A series of 18 patients with odontoid fractures due to metastatic cancer were treated at Memorial Sloan-Kettering Cancer Center between 1974--1980. The primary source of cancer was breast (12 cases), lung (two cases), nasopharynx (one case), multiple myeloma (one case), colon (one case), and rhabdomyosarcoma (one case). The clinical features consisted of severe neck pain and neck stiffness in 17 patients; signs of cord compression were noted in only four patients. Tomography and computerized tomography were useful in identifying both the osseous and soft-tissue involvement by tumor. Initial treatment in all patients except those with myelopathy consisted of high-dose steroids, and immobilization in a hard collar. Ten patients were treated with radiation therapy alone; six patients underwent surgical fusion (four before and two after radiation therapy); and two patients died before completion of treatment. Conservatively treated patients were allowed to walk with the support of only a collar following radiation therapy. We believe that the initial management of patients with odontoid fractures secondary to cancer should be high-dose steroids and radiation therapy, unless displacement is marked. Assessment for surgical fusion should be made following radiation therapy, since conservative treatment may suffice in most patients. Early recognition is important so that treatment can be instituted before C1--2 subluxation becomes severe.
We performed seven total hip replacements in six patients with the adult form of Gaucher's disease. Excessive perioperative hemorrhage was a common complication, and three patients have required revision of loosened components. Two of these were young patients with a more severe form of the disease. Total hip replacement is feasible and likely to be successful in the older patient with Gaucher's disease and osteonecrosis or failed prior hip surgery.

Paraquat and diquat facilitate formation of superoxide anion in biological systems, and lipid peroxidation has been postulated to be their mechanism of toxicity. Paraquat has been shown to be more toxic to selenium-deficient mice than to controls, presumably as the result of decreased activity of the selenoenzyme glutathione peroxidase. The present study was designed to measure lipid peroxidation and to assess toxicity in control and selenium-deficient rats given paraquat and diquat. Lipid peroxidation was measured by determining ethane production rates of intact animals; toxicity was assessed by survival and by histological and serum enzyme evidence of liver and kidney necrosis. Paraquat and diquat were both much more toxic to selenium-deficient rats than to control rats. Diquat (19.5 mumol/kg) caused rapid and massive liver and kidney necrosis and very high ethane production rates in selenium-deficient rats. The effect of paraquat (78 mumol/kg) was similar to that of diquat but was not as severe. Acutely lethal doses of paraquat (390 mumol/kg) and diquat (230 mumol/kg) in control rats caused very little ethane production and no evidence of liver necrosis. These findings suggest that paraquat and diquat exert their acute toxicity largely through lipid peroxidation in selenium-deficient rats. Selenium deficiency had no effect on superoxide dismutase activity in erythrocytes or in 105,000 g supernate of liver or kidney. Glutathione peroxidase, which represents the only well-characterized biochemical function of selenium in animals, was dissociated from the protective effect of selenium against diquat-induced lipid peroxidation and toxicity by a time-course study in which selenium-deficient rats were injected with 50 mug of selenium and later given diquat (19.5 mumol/kg). Within 10 h, the selenium injection provided significant protection against diquat-induced lipid peroxidation and mortality even though this treatment resulted in no rise in glutathione peroxidase activity of liver, kidney, lung, or plasma at 10 h. This suggests that a selenium-dependent factor in addition to glutathione peroxidase exists that protects against lipid peroxidation.

Survey teams visited 163 rural villages and 41 urban quarters in Togo and collected data on weight, height, arm circumference, age, and pedal oedema from 6120 children aged 6-72 months. Haemoglobin levels were determined for one-fifth of the survey children. When 80% of the reference median weight-for-height was used as the cut-off point, the prevalence of acute undernutrition was 2.0%. When 90% of the reference median height-for-age was used as the cut-off point, the prevalence of chronic undernutrition was 19.1%. The prevalence of both types of undernutrition was significantly higher in the northern rural regions, with the lowest prevalence in the urban areas. The prevalence of anaemia was 58.6%, with the highest prevalence in the northern regions. Anthropometric data on socioeconomically privileged preschool children were collected and compared with those for the survey children and the reference population. The socioeconomically privileged preschool group of Togolese children had a nutritional status substantially better than that of the survey children and nearly equivalent to that of the American reference population. The survey method was economical in terms of money, time, and personnel. The methodology employed rapidly provided objective data on the extent and distribution of protein-energy undernutrition and anaemia in the preschool children of Togo.

In 167 consecutive pathological or impending fractures of the hip treated by endoprosthetic replacement from 1975 to 1978, there was dramatic relief of pain in all patients. Either a long-stem femoral endoprosthesis or a total prosthetic hip was used. The ambulatory status was significantly enhanced in those patients who were able to walk but it was not in the gravely ill. Cementing the prosthesis allowed stabilization of the entire femur as well as resection of diseased bone. The patients had a median survival time of 5.6 months. There were no dislocations, instances of loosening, or failures of the device, and the incidence of deep infection was 1.2 per cent. If the anticipated life of the patient exceeds one month, that constitutes an indication for prosthetic replacement for treatment of a pathological or impending fracture of the hip.

Articular cartilage slice explants were stored under various conditions, including freezing-thawing at various rates by using dimethyl sulfoxide (DMSO) as a cryoprotective agent, incubating in standard tissue culture medium (MEM Eagle:NCTC 135:15% fetal calf serum) in 5% CO2 and air at 4 degrees, 21 degrees, and 37 degrees C, and incubating in standard tissue culture medium containing 200 micrograms/ml alpha-tocopherol (vitamin E) at 37 degrees C after first ascertaining a dose-response curve of vitamin E. Results indicated that articular cartilage slice explants did not survive freezing or storage at 4 degrees and 21 degrees C as measured by 35S uptake. When stored at 37 degrees C in standard tissue culture in 5% CO2 and air, the slice explants remained viable for up to 60 days. The addition of alpha-tocopherol to the medium resulted in significantly less release of previously incorporated 35Sin stored cartilage slices and significantly less reduction of the amount of hexosamine present in the stored explants. alpha-Tocopherol in the medium also preserved safranin O staining. Thus, the application of tissue culture techniques to the storage of articular cartilage made it possible to preserve cartilage slice explants in a viable, biochemically "normal" state.

Utilizing a knee joint instability model for traumatic osteoarthritis, quantitative measurements of the biomechanical properties of the articular cartilage demonstrate that the cartilage deforms and that the deformation rapidly occurs within 24 hours of injury. These changes in mechanical properties may be a more sensitive indicator of early degenerative changes in articular cartilage than the ordinary histological measurements.
