Chemotherapy of advanced and relapsed squamous cell cancer of the head and neck with split-dose cisplatinum (DDP), 5-fluorouracil (Fura) and leucovorin (CF).

58 patients with locally advanced or relapsed squamous cell head and neck cancer were treated on 5 consecutive days with DDP 20 mg/m2/d IV-push, followed by CF 100 mg/m2/d IV-bolus and Fura 400 mg/m2/d IV-push 60 minutes later. Treatment was recycled on day 22(-29), according to toxicity. CF was added to the widely used DDP/Fura combination, because recent studies showed enhancement of Fura-activity by CF. 45/58 patients had no prior therapy and 13/58 pts were relapsed after chemotherapy and/or radiation therapy. All patients were evaluable for toxicity and response. After 3 courses of induction chemotherapy 23/45 pts in the previously untreated group had a complete response (CR); 20/45 a partial response (PR), 2/45 were restaged as no change (CR + PR: 95%). Induction chemotherapy was followed by radical surgery and postoperative radiation therapy. In the pretreated group 4/13 pts had a complete response; 6/13 a partial response; no change in 3/13 pts. Median duration of remission (MDR) has not been reached in the primarily untreated group, whereas in pretreated patients the MDR was 3.8 months (range 2.3 - 11.7 months). Hematologic and gastrointestinal toxicity was substantial but manageable and therapy was performed on an outpatient basis. The combination of DDP, CF and Fura has high activity in untreated and pretreated head and neck cancer patients. The obtained results are comparable to the widely used DDP/Fura continuous infusion regimen.

[Comparison of the results of the measurement of O2 consumption with a conventional bacteriologic method for urine obtained by bladder puncture].

A method is described which utilizes the O2 consumption of aerobes and facultative aerobes from urinary tract infections for their detection. Urines obtained by puncture and the corresponding midstream urines were examined for verification. Irrespective of secondary contamination and the presence of non-causative organisms, the O2 consumption method permits the detection of a bacterial urinary tract infection within 1 h. When this method was used, a comparison between midstream urine and urine obtained by puncture showed a complete coincidence. The O2 method has shown a sensitivity of 96.9% at a specificity of almost 100% for bacterial concentrations of more than 10(5)/ml in the urine obtained by puncture.

[Magnetic resonance tomography in tumors of the facial bones and neck area].

32 patients with lesions of the facial skeleton, the neck and the parapharyngeal space were examined with NMR (Nuclear Magnetic Response). The results are compared with CT scanning. NMR offers high soft tissue contrast. Multiplanar imaging easily distinguishes tumours and lymph nodes from the cervical vessels and muscles. In contrast CT, is more useful for the imaging of destruction of the facial skeleton and the skull base.

[Livedo racemosa].

Livedo racemosa is a very rare disease with an idiopathic and symptomatic form. The diagnosis is very difficult to make because many primary diseases are responsible. Skin reactions show no typical form and range from urticarial nodules and purpuriform papillae to ulceration. The symptomatic form of livedo racemosa causes circumscribed, asymmetric lesions restricted to one half of the body, while the idiopathic form is characterised by arborization figures and livid tendril-like discolorations. There are many suggestions given in the literature for the therapy of the primary disease and of the skin, including long term therapy with high dosed rubifacient drugs or steroids. However, livedo racemosa often shows a slow progression despite treatment.

Aerial Dispersal of Epiphytic Bacteria over Bean Plants.

Plant canopies are strong sources of bacterial aerosols during sunny days when the leaves are dry. Bacterial concentration, upward flux, and deposition onto exposed petri plates were measured over snap beans during three growing seasons. A net upward flux of bacteria occurred only during the warm part of sunny days, not at night when leaves were wet with dew or when a thermal inversion was present. Aerosol source strength was positively correlated with wind speed. Upward fluxes were higher on days after rain than on days when the soil was dry. Other unidentified sources of variability in source strength probably exist. Canopy-level deposition, apparently due to intermediate-scale transport of bacteria in fairly concentrated clouds, can occur in the early evening.

[Indications for nuclear magnetic resonance tomography in tumors of the facial skeleton and neck area].

The diagnostic value of magnetic resonance tomography is examined in relation to computed tomography on the basis of 32 patients subjected to magnetic resonance tomography who had pathological processes in the craniofacial, parapharyngeal and neck regions. The high power of contrast resolution, multiplanar imaging, and good circumscription of cervical vessels, offer advantages over computed tomography in respect of space-occupying growths in the median and lower parts of the craniofacial skull. A disadvantage is the poorer detectability of tumour-conditioned bone destruction at the base of the skull and at the facial bones; in such cases, computed tomography remains the method of choice.

[Is there a risk of noise-induced hearing loss in automobile drivers and in automobile sport racing?].

Professional racing drivers are exposed to extreme noise pollution by their high-capacity cars. For the purpose of evaluating the emissive noise of racing-cars, noise-level measurements on race-courses were undertaken. As a result noise-levels up to 130 dB (A) for the outer noises and 125 dB (A) for the inner region of the car were measured. Moreover, eighteen professional racing drivers as well as eleven employees, working in the pit, underwent audiometric investigations. Only one of the racing drivers showed noise deafness. This result can be explained by the short periods of exposure to the noise, which is 150-180 hours every year, and the regular wearing of ear-plugs. The employees working in the pits, however, showed distinct hearing impairments, explained above all, by their activities at the motor-test blocks. Incidentally this paper also deals with the noise sources of vehicles and the origin of noise deafness. There is no risk of noise deafness for this group of persons.

Plants as sources of airborne bacteria, including ice nucleation-active bacteria.

Vertical wind shear and concentration gradients of viable, airborne bacteria were used to calculate the upward flux of viable cells above bare soil and canopies of several crops. Concentrations at soil or canopy height varied from 46 colony-forming units per m over young corn and wet soil to 663 colony-forming units per m over dry soil and 6,500 colony-forming units per m over a closed wheat canopy. In simultaneous samples, concentrations of viable bacteria in the air 10 m inside an alfalfa field were fourfold higher than those over a field with dry, bare soil immediately upwind. The upward flux of viable bacteria over alfalfa was three- to fourfold greater than over dry soil. Concentrations of ice nucleation-active bacteria were higher over plants than over soil. Thus, plant canopies may constitute a major source of bacteria, including ice nucleation-active bacteria, in the air.

[Use of rapid electrochemical methods for detection of bacteriuria in kidney transplant patients].

A total of 1299 urine samples from kidney-transplant patients were tested for bacteriuria using the electrochemical rapid procedure. For germ counts of more than 10(5)/ml the registration rate was 97.6% after incubation for 1 hour and 100% after 2 h, providing the patients were not under the influence of antibiotics at the time of examination. Bacteria inhibitors in the urine delayed the reaction so that an incubation period of 2-3 hours was necessary. The electrochemical rapid procedure proved to be unsuitable in cases where small quantities of germs are expected to be discharged.

[Reproduction of the abutment clearance in casting fixed tooth replacements].

After casting one model with various procedures and materials, models were constructed of Duroc. the abutment distance of the Duroc model depended on the impression method and the impression material. In general, double-mix impressions were more accurate than corrective impressions. Corrective impressions could be improved by placing flow-off grooves in the primary impression.
