[Once cesarean section--always cesarean section?].

Between 1975 and 1985 from 522 patients, who had undergone caesarean section during their previous delivery or deliveries, per cent 63 have been delivered vaginally and 37 per cent with a caesarean section (52.8% primary, 47.2% secondary). The rate of spontaneous labour was higher, if patient had a spontaneous delivery before caesarean section or the first caesarean section has been performed because of a placenta praevia, a breech presentation or a fetal distress syndrome. Cephalopelvic disproportion went on in 67.2 per cent with a caesarean section. Rupture of the scare occurred in 2.9 per cent. Expectative management of delivery is justified following previous caesarean section. Oxytocin infusions are possible in cases if internal tocography will be done.

Long-term results following surgical removal of pulmonary metastases in children with malignomas.

Between 1972 and June 1983, 21 children (mean age 9 years, range 20 months to 16.5 years) were operated on for pulmonary metastatic disease. Primary malignant tumors were Wilms-tumor (7 patients), osteogenic sarcoma (7 patients), Ewing's sarcoma (4 patients), hepatoblastoma (2 patients), and rhabdomyosarcoma (1 patient). The surgical intervention was part of a therapeutic pediatric oncological concept with curative purpose including chemotherapy and/or radiation in different combinations. Ten out of these 21 children survived disease-free 3 years and more after pulmonary metastasectomy. An aggressive surgical approach towards pulmonary metastatic disease in children thus appears to be justified.

[Effectiveness of therapy of endometriosis with danazol and chlormadinone acetate and their effect on paraclinical parameters].

The effect of danazol in a dose of 400 mg-800 mg per day for 6 month was studied in 10 women and also chlormadinone acetate 2 X 2 mg per day. Clinical investigation and laboratory test were done before the therapy has been started, during treatment (after the 2 and the 6 month) and 2 month after the end of therapy. Two month after discontinuation of danazol 5 women were free of symptoms. The metabolic studies revealed significant changes in many biochemical parameters. Two month after discontinuation of chlormadinone acetate also 5 women were free of symptoms. The metabolic studies revealed significant changes only for the blood plateled concentration and serum bilirubin.

[Endometriosis. Classification, etiology and therapy].

The authors supply a survey about the aetiology varied classification systems and the hormontherapy of endometriosis. We have described the advantages and disadvantages of the therapy with progestagens, danazol, gestrinon and Gn RH agonists.

New species definitions in phages of gram-positive cocci.

About 290 phages of the bacterial genera Leuconostoc, Micrococcus, Staphylococcus, and Streptococcus were surveyed. The phages belong to the Myoviridae, Siphoviridae, and Podoviridae families and comprise seven basic morphotypes. Fourteen new species are proposed, bringing the number of phage species for gram-positive cocci to 27. The distribution of particular phage types may indicate phylogenetic relationships between host bacteria.

The bacteriophages of lactic acid bacteria with emphasis on genetic aspects of group N lactic streptococci.

The paper reviews the bacteriophages of the group N lactic streptococci centering on isolation, ultrastructure and morphology, phage receptors, the structure of the genome, protein components, the phenomenon of the lysogenic state, restriction-modification systems and genetic exchange by transfection and transduction. The resulting consequences on industrial fermentations are briefly discussed.
