[Gynecomastia. Investigation and surgical treatment].

Enlargement of the male breast, gynecomastia, commonly occurs in otherwise healthy men. If the gynecomastia is marked, surgical treatment may be necessary. Underlying disease must be considered. History, physical examination, laboratory tests and in some patients ultrasonography of the testes should be conducted prior to surgery. If the gynecomastia is due mainly due to fat, suction lipectomy alone is sufficient treatment. However, liposuction will not remove breast parenchyma, and some patients also require local excision of the parenchyma. In our experience, suction lipectomy provides a new means of acquiring better contour, with reduced morbidity and complications.

[Surgical treatment of angina pectoris in elderly patients].

Twenty-eight patients, age 65 years or more, received aortocoronary bypass surgery during the period 1983-85. There was one early death. There were no serious complications connected with the operation, but increased risk of complications is to be expected with higher age. The postoperative stay in hospital is somewhat longer in the elderly. At follow-up, after 19-52 months, 14 patients were free of symptoms, seven were much improved and four were improved.

[Surgical treatment of renal artery stenosis].

Renal artery stenosis may be the cause of hypertension and reduced renal function. The aim of surgical treatment is to eliminate or reduce the hypertension and to preserve or improve kidney function. At our hospital the standard surgical procedure is aortorenal bypass. We present the results of this procedure in the treatment of 23 patients (27 bypasses) during a five-year period. During the same periode nine patients were treated with percutaneous transluminal renal angioplasty. Arterial reconstruction of the renal artery is a relatively simple and cost-effective treatment. Furthermore, the rate of postoperative complications is low.

Aortoduodenal fistula reconstructed with autogenous tissue. Case report.

Aortoenteric fistula is a rare but serious complication of aortic surgery. A case of secondary aortoduodenal fistula is presented. Treatment with closure of the fistula and removal of part of the graft was successful. The circulation was restored by autogenous saphenous vein bypass grafting.

Three-dimensional ultrasound-based navigation combined with preoperative CT during abdominal interventions: a feasibility study.

PURPOSE: Three-dimensional (3D) intraoperative ultrasound may be easier to interpret when used in combination with less noisy preoperative image data such as CT. The purpose of this study was to evaluate the use of preoperative image data in a 3D ultrasound-based navigation system specially designed for minimally invasive abdominal surgery. A prototype system has been tested in patients with aortic aneurysms undergoing clinical assessment before and after abdominal aortic stent-graft implantation. METHODS: All patients were first imaged by spiral CT followed by 3D ultrasound scanning. The CT volume was registered to the patient using fiducial markers. This enabled us to compare corresponding slices from 3D ultrasound and CT volumes. The accuracy of the patient registration was evaluated both using the external fiducial markers (artificial landmarks glued on the patient's skin) and using intraoperative 3D ultrasound as a measure of the true positioning of anatomic landmarks inside the body. RESULTS: The mean registration accuracy on the surface was found to be 7.1 mm, but increased to 13.0 mm for specific landmarks inside the body. CT and ultrasound gave supplementary information of surrounding structures and position of the patient's anatomy. Fine-tuning the initial patient registration of the CT data with a multimodal CT to intraoperative 3D ultrasound registration (e.g., mutual information), as well as ensuring no movements between this registration and image guidance, may improve the registration accuracy. CONCLUSION: Preoperative CT in combination with 3D ultrasound might be helpful for guiding minimal invasive abdominal interventions.
