sinusitis is suggested by the clinical and radiological features, and is confirmed by histological study. Culture of the tissue sometimes leads to identification of the fungus. Surgical removal of the infection is the main treatment for every form of fungal sinusitis.

Cobalt(II), nickel(II), and copper(II) complexes of sulfanilamide derivatives: synthesis, spectroscopic studies, and antibacterial activity. Crystal structure of [Co(sulfacetamide)2(NCS)2].

The synthesis and characterization of new coordination compounds of Co(II), Ni(II), and Cu(II) with sulfacetamide (N-[4-(amino-fenil)sulfonil]acetamide) is reported. The complex [Co(sulfacetamide)2(NCS)2] crystallizes in the triclinic space group P-1. The cell dimensions are a = 7.80(2) A, b = 8.327(9) A, c = 9.568(3) A, alpha = 90.5(1) degrees, beta = 90.5(1) degrees, gamma = 97.8(2) degrees, V = 616(1) A3, Z = 2, and Dx = 1.689 g/cm3. The final conventional R-factor = 0.039 (Rw = 0.039) for 3535 "observed" reflections and 173 variables. The Co(II) is surrounded in a regular octahedral arrangement by two Nthyocianato from the NCS, two Namino and two Oacetamido atoms from the sulfacetamide. Each sulfacetamide, acting as a bidentate ligand, chelates two Co(II) ions as a bridge through the Namino and the Oacetamido atoms. IR, Reflectance Diffuse, EPR, and magnetic properties of the obtained complexes are discussed. The complexes were screened for their activity against E. Coli and S. aureus, showing an appreciable antimicrobial activity compared with the ligands.

[Fulminant hepatic failure in children].

We report the clinical characteristics and course of 38 children with fulminant liver failure. A viral etiology was demonstrated in 19 patients out of 25 with serologic screening (virus A in 17 patients, B in 2 patients). One patient had toxic liver damage from ingestion of a caustic substance. Mean age was 4 years and 6 months. Jaundice was present in all but 2 patients at admission. Encephalopathy developed at a mean of 13 days and 17 children were admitted with stage 3 or 4 coma. Evidence of severe liver failure was present in every patient. A lower prothrombin time and higher bilirrubin concentration were shown by non survivors. Hypoglycemia developed in 37% of patients, seizures in 37%, gastrointestinal bleeding in 45%, respiratory failure in 32% and severe infection in 32%. Overall mortality rate was 42% raising to 83% in patients with stage 3 coma at admission. Thus, although the etiology of fulminant liver failure differs in children as compared to adults, mortality rate and complications are similar.

[Isolated hypoplasia of the right ventricle. Study of 3 cases].

Right ventricular hypoplasia without additional congenital heart disease is an uncommon abnormality. It is characterized by partial absence of right ventricular inflow tract, both tricuspid and pulmonary valves being normally shaped. We report three cases; a newborn, an infant and a twelve years old boy. All of them had cyanosis and right atrial enlargement and both of the younger patients had electrocardiographic signs suggesting right ventricular hypoplasia. Right atrial mean pressure were raised. A differential diagnosis should be stablished with all sorts of right ventricular hypoplasia with cyanosis and with restrictive myocardiopathyes. Surgery for symptomatic patient should be palliative during first months of life and corrective from age one year onwards.

[Severe early-onset thrombopenia induced by non-fractionated heparin: therapeutic trial with low molecular weight heparin].

A case of severe thrombocytopenia (27 x 10(3)/mm3) of early onset during unfractionated heparin (Héparine sodique Rovi) treatment is reported in a 75 year-old man suffering from a deep vein thrombosis of the right calf. It was not associated with thromboembolic or haemorrhagic complications. A serum heparin-dependent aggregating factor was not detected. Substitution of low molecular weight heparin (Choay Laboratory, CY 222, PM 4 500, 25 000 U anti-Xa/ml) for unfractionated heparin resulted in a progressive normalisation of the platelet count and improvement of the venous thrombosis.

Different functional characteristics of residual nephrons in infantile vs adult diffuse cortical necrosis.

