Determination of the position of the pelvic girdle using surface markers.

The overall movement of the trunk is made up of two components, namely the movements of the pelvic girdle and the vertebrae. In the frontal plane, the amplitude of the pelvic movements appears to be relatively limited compared to the vertebral column whereas the pelvis makes the major contribution to the total rotational movement in the axial plane.

Leg cycling tracking by dynamic vision.

This study describes a method of tracking of human body limbs from a monocular sequence of perspective images. These objects and the associated articulations must be modelled. The principle of the method is based on the interpretation of image features as the three-dimensional perspective projections points of the object model and an iterative process method to compute the model position in accordance with the analysed image. This attitude is filtered (Kalman filter) to predict the model position relative to the next image of the sequence. The image features are extracted locally according to the computed prediction. Tracking experiments, illustrated in this study by a leg cycling sequence, have been conducted to demonstrate the viability of the approach.

Plasma exchange in Rasmussen's encephalitis.

The authors observed a 4-year-old girl who has Rasmussen's encephalitis. She started with frequent localized and generalized seizures. Standard antiepileptic treatment was almost ineffective. The frequency of the generalized seizures decreased, but the myoclonic jerks of the left part of the body persisted. An EEG showed partial status epilepticus. The results of the CT scan were normal. Antibodies to viruses were absent from the blood and cerebrospinal fluid. An MR scan showed a T2-weighted hypersignal zone in the right frontal region. Intravenous bolus injections of corticosteroids and drips of immunoglobulins were inefficient, and we started plasma exchanges which have continued for 9 months. The clinical state stabilized, and the images on the MR scan improved, but the results of the EEG did not improve. The authors discuss the effect of the plasma exchange, the use of which is questionable in this disease.

[Kinematics of the pelvic girdle and the thoracic and lumbar segments in the course of the lateral inclinations and rotations of the spine].

Physiological displacements of the trunk are the addition of these of the pelvic girdle and thoracic and lumbar spine segments. For a long time (3), this conjunction had been noticed but without numbered precisions. The purpose of this communication is to appreciate quantitatively, from a series of 16 subjects, the respective share of each components during lateral bending movements in the frontal plan and movements of rotation in the transverse plan. It results from this work that the pelvic girdle presents as reduced amplitude (4 degrees) in lateral bending on the other hand, displacement predominate to the level of thoracic spine (50 degrees). In rotation pelvic displacements are very important (30 degrees), while the spine so thoracic that lumbar has a weak participation (inferior to 5 degrees). Moderated abduction of hips increases by significant manner the motility of the pelvic girdle.

Appendicitis in children and young adults: Doppler sonographic-pathologic correlation.

OBJECTIVE: The purpose of this study was to determine the following: whether the arterial supply of a normal appendix is visible with Doppler sonography; whether the physiologic vasodilatation that accompanies childhood appendicitis is visible; what Doppler patterns appear once necrosis of the appendix has occurred; what Doppler shifts are visible with chronic, recurrent appendicitis; and whether other conditions in the right lower quadrant can mimic the Doppler sonographic patterns of appendicitis. MATERIALS AND METHODS: Twenty-five fasting patients without abdominal pain or intestinal disease and 45 patients (1-25 years old; mean, 8 years old) with right lower quadrant pain and suspected appendicitis were examined sonographically, using 5- and 7-MHz linear, color, and pulsed Doppler transducers. Arterial signals were sought within the appendix and neighboring tissues, counted, and classified as sparse (1-2), moderate (3-4), or abundant (>4). The resistive index (RI) was measured. Thirty patients who underwent surgery were retained for surgical and pathologic correlation, and only they formed the appendicitis study. RESULTS: A normal appendix was found in 10 patients. Doppler shifts were sparse, and diastolic flow was low or absent (RI, 0.85-1). Acute uncomplicated appendicitis was found in 13 patients, who had abundant color Doppler signals throughout most of the appendix, with high diastolic flow (RI, 0.40-0.77; mean, 0.54). Acute necrotic appendicitis with perforation was found in 11 children, eight of whom showed no signals in the necrotic area at the tip. Few or no signals were seen in the remainder of the appendix (RI, 0.33-0.90; mean, 0.54). In two patients, signals were abundant in the tissues surrounding the appendix. Recurrent or chronic appendicitis was found in three patients, previously diagnosed as having Crohn's disease, psychosomatic illness, or nonspecific abdominal pain. The appendix had Doppler signals confined to the tip (RI, 0.63-0.83; mean, 0.75). Other diagnoses were found in three patients, in whom the appendix was not seen. There were abundant color signals in the intestinal wall and adjacent tissues in the right lower quadrant. Two patients had Crohn's disease, and the third had an unsuspected early pregnancy. The appendix was normal in all. CONCLUSION: Acute appendicitis is accompanied by inflammatory hypervascularity reflected as an increased number of color signals and higher diastolic Doppler shifts as compared with those found in normal persons. No Doppler shifts are identified in areas of appendiceal ischemia. Other acute inflammation in the right lower quadrant also produces numerous Doppler shifts with high diastolic flow, as does ovulation. Care must be taken to identify the source of these Doppler signals. The changing vascularity of healing, recurrent, and chronic appendicitis promises to further our understanding of the pathogenesis and evolution of this disease.

