This article reviews 11 cases of distal revascularization of the extracranial vertebral artery above C2. Revascularization was indicated in 7 cases of extensive atheromatous stenotic or occlusive lesions, 3 cases of dissecting aneurysm, and one case of cervical malformation of the vertebral artery. The procedure of choice was a venous graft - in the C1/C2 space in 10 cases and above C1 in the eleventh. Doppler tests, ultrasound and arteriography demonstrated the patency of all the bypasses, and this was confirmed, 6 months to 3 years postoperatively, by Doppler and ultrasound. No neurological anomalies were observed either during the postoperative course or subsequently. The satisfactory results obtained would seem to justify extension of indications to include the treatment of atheromatous lesions of the supra-aortic trunks, and also adjacent vascular tumors (vertebral bone tumors, or complex cervical malformations of the vertebral artery).

The technique used for an isolated lesion, involving an emptying-graft procedure, is discussed initially, together with particular features related to proximal and distal lesions. The treatment of multiple aneurysms is then evoked, including the attitudes necessary when confronted with either atheromatous disease or aneurysmal polydystrophy.
Operative treatment was applied in 139 patients with hemodynamically significant bilateral stenosis of the carotid arteries due to atherosclerosis. Three groups could be distinguished: group I--stenosis greater than 75% on one side and between 50 and 75% on the other side; group II--stenosis greater than 75% on both sides; group III--obstruction on one side and stenosis greater than 50% on the other side. The risk of clamping did not appear abnormally high, while the peroperative neurologic risk was slightly enhanced due to the possibility of accidents in the region which lies contralateral to that of the operated side, or in the region of the brain stem. The risk in the group III patients did not appear to be increased. Immediate and secondary neurologic prognosis is mainly a function of the presence of multiple peculiar lesions, and also of the presence or absence of parenchymatous ischemic lesions. Preventive surgery on narrowed, stenotic, progressive lesions, detected by ultrasound imaging systematically repeated every 6 months, diminishes the incidence of onset of neurologic complications.
Between 1977 and 1981, 104 patients underwent 115 axillo-femoral bypass operations (AFB). All the indications were based upon limb salvage. Ninety-nine unilateral bypasses and only 8 axillo-bi-femoral procedures were performed. Prolongation of revascularisation by femoro-popliteal bypass was necessary in 10 cases, in 1 or 2 stages. Secondary bilateralisation of an unilateral bypass was required in only 6 cases because of the development of gangrene of the other limb. The postoperative mortality was 6% (5 patients) in the gangrene group and was 33% (6 patients) in the acute ischaemia group. Study of the cumulative permeability of bypasses until the time of their first thrombosis showed that 62% were permeable at 1 year and 44% at 4 years. Thrombectomy was followed by a percentage permeability of 10%. Weight bearing was still possible by 75% of limbs after 5 years. In conclusion to this study: axillo-femoral bypass remains reserved for salvage of the limb; extension of implantations to the distal and/or pathological artery remains legitimate; PTFE seems the most suitable material at present; the development of a thrombosis in the first 18 months should be treated by thrombectomy in order to decrease the number of amputations.
Using an Esmarch's bandage and a lower thigh tourniquet makes it possible to suppress flow and backflow in arteries below the knees. Distal anastomosis without proximal and distal clamps can then be performed on arteries with pathological wall but with adequate residual lumen. The elective indication is bypass on partially calcified arteries of the leg.

Takayasu's disease is a syndrome of inflammatory arteritis involving the aorta and its branches due to varying etiologies. The diagnosis is usually made after clinical, radiological and biological investigations, but the pathognomonic sign of the disease is inflammatory sclerosis of the adventitial media. There appear to be five prognostic factors: hypertension, arterial aneurysm, aortic regurgitation, coronary insufficiency, and retinopathy.
Six cases of this rare association (7 to 10 p. 100 of Takayasu's disease) are reported. The authors also review 73 previously reported cases. The valvular lesion is usually detected secondarily during follow-up (2/3 of cases), but is observed at the same time as the arterial disease in about 1 out of 5 cases. In rare cases, it may be the presenting feature before the vascular disease becomes clinically apparent. Quantification of the regurgitation may be difficult because of stenosis of the thoracic aorta and the supra aortic vessels. In some cases it is severe and poorly tolerated but has no particular distinguishing features apart from the incidence of aortic wall calcification (ascending aorta to all of the aortic arch). Twenty five anatomical observations (operative or autopsy) are sufficiently well documented to show the mechanism of the aortic incompetence. It is caused by an inflammatory aortitis: valvular lesions were found in 2/3 of cases but other causes may be observed, dilatation of the aortic ring (1 out of 4 cases), disunion of the commissures (1 out of 4 cases) and changes in aortic compliance causing systolic hypertension. Aortic valve replacement is rare (11 cases including 3 personal cases) and sometimes completed by resection of an aneurysmal ascending aorta.
The complications of angiography, arterial puncture and catheterisation in the context of iatrogenic arterial disease are extremely variable and they are related to the condition of the artery (healthy or pathological), the site of puncture and the type of catheterisation; their clinical manifestations and prognosis are therefore wide ranging. Three types of complications can be distinguished by their high incidence: --complications of the Seldinger method comprising 110 cases between 1967 and 1980; --complications of high translumbar aortography, including one rare complication which should be underlined because of the severity of the prognosis: septic rupture in two observations; --complications of angioplastic dilatation which are now well-recognised. Their character allows precise determination of the indications of this type of therapeutic angiographic procedure: from December 1979 to March 1982, 6 complications observed out of a total of 114 angioplasty performed. The relatively high incidence of iatrogenic complications explains the necessity of early diagnosis, ensuring a good surgical result.

Supraceliac revascularization of digestive (38 cases) and renal (12 cases) arteries, was performed in 50 patients between 1976 and 1982. Excluding acute ischemic lesions of the digestive arteries, and acute aortic thrombosis in one case, there was no postoperative mortality and only one case of late occlusion of an aortoceliac shunt. Short-and long-term follow-up examinations failed to reveal any morphologic or hemodynamic alterations. Implantation of the shunt in a zone that avoids the atheroma and the anterograde mode of revascularization appear to provide favorable results, this method being therefore competitive with conventional techniques.
