[Neoplasms during the 1st year of life: a study of 116 cases treated during the last 10 years].

116 infants under one year of age (14 per 100) with malignancies including 35 neuroblastomas, 23 retinoblastomas, 14 Wilm's tumours, 10 hepatoblastomas, 10 brain tumours, 9 germ cell tumours, 8 histiocytic and 7 soft tissue sarcomas, were treated in the last ten years. Hepatoblastoma is the highest relative incidence tumor in the first year of life and with brain tumours has the worst prognosis (50 and 40 per 100, respectively). The disease-free survival rate is most than 80 per 100 in neuro and nephroblastoma with medical and surgical treatment. Familiar incidence in Wilm's tumor is 42 per 100 and chemotherapy side effects are 23 per 100. Familiar incidence in neuroblastoma is 20 per 100 and chemotherapy side effects 21 per 100. In conclusion, in children with malignancies under one year of age have good prognosis (75 per 100 survival at five years), with lower surgical complications rate. Early diagnosis and response to chemotherapy, the side effects of which are considerable and should be strictly controlled, are key factors in the better prognosis and increased life expectancy in this group of patients.

[Should the medico-surgical strategy for biliary atresia be changed in view of liver transplantation as a reality?].

Biliary atresia (BA) is the main source of candidates for paediatric liver transplantation (PLTx) with 64 per 100 of survival. The follow-up of 35 immediate candidates of BA, 19 of them transplanted, induced to the authors to change some details on the BA treatment. This disorder does not finish already with portoenterostomy. Eventually mortality causes are infection and chronic rejection. The functional loss of jejunal segment in the Roux en Y may explain the poor cyclosporin absorption and therefore chronic rejection. By other hand, chronic malnutrition presented by these patients before PLTx may favour infection. Authors recommend more aggressive medical treatment before PLTx and technical improvement in the portoenterostomy with the aim of previous surgery does not difficult posterior PLTx.

[Meconial peritonitis: conclusions based on 53 cases].

The study of 53 patients with neonatal meconium peritonitis (MP) over a twenty-two year-period, the largest surgically-treated series at a single centre, is presented. Three different types of MP were established: Generalized, Localized and Cystic. Prognosis and surgical techniques to be used were determined by each MP type. The following survival rates achieved: for Generalized MP, 70% with "one stage" and 89% with "two stage"; for Localized MP, 80% with "one stage" and 100% with "two stage" and for Cystic MP, 57% with "two stage". The overall survival rate was 77%, rising to 85% in the last fifteen years, probably due to the advent of parenteral nutrition and advances in neonatal intensive care.

[Ovarian tumors in childhood].

Authors report their experience with 33 pediatric patients with ovarian tumors seen in the last 17 years. Total mortality was 12%. The condition was beginning in 76% of cases, most of them cystic tumors, with a 100% survival rate. Only 4 patients, with malignant tumors of different histologic nature, died. Clinical features, diagnostic procedures, therapeutic approaches and outcome are analyzed. The fact that the older the patient higher the probability of harboring a malignant tumor is stressed.