[Total rectal prolapse in children. Diagnostic and therapeutic trends. Statistics apropos of 52 cases].

Rectal procidentia represents classically only 2% of rectal prolapse in children. This rarity is more apparent than real, because peritoneographies showing the excess of migration of the rectogenital pouch, allows to diagnose more often this pathology. The normal trend of this anomaly of the infant and the young child even if it could be serious with strangulation is usually a spontaneous healing. Accordingly therapeutic indications should be cautious. Surgery is only indicated in confirmed or complicated forms. After analysis of the statistic of 52 observations, the modified Lockhart-Mummery is our prefered procedure. It needs a short and simple surgery and the results appear satisfactory.

Determination of the position of the pelvic girdle by the method of external markers in the sagittal plane: a preliminary feasibility study.

The global amplitude of the movements of the trunk is the resultant of the sum of two components: the movements of the pelvic girdle and those of the vertebral segments. In order to assess the precise position of the pelvis in kinetics, we endeavored by the use of skin markers to determine whether reliable measurement points existed. The experiment showed that a line drawn from the median sacral crest at the level of S1 to the highest point of the iliac crest appeared relevant and practical for assessment of the position of the pelvic girdle in the sagittal plane.

[Preliminary analysis of the modifications of spinal curves, in extreme amplitudes, observed in rhythmic and athletic gymnastics].

Trained users of Rhythmic and Athletic Gymnastic, present kinetic spinal vast extents. Analysis, with pasted cutaneous markers, shows that, according to the codified movement, various segments of the vertebral column are requested and realized important displacements. Nevertheless, to have an homogeneous attitude, some subjects request more either lumbar or thoracic segments, of both. To prevent frequent spinal pain, it is important to realize a preliminary subject selection using their total spine and to improve protection by adapted gymnastic.

[Comparative study of static curves of the human spine in vivo during exercise and stretching in men and women].

Comparative study of static curves of the thoracic and lumbar spine in men and women has been realized under constraints of compression by weightlifting and elongation by exercise to parallel bars. In two athletic practices, curves decrease their arrow, ending to a straighter and more rigid column. This behavior expresses muscular actions. The greatest amplitude of displacement, observed in female series, shows lesser muscular forces in connection with a poor practice of exercises of body building.

[Measurement of the torsion of the two bones of the forearm: evaluation of tomodensitometric reference points].

