[Incidence of allergy to cow's milk protein in the first year of life and its effect on consumption of hydrolyzed formulae].

OBJECTIVES: The aims of this study were to determine the incidence of allergy to cow's milk protein in infants and the cost of treatment with hydrolyzed formulae in the Autonomous Community of Madrid. PATIENTS AND METHODS: Infants with suspected adverse reaction to cow's milk protein born between March 1, 2000 and February 28, 2001 within the catchment area covered by the pediatric services of Hospital La Paz were studied. The diagnostic algorithm of allergy to cow's milk protein of our department was used. The theoretical consumption of hydrolyzed formulae in the treatment of allergic infants was calculated and extrapolated to the Community of Madrid. RESULTS: There were 5367 live births in the above-mentioned catchment area. Adverse reaction to cow's milk protein was suspected in 185 infants. Allergy to cow's milk protein was confirmed in 101 (54 %). The incidence of allergy to cow's milk protein was 101/5356 (1.9 %). In the same period there were 57 078 live births in the Autonomous Community of Madrid. The number of infants that might be allergic in one year would be 1084. The cost of hydrolyzed formulae per infant and year would be 1,585.72 Euros. Thus, the cost of hydrolyzed formulae in the 1084 allergic infants would be 1,718,922.9 Euros; in the 923 in whom allergy was ruled out, the cost would be 1,463,621.8 Euros. CONCLUSIONS: The incidence of allergy to cow's milk protein in the first year of life is at least 1.9 %. The correct diagnosis allows us to rule out allergy to cow's milk protein in almost half of the cases, thus avoiding the use of unnecessary substitutive diets involving a high cost.

Analysis of cross-reactivity between sunflower pollen and other pollens of the Compositae family.

The sera of 20 patients with Compositae pollen allergy were investigated for the presence of IgE antibodies reacting against sunflower pollen by means of RAST and immunoblotting studies. Thirteen IgE-binding bands were detected with molecular weights ranging from 14.4 to 94 kd. Two of these bands, with molecular weights of 24 and 25 kd, contained major allergens that reacted strongly with 100% (24 kd) and 95% (25 kd) of the sera, respectively. Cross-reactivity between sunflower and other Compositae pollens (mugwort, marguerite, dandelion, golden rod, and short ragweed) was revealed by RAST and immunoblotting inhibition experiments. Mugwort pollen exhibited the greatest degree of allergenic homology (cross-reactivity) with sunflower pollen, whereas at the other end of the spectrum, short ragweed showed less cross-reactive epitopes.

Possible consequences of elimination diets in asymptomatic immediate hypersensitivity to fish.

The natural history of IgE antibodies to food without related symptoms is unknown. We have followed the progress of 7 children with various atopic diseases and asymptomatic immediate hypersensitivity to fish, treated with elimination diet in spite of full alimentary tolerance. During the diet period, between 24 and 113 months, all 7 patients presented immediate symptoms upon accidental exposure to or challenge tests with fish (skin symptoms in all 7 cases, digestive in 5, respiratory in 4, and anaphylaxis in 2), which differed from those related to atopic diseases previously present. The levels of fish-specific IgE (prick test, RAST) remained unchanged or were increased. These findings suggest that during elimination diet, and perhaps due to minimal and hidden contact with the allergen, the patients' degree of sensitization may increase, turning an asymptomatic into a symptomatic immediate hypersensitivity.

[Allergic drug reactions in children].

We present a retrospective study of allergic drug reactions seen in our pediatric allergy consulting room during the last 6 years. During this time, 840 patients were examined for suspected adverse drug reactions. Drug allergy was confirmed in 72 cases (8.5%). Of these cases, 29 (40.2%) were considered to be IgE mediated, or immediate hypersensitivity reactions. We have not found significant differences with regards to age, sex, atopic family history or atopia between patients with IgE mediated reactions compared to patients with allergic drug reactions of different mechanisms or to patients without drug allergies. Sulfonamides, streptomycin, beta-lactam and analgesics were the drugs most frequently involved in immediate type reactions. Among non-immediate reactions, fixed eruption by sulfonamides and contact dermatitis due to Mercurochrome were the most frequent.

[Asthma and gastroesophageal reflux. Outcome of asthma after longterm medical treatment of the reflux].

