[Sodium gamma-hydroxybutyrate. Effects on visual flash-evoked potentials].

Na gamma hydroxybutyrate being recognized as a mean of brain protection, the effect of Na gamma hydroxybutyrate on visual evoked potentials is studied in ten neurosurgical patients. Recordings are made in the same intensive care unit and with the same equipment. Latencies of the three principal peaks (III, IV, V) are analysed. No gamma hydroxybutyrate, given intravenously (60 mg . kg-1), significantly increased latencies (p less than 0,001). So, it must take into account of this effect when visual evoked potentials are studied in Na gamma hydroxybutyrate treated-patients. However, visual evoked potentials peaks are only delayed by Na gamma hydroxybutyrate and none of the studied peaks is abolished.

[Effects of anterior chamber perfusion on the rabbit corneal endothelium: a scanning electron microscopy study (author's transl)].

The effects of intraocular irrigating solutions (TC Earle and NaCl 9%) on the rabbit corneal endothelium were evaluated utilizing scanning electron microscopy. Introcular pressure was maintained at 15 or 50 mm Hg. Rate of perfusion was between 50 microliter/min and 2 ml/min. With NaCl 9%, in eyes exposed to high pressure (50 mm Hg) for 2 hours with a rate of 50 microliter/min changes could be detected: rupture of apical junctions and minimal structural changes. Similar lesions appeared with a pressure of 15 mm Hg at a rate of ml/min. With Earle solution, no lesions were seen after 3 hours and minimal changes after 4 hours. Intraocular irrigating solution seems to be more toxic than increased intraocular pressure on the structure of endothelial cells, as seen on scanning electron microscopy. Greater sensitivity of endothelium to perfusion fluid than intraocular pressure probably explains the paradoxical appearance of corneal edema in diseased states, and suggests that endo-ocular surgical procedures require modification.

[Craniopharyngioma in adults].

Initial signs of a craniopharyngioma in a 58 years old woman were a decrease in visual acuity and suppression of the pupillary light reflex. Perimetric signs were: left: central scotoma with an inferior temporal defect of the peripheral field; right: superior temporal field defect. The CT scan showed an intrasellar tumor, extending into the suprasellar region, isodensity of the tumor being the main finding. The radiological diagnosis was adenoma, but all the perimetric defects disappeared spontaneously, and the post-operative diagnosis was craniopharyngioma. Variations of the visual field defect are one of the most typical signs of craniopharyngioma, and absence of calcification, with isodensity of the tumor on the CT scan may lead to a mistaken.

[Antiviral agents and corneal epithelium cicatrisation: study in rabbits (author's transl)].

Five clinically-used antiviral drugs (3% adenine arabinoside ointment; 3% acycloguanosine ointment; 0.24% idoxuridine ointment; 1% trifluorothymidine drops) were compared with a control (petrolatum base) to determine their toxic effects on rabbit corneal epithelium after injury by iodine vapors: --Only trifluorothymidine significantly retarded healing of epithelial erosions. --Histopathologic examination after seven-day treatment showed that all five drugs, except vidarabine and to a lesser degree acycloguanosine, caused toxic changes in the regenerating epithelium.

[Post-traumatic intramural hematoma of the duodenum].

A post-traumatic intramural hematoma of duodenum in a 9 year old child was detected during exploratory laparotomy but left undisturbed. Postoperative follow up during parenteral feeding included surveillance by repeated gastrografin follow through examinations, normal transit being restored by the 12th day. Intramural hematoma of duodenum is usually due to injury and often affects male children. Symptomatology is that of upper digestive occlusion. The principal investigation should be gastro-duodenal follow through examinations with gastrografin, because of the risk of an associated perforation, to reveal possible partial or total duodenal obstruction. Conservative treatment is possible, but in case of failure or in adults surgery is indicated with evacuation of the hematoma and in some cases a gastro-jejunostomy.
