[Successful treatment of kappa light chain nephropathy using membrane plasma separation].

Often the diagnosis "light chain nephropathy" is not made before renal failure has developed. In the past these patients had a poor prognosis. Special features of this disease and improvement of renal function by plasma exchange therapy are shown in a case report. A 51-year-old man admitted to hospital with renal failure, proteinuria and hypercalcaemia was treated by Ca-free dialysis and membrane plasma separation (MPS). Renal biopsy studied by light and electron microscopy showed mainly tubular protein casts. It is assumed that this nephropathy is caused by a direct toxic effect of the kappa light chains on function and structure of tubuli. Proteinuria was markedly lowered and renal failure was made partly reversible by prompt and effective MPS. Thus, MPS may improve the prognosis of this stage of disease.

Effects of experimental emotional stress and physical exercise on the circulation in hypertensive patients and control subjects.

Heart rate, intra-aortal blood pressure, cardiac output and peripheral vascular resistance were registered in 11 hypertensive and 11 normotensive subjects during exercise (up to 80 W on a bicycle ergometer), a semi-standardized interview concerning personal life situations, exposure to slides showing parents and marriage partners of the subjects, and during pressure on a hand ergometer. The blood pressure in all subjects increased during all these experimental conditions. During exercise this was associated with increases in heart rate and cardiac output and a decrease in peripheral resistance (hyperkinetic reaction). During the interview the heart rate and cardiac output also increased but to a lesser degree and in 15 of the 22 cases the peripheral resistance rose (predominantly hypertonic reaction). During the slide viewing the effects were similar to those of the interview but less marked. During the hand ergometer test the subjects showed a combination of the hyperkinetic and hypertonic response. Although there was a difference in their basal blood pressure and total peripheral resistance, these haemodynamic reaction patterns showed no difference between the hypertensive and normotensive subjects. These results support the hypothesis that essential hypertension is a quantitative 'exaggeration' of the same physiological processes which regulate the blood pressure in different situations in normotensive individual.

[The clinical significance of compliance].

The authors deal with a particular type of working alliance, called "compliance", which may be defined as mutual agreement between patient and physician with respect to prevention and therapy. This type of compliance may be influenced by the kind of disease (e.g. low compliance in psychiatric illness), by the mode of referral (verbal or written), by the kind of hospital care (an individual appointment with one particular physician in an outpatient department makes for better compliance) and by medication (intravenous or oral administration and color and size of pills). The number of medicines prescribed is of particular importance for compliance, i.e. the greater the number of drugs prescribed per day, the more the patient tends to be unreliable in taking them. Furthermore, the time spent in the waiting room, the amount of information given to the patient and the clarity of the instructions given by the physician play an important role. Situational factors and interaction in the physician-patient relationship seem to be more relevant than specific personality traits.

[Propranolol, triamteren and hydrochlorothiazide in fixed combinations in the treatment of hypertension (author's transl)].

The antihypertensive effect of propranolol, in fixed combinations with two dosages of triamteren and hydrochlorothiazide (Dociteren), was evaluated in a double-blind test on 19 patients with uncomplicated essential hypertension. No significant differences between the two dosages were found with respect to heart rate, as well as systolic and diastolic pressures on lying and standing up. Mean systolic and diastolic pressures were within the normotensive range with both dosages after four weeks. Increasing the diuretic component increased the type and frequency of side effects, without any additional therapeutic advantage. In seven of nine patients the previously present signs of left ventricular hypertrophy disappeared. In 9 patients the mean value of the Sokolow-Lyon index (RV5 + SV1) decreased from 4.078 mV before to 3.31 mV after an eight-week period of treatment. The results of various laboratory tests, except for a transient hyperuricaemia, did not change significantly.

Different endocrinal and hemodynamic response patterns to various noxious stimuli.

