[First infantile liver transplants in Spain].

For the first time in Spain, authors report the experience, started by the Hospital Infantil "Valle de Hebron" of Barcelona on pediatric liver transplant in terminal liver disease in childhood which means a new opportunity for these patients otherwise facing a fatal out come in a short time. We show the 4 first pediatric liver transplants performed in our country and we point out some of the most important factors of a pediatric liver transplant program in a consolidation phase.

Nifedipine in the treatment of renal colic.

A double-blind randomized, clinical trial was conducted in 43 patients to evaluate the efficacy of sublingual nifedipine in the treatment of pain in renal colic. Comparison was made with a combination of dipyrone, pitofenone and fenpipramide, which is usually administered intravenously in Spain. With nifedipine, pain completely disappeared in 44% of the cases, and with the combination of drugs, in 89% (p less than 0.005). More adverse effects were noted with the combination than with nifedipine. Both treatments brought about a slight drop in arterial pressure without leading to hypotension in any case. There was no change in heart rate. The possible causes of the relatively low level of success with nifedipine were analyzed. The possibility of using nifedipine under certain circumstances in which other drugs are contraindicated or when their administration is unfeasible is suggested.

[Gastric chemistry in the pathology of the cardiohiatal sector].

Study of gastric chemistry in patients with cardiohiatal anomalies is an efficient test for distinguishing those cases that can evolution with complications and thus require surgical intervention. From a total of 720 patients affected with this pathology and observed between 1976 and 1982, a group of 164 were submitted to this study, 72 resulted with gastric acid rates higher than normal. 55% of these patients were operated. It is an easy test to realize in any service of Pediatric Surgery.

[Treatment of type-b H. influenzae meningitis resistant to ampicillin and chloramphenicol].

Three cases of H. influenzae type b meningitis with resistance to ampicillin and cloramphenicol are described. All infants were treated with new cephalosporin derivates. CSF sterilization was achieved in all patients. Clinical evolution was good in the two old infants treated with cefotaxime, nevertheless younger infant treated with cephoxitin developed serious neurologic and psychologic sequelae. H. influenzae b was erradicated of a carrier mother after four days of rifampicin therapy. Finding of multiple-resistance H. influenzae type b meningitis and therapeutics problems are emphasized by authors.