In this report we study the functional characteristics of residual nephrons in a 37 year-old woman, 7 months after diffuse bilateral cortical necrosis (CN) of unknown etiology, and in two infants, aged 13 and 15 months, who suffered CN in early infancy after surgical shock and acute dehydration, respectively. In the three cases CN was proven histologically by renal biopsy but undamaged nephrons were only present in the juxtamedullary area in the adult patient whereas in the two infants they were located in the outer part of the cortex. At the time of the study all patients presented a similar degree of renal insufficiency (creatinine clearance: 17-23 ml/min/1.37 m2). The adult patient showed a partly conserved ability to concentrate the urine, a marked free water formation in relation to the degree of distal sodium delivery and an unimpaired capacity to acidify the urine after an acid load. Both infants, by the contrary, were unable to concentrate the urine, had lower free water formation at similar rates of distal sodium delivery and presented a clear incapacity to acidify the urine. These results confirm previous finding indicating the sparing of juxtamedullary nephrons after CN in the adult subject but favor the existence of a surviving population of superficial nephron when CN occurs in early infancy. These differences are probably in relation with associated damage of deep cortex and medulla infancy due to the specific characteristics of blood flow distribution present at that age.

[Postoperative spinal cord ischemia in thoracoabdominal aneurysms].

At the present article, 12 cases of paraplegia secondary to the surgical treatment of thoracoabdominal aneurysms are presented. Study includes patients admitted between 1973-1987 for treatment and rehabilitation of its medular injury. Factors of risk, surgical technics practiced, peroperative complications and type of medular injury are analyzed. The most common medular injury was an anterior medular syndrome (rather than 60%). In the same way, preventive methods are analyzed and somatosensory evoked potentials (SEP) and motor evoked potentials (MEP), both in developing phase at the present moment, are recommended as the most viable method for detecting changes during the surgical procedure.

[Thrombolytic efficacy of a Lys-plasminogen-urokinase combination: studies in experimental animals and humans].

During animal experimental phase, lis-pg combined with UK produced a thrombolysis of about a 62.5%. This effect is accompanied by an important fibrinolytic system activation, a decrease in fibrinogen levels (0.37 +/- 0.2 gr/l) and an increase PDF/Fg (120.5 +/- 30 ng/ml). Such thrombolytic stage produced diverse hemorrhagic complications in experimental animals. During human clinical trial stage, then patients with Deep Venous Thrombosis (DVT) at proximal lower limbs level were submitted to diverse treatment protocols with Lis-Plasminogen (Lis-plg) and Urokinase (UK). After preliminary outcomes we can conclude that administration of Lis-plg followed by UK increases the fibrinolytic activity but also increases the risk of hemorrhagic complications. This second effect is not probably caused by an specific absorption on the thrombo surface, but by an increase of circulating plasminogen levels Lis-plg exogenous-induced.

[Capillaroscopic observation in angiologic diagnosis].

The authors present their experience with Morphologic Capillaroscopy, and analyze the diagnostic dates provided by this technique to the current Angiology and Vascular Surgery protocols. Morphologic anomalies founded during diverse systemic diseases as well as the changes on the capillary field produced in cases of chronic ischaemia and venous insufficiency were analysed. The study was made on the basis of 236 capillaroscopic images and it was completed by an extensive bibliographic review of the subject.

[Plasminogen depletion and incorporation in experimental venous thrombosis].

The objective of this piece of work is to study (using animals): venous thrombosis, the depletion and incorporation of plasminogen in thrombus of different stages of time, in order to devise better guide lines for the treatment of thrombosis than the conventional treatment. The studies relating to the plasminogen content of the thrombus show us that, on experimental venous thrombosis, as from the fifth day after which it occurred, the content of the plasminogen thrombus is reduced considerably. This data underlines the fact some venous thrombosis of over five days are resistant to conventional thrombolytic treatment. In relation to this we have to report that the mean lifespan according to our experiments of the plasminogen on beagle dogs is approximately sixteen hours. On the other hand the study of the addition of 125Lis-plasminogen, on experimental thrombosis of different stages of oldness show us that the above addition results in a 54% intake into the thrombus on three day, old thrombosis and 17% on five day old thrombosis. In conclusion we can suggest that the resistance to conventional thrombolytic treatment of old venous thrombosis (5-7 days old) is probably due to a reduction of efficiency of the thrombus plasminogen. At the same time the contribution of exogenous lis-plasminogen does not produce a sufficient enrichment of the thrombus in plasminogen. Therefore the therapeutic exogenous plasminogen addition is not advisable for relatively old thrombosis (less than or equal to 5 days old).
