Patient selection for assisted reproduction.

Patient selection is a critical factor in influencing results in assisted reproduction. Patients should be assessed individually in terms of all relevant fertility factors including age, duration and aetiology of infertility and psycho-social attitudes so that an approximate prognosis of their management can be made. Restrictive criteria for patient selection will enhance a programme's results but at the expense of depriving some couples of their only chance of conception. The best overall management of each individual infertile couple should always remain the main criteria for selection into a particular assisted reproduction programme.

Turner's syndrome patients lack tight junctions between uterine epithelial cells.

Eleven endometrial biopsies, taken from six Turner's syndrome patients receiving hormone replacement therapy prior to treatment by oocyte donation and embryo transfer, were assessed by freeze fracture followed by electron microscopy for epithelial tight junctions. Nine of the eleven biopsies had no discernible tight junctions; the other two biopsies had reduced and disorganized junctional structures. Two patients subsequently became pregnant following embryo transfer. It is concluded that compromised epithelial integrity does not prevent embryo implantation in the human, an observation that is consistent with a barrier role for the epithelium except at times when appropriately conditioned with oestrogen and progesterone to induce receptivity for implantation.

The effects on sexual response and mood after sterilization of women taking long-term oral contraception: results of a double-blind cross-over study.

Twenty women using oral contraception for a minimum of 2 years with no side effects volunteered to take part in this experiment which commenced after they had undergone laparoscopic sterilization. In a double-blind cross-over design, patients were administered either 1.0 mg ethynodiol diacetate + 0.1 mg mestranol ("Ovulen") each day for 21 days or an identical-looking placebo for the same number of days. The following month the alternative (cross-over) tablet was given. On days 12 and 25 of each cycle the women completed a modified form of Pitt's Depression Index and a short Sexual Response Score. The result indicated that the active pill ("Ovulen") was associated with a decreased sexual response, but with no increase in the depression index (relative to placebo). The significance of these findings is discussed in relation to previous work, the methodology of the study and hormone-behaviour experiments.

The tortuous tube: pregnancy rate following laparoscopy and hydrotubation.

256 infertile women whose ovulatory and related male fertility factors were normal were investigated by diagnostic laparoscopy and dye hydrotubation. The tubes were objectively classed into 4 groups--normal, patent with macroscopic tubal adhesions, patent with one tortuosity, and patent with multiple tortuosities. The latter group had a highly significant response to the above procedures and 66% were pregnant within 1 year and 81% within 2 years. No significant effect was found in the other 3 groups. This multiple tortuosity group represents the type of tube that significantly responds to treatment inherent in all tubal patency tests. Its pathogenesis and management are discussed.

Incorporation of a flexible plastic rod into the new Gravigard plastic inserter assembly has, in our opinion, allowed for easier insertion and positioning of the device in the uterus compared with the previously available metal rod.

Recent advances in female sterilization.

Advances in female sterilization have been made over the last five years in both its techniques and medico-social acceptance. Prior to 1970, the majority of tubal sterilizations were carried out by partial or complete surgical excision of both tubes by laparotomy in most cases, or by partial salpingectomy by an anterior or posterior colpotomy in selected cases. The traditional operations by the abdominal route comprised the Madlener method, the Pomeroy method, the Irving method and the Aldridge method. These methods will not be discussed further.

The incidence of unwanted pregnancy in Australia.

A great number of unwanted pregnancies occurs annually in Australia, but more sociological data are needed to explore this current problem. If one defines unwanted pregnancy as one which was unwanted by either partner weeks before the conception, it can be estimated that approximately two in three pregnancies are unplanned and one in two is unwanted.