Over one half of the cases of chronic severe childhood asthma, refractory to conventional therapy and without other over aetiology, are associated to gastroesophageal reflux (GER). The aetiopathogenic role of GER in asthma is uncertain, and is only confirmed when therapy of the former induces evident improvement of the latter. We have instituted medical antireflux therapy over two years in 17 patients (17.6%) became asymptomatic after less than three months of therapy, and a further four (23.5%) by the sixth month. Seven patients showed clinical improvement by the third month (41%) and even a greater one by the sixth month, only very slight symptoms persisting thereafter. In three cases (17.6%) there was no improvement after two years of outcome of asthma and the persistence or not of pathological between the outcome of asthma and the persistence or not of pathological 24-hour pHmetry. On the contrary. macroscopic oesophagitis disappeared in the cases of asthma with good outcome, but persisted in all the cases who remained symptomatic. In conclusion, we consider that medical management of GER associated to asthma should be maintained for at least six months (if the clinical course so permits) before considering a surgical indication.

[Food allergy in children. I. Clinical aspects and diagnosis].

Two hundred and thirty children with suspected food allergy have been reviewed retrospectively. One hundred and nine fulfilled the following three selection criteria: suggestive anamnesis, presence of specific IgE antibodies (in serum or by skin prick test), and having been reassessed at least once. The major clinical data analyzed were: age of onset, sensitising foods, symptomatology and diagnostic procedures. The results show a mean age of onset for food allergy of 11.5 months, with variations related to the sensitizing food, cow's milk allergy being the earliest in onset time. The most frequent sensitizing foods in this group of patients were eggs, fish and cow's milk. The skin and the gastrointestinal tract have been the most affected shock organs, both together or isolated; respiratory symptoms were an uncommon finding and their isolated presentation as the sole manifestation of food allergy exceptional. The skin prick tests were positive in 99.2 percent of the sensitizations and in 84 percent high levels of specific IgE antibodies were found. Food allergy seems to be a common situation in children, clinical symptomatology is similar to that of the other atopic diseases and the diagnosis is based on the demonstration of specific IgE antibodies against the sensitizing foods.

Syndrome auriculotemporal.

Two cases of auriculotemporal syndrome are presented in two male children that began in the first years of life to present reactions of linear erythema itinerary on the cheeks after eating several foods. The allergologic study with these foods was negative, reproducing the clinical picture after their ingestion.

Nephrotic syndrome and respiratory allergy in childhood.

The etiology and pathogenesis of idiopathic nephrotic syndrome remain obscure. Several investigations have reported a role for allergy in the development and maintenance of this disease, especially in childhood. We have studied 20 pediatric patients with relapses of nephrotic syndrome related in time to respiratory symptoms. Sensitization was demonstrated to one or more allergens in 7 patients with episodes of proteinuria of seasonal tendency. Preventive management with disodium cromoglycate was successful in preventing new relapses in 3 patients; specific immunotherapy was assayed in another 2 without beneficial outcome. There appears to be a pathogenic relationship between respiratory allergy and proteinuria in some cases of nephrotic syndrome.

Asthma in children and ASA intolerance.

Intolerance to acetylsalicylic acid (ASA) in asthmatics has been widely studied in the adult population, and to a lesser extent in children. In the present study, we present 16 asthmatics between the ages of 2 and 14 suffering from asthma induced by ASA ingestion, and the clinical characteristics are compared with a population of asthmatic children with a negative challenge test. The following results were obtained: 1) in contrast to in adults, females are not predisposed to ASA intolerance in childhood, the male:female ratio being the usual 2:1 in infantile asthma; 2) ASA intolerance can appear at a very early age (in our series the youngest was 1 year old); 3) extrinsic asthmatics are the most commonly affected, and also children with exercise-induced asthma; 4) in extrinsic asthmatics with asthma attacks precipitated by ASA, sinusitis is more frequent than in extrinsic asthmatics with ASA tolerance; 5) polyposis is exceptional; 6) the presence of associated urticaria is frequent, and much greater than in adult ASA-intolerant asthmatics; and 7) the results of the challenge with NSAIDs are similar to those obtained in adult patients, which would indicate a common pathophysiological mechanism related to the capacity of these drugs to inhibit cyclooxygenase activity.
