Enhancing mediastinal mass at MR: Castleman disease.

We report a case of an enhancing middle mediastinal mass imaged by MR and CT. Repeated nondiagnostic biopsies in this 25-yr-old male patient had resulted in profuse hemorrhage. Final pathological diagnosis after thoracotomy and debulking was Castleman disease. This is a rare, benign lymphoproliferative disorder of uncertain etiology. Histological varieties include the hyaline vascular type (80-90%) and the plasma cell type (10-20%). Cure is generally accomplished via surgical excision. This entity should be included in the differential diagnosis of enhancing mediastinal lesions, prompting awareness of the potential for bleeding complications following biopsy.

Computed tomography of gangrenous acute postoperative acalculous cholecystitis.

The computed tomographic findings in a case of complicated acute cholecystitis after surgery unrelated to the biliary tree are described. Postoperative cholecystitis is associated with high morbidity and mortality, and early diagnosis by appropriate imaging modalities and prompt surgical intervention is essential in the successful management of this complication.

Acute Disease↗

An unusual ventilation perfusion scintigram in a case of immunologic pulmonary edema clinically simulating pulmonary embolism.

A case of immunologic pulmonary edema secondary to hydrochlorothiazide allergy developed in a 55-year-old woman that clinically simulated pulmonary embolism. The patient had abnormal washin images with normal washout images on an Xe-133 ventilation study. On the perfusion study, large bilateral central and posterior perfusion defects were present that showed an unusual "mirror image" pattern on the lateral and posterior oblique views. Resolution of radiographic and scintigraphic abnormalities occurred over a 3-day period in conjunction with corticosteroid therapy.