In order to evaluate the rotation following radial and ulnar fractures, the authors propose radial, ulnar and humeral landmarks for a CT scan measure of radial and ulnar torsion. In the ulna, the dorsal at the level of the proximal radio-ulnar articulation, as well as the bicondylar axis of the humerus seem to be fair landmarks. Distally, the styloid process is regularly identified and may serve as distal reference for the measure. In the radius, the ventral face of the distal epiphysis is always clearly visualized and seems to be a good landmark; the radial tuberosity, which was primarily chosen as proximal reference, is not consistently identified and cannot be used as landmark. The variation of position of the radial head with respect to the capitulum during pronation needs too know the amount of this movement before the measure if the bicondylar humeral axis is chosen as proximal reference.

Preliminary study of the human lumbar spine using speckle technique. Limits of compression, shear and torsion. Speckle photography--electronic speckle pattern interferometry.

Optical techniques using single wavelength lasers allow precise study of the superficial displacements of deeper anatomical structures through direct mechanical pull. Two techniques have been used in this study: 1. Double exposure speckle photography. Since 1980 the relationship between the surface displacements and minimal traction applied to the transverse processes of the spine, either unilateral or bilateral, was studied in an attempt to simulate muscular action exerted at this level. 2. Computerised speckle interferometry. This allowed analysis of the deformations between the lumbar vertebrae under vertical compression, shear or torsion outside the usual anatomical planes. This preliminary study outlines the mechanical behaviour and the plasticity of the vertebral structures but has been performed on only a few anatomical specimens thus not allowing statistical analysis. This would require a larger series.

A model of the human lumbar vertebral column: a preliminary study.

Finite element modelling of the human lumbar vertebral column employs data-processing procedures for study of the linear and nonlinear elasticity of materials such as are currently used in mechanics or in civil engineering. Thanks to developments in computer science, requiring a close collaboration between doctors and engineers, we put forward in this preliminary study a linear computerised model of the lumbar column comprising 4824 meshes and 6813 nodes. By reducing the simplificatory hypotheses and integrating new parameters, this model as developed is capable of important clinical applications in surgery and ergonomics.

[The intervertebral disk. Attempt at a "finite element" model].

Intervertebral disk composite model is unsatisfactory definite in biomechanical behaviour despite multiples technics used. Using histologic and histoenzymology it's possible to determinate proportions of collagen fibers in the different parts of the disk. A trustworthy finite element 3D model is proposed and tried by real experiments.

Preliminary opto-electronic study on vertebral movement.

Opto-electronic systems utilising measurement of displacement of skin markers allows study of movement in the living subject. The authors have used this method in a kinematic study of the thoracic and lumbar spine measuring the displacement of skin markers placed over the spinous processes. It was possible to approach the physiological state of these complex movements once the apparatus had been calibrated to the correct level, and the error margins minimised. Repeated measurements confirmed the reliability of this method even if movement of the skin with respect to the bony reference points introduced some margin of error. Three dimensional displacement of the vertebrae were measured during voluntary movements of the spine demonstrating the complex geometry. Since opto-electronics are non-invasive they constitute an important advance in the study of the kinematics of the spine.

The anatomical basis for ultrasonography of the lateral pterygoid muscle and the infratemporal fossa.

The infratemporal fossa and its contents (particularly the pterygoid muscles) is a region difficult to explore, in spite of its importance in odontostomatological pathology. In order to reduce the indications that justify examination by computed tomography and magnetic resonance imaging, for reasons of economy of health, the authors describe by application of orientated anatomical slices the way in which ultrasonography can be performed. Only the posterior approach between the mandible laterally and the mastoid process medially provided consistently adequate exploration of the infratemporal fossa and the pterygoid muscles.

[Reliability of cutaneous markers in kinetic studies of the human thoracic and lumbar spine].

The use of cutaneous markers as an accurate method of measuring spine motion was studied in five cadaveric spine samples and two live subjects. The markers were placed on the skin over the spinous processes, a particularly suitable site in thin subjects for determining the displacement of the underlying skeletal segments because the deep layer of the skin is in adherence with their fibrous surrounding sheath. Fairly close correlations were obtained in the sagittal and frontal planes between the two sets of measurements. However in the transversal plane the technique is unreliable because of tightening of the skin.