Various endocrinal and hemodynamic parameters of 5 patients during a cardiovascular investigation were assessed in and after the adaptation phase, and together with the following noxious stimuli in random order: reading a confusion color chart based on the principle of interference in color perception, cold pressure test, isometric stress with a hand ergometer. The following parameters were assessed: arginine vasopressin, renin, heart rate, systolic, diastolic and mean arterial blood pressure, cardiac index, stroke volume and total peripheral resistance in the adaptation phase and during the various psychological and physical demand situations as well as 5 min afterwards. The results show that the organism reacts to the various stimuli with endocrinal and hemodynamic responses in a differentiated pattern in contrast to the model of the "alarm reaction" which, according to Cannon, states a homogeneous directed response with regard to the various parameters.

Dysplasia of the lymphatics with lymphoedema, generalized lymphangiectasis, chylothorax and "pseudo-storage-disease".

A patient showing an unusual association of various abnormalities of the lymphatic vascular system is reported. These abnormalities became first evident in early childhood and consisted of lymphoedema of the left leg, lymphangiectasis in various organs and occlusion of the thoracic duct at its entrance into the venous angle. Chylous effusions and subcutaneous chyloedema appeared in adolescence. Diagnostic biopsies of spleen, liver and bone-marrow revealed the presence of multiple foamcells, suggesting the diagnoses of lipid storage disease. The patient died from severe honeycomb-lungs at the age of 20. This case cannot be attributed to one of the wellknown disease entities of the lymphatic system. The "pseudo"-storage disease is regarded as secondary to the backflow of chylus into the tissues.

[Dynamic study of tissue circulation in the cervix uteri and the prognosis of the newborn infant].

A new machine has been developed to measure heat dissipated in the tissues especially by convection linked to the blood flow. Its use is simple and without any danger as far as application to the pregnant cervix goes. New discoveries are presented. A new concept of the micro-circulation is considered together with the consequences. The practitioner may be able to draw conclusions as to the prognosis, as to whether a small-for-dates fetus together with or without premature labour is likely. This depends on the tracings obtained which register the intrinsic activity of the periarteriolar muscles.

[The Roemer Award of the German College of Psychosomatic Medicine (1976-1998): quantitative data and contents].

The Roemer-Award of the German College of Psychosomatic Medicine might be the most attractive research award in the German Psychosomatic field. This review starts with an appreciation of its namer, Hans Roemer. Then the award winners between 1976 and 1998 are summarized. Quantitative analyses of the material reveal interesting data about the number of applications during these years and about how the applications and the awarded work are distributed over the different issues of the research field. It is shown that studies dealing with psychosomatic aspects in internal medicine were submitted and awarded most frequently.

A context study of psychological conditions prior to shifts in blood pressure.

Shifts in intra-arterially registered blood pressure during an initial interview were studied in 4 young patients with essential hypertension by means of the Symptom Context Method. The relationships found between both formal variables and thematic categories of speech anteceding blood pressure shifts were in terms of patient and interviewer. All patients showed an increased rate of speech before blood pressure elevations, and in 3 of them the thematic content of 'personal failing, personal failure', 'loss of status of being useful and needed', and 'failure of others to meet the standards of the patient' was more often present before pressure elevations than decreases.

[The effects of heparin in underdevelopment of the fetus due to maternal vascular conditions (author's transl)].

The use of heparin to interfere with the appearance of fetal maldevelopment (or to correct it) is logical but the results are difficult to prove because it is difficult to be certain of early diagnosis at the time when therapy has the best chances of being efficacious. The authors compare two groups of pregnant women in whom the muscular vascular tone of the cervix is abolished. The second group was treated with heparin. The results are statistically significant and prove that treatment with heparin is well based in cases of underdevelopment of the fetus when it is due to abnormalities in the maternal microscopic circulation (acute atherosis). They confirm that the enzyme, hormone and ultrasound parameters return to normal from the beginning of treatment with heparin.

[Relaxation therapy with respiratory feedback in patients with essential hypertension].

During a stationary rehabilitation program for four to six weeks 170 patients with essential hypertension participated in a respiratory feedback (RFB) evaluation. 91 patients were treated with this relaxation therapy and compared with 79 controls. Furthermore the anti-hypertensive medication was controlled. One treatment group (without antihypertensive medication) had a higher systolic blood-pressure decrease than controls. In both treatment groups psychological effects in the sense of more positive mood and less physical complaints were found.
