[Reduction of corneal permeability in patients treated with HP-guar: a fluorophotometric study].

INTRODUCTION: The incidence of dry eye disease, which mainly affects the corneal epithelium, is rising. The main treatment is still the use of artificial tears capable of improving the humidification and lubrication of the corneal epithelium, and avoiding its progressive functional failure. HP-Guar is a new compound used for this purpose. METHODS: We performed a prospective, masked and paired study on the left eye of 10 patients suffering from dry eye disease. Corneal permeability measurements were performed by fluorophotometry after instilling 40 microL of a solution of 2% sodium fluorescein, before and after treatment with the HP-Guar drops. The results were analysed using Wilcoxon test for paired data. RESULTS: A mean decrease in corneal permeability of 45%, after the use of HP-Guar drops was found (p=0.002). CONCLUSIONS: HP-Guar provides a new therapeutic option significantly decreasing corneal epithelial permeability.

[Optic nerve drusen and deep visual fields defects].

PURPOSE: Optic nerve drusen needs to be included in the differential diagnosis of pseudopapilledema. As the identification of this entity by funduscopy may be difficult, ultrasonography has thus become the gold standard for its diagnosis. Severe optic nerve drusen has been correlated with a reduction of the nerve fiber layer measured by optic coherence tomography and with the presence of serious visual field defects. To demonstrate the relationship between extensive optic nerve drusen and visual field defects. METHOD: A prospective observational study of the visual fields of a series of 5 patients with severe optic nerve drusen diagnosed by ultrasonography. RESULTS: Visual field defects of widely differing severity, from inferior nasal quadrant to severe hemivisual field defects, were described in each patient studied. CONCLUSIONS: Visual field defects of diverse severity are common in patients with deep optic nerve drusen. For that reason ultrasonography and/or optical coherence tomography is highly recommended where such visual field defects exist.

[A comparative study of 0.15% sodium hyaluronate versus polyvinyl alcohol in the treatment of dry eyes].

PURPOSE: To evaluate the efficacy of two artificial tears for the treatment of dry eyes: 0.15% sodium hyaluronate and polyvinyl alcohol. METHODS: The study included 60 dry eyes in 30 patients. Fifteen of the patients were treated with 0.15% sodium hyaluronate and 15 with polyvinyl alcohol. Five drops a day for one month was given in both groups. Visual acuity, Schirmer's test, tear clearance, dry eye symptoms questionnaire, fluorescein and rose bengal staining, corneal sensibility, bulbar hyperemia, tear detritus, meibomian gland plug, palpebral erythema and edema before and after treatment were evaluated. RESULTS: Symptoms as judged by the dry eye questionnaire, as well as both fluorescein and rose bengal staining, were significantly improved by 0.15% sodium hyaluronate. The polyvinyl alcohol treated patients did not show a significant improvement in any parameter. CONCLUSION: Sodium hyaluronate was more effective than polyvinyl alcohol in the treatment of patients with dry eyes (Arch Soc Esp Oftalmol 2005; 80: 387-394).
