Comparison of the human electro-oculographic response to green and near-ultraviolet stimuli.

We recorded the electro-oculogram from 27 normal subjects by means of green and near-ultraviolet (UVA) stimulation. After a 40-minute dark-adaptation period, baseline responses were recorded. In response to the green stimulus, the electro-oculogram increased from this level by an average (+/- standard error of the mean) of 49.5% +/- 4.0%. Although the predicted scotopic effectiveness of the ultraviolet stimulus was more than 3 log units below that of the green stimulus, the near-ultraviolet-induced electro-oculogram increased to an average of 21.9% +/- 3.0% above baseline. This response cannot be due to lens fluorescence to the near-ultraviolet stimulus, since two aphakic subjects had electro-oculographic responses of 32% and 76% above baseline to near-ultraviolet stimuli. Neither the green nor the ultraviolet electro-oculogram changed significantly with age. These large responses to near-ultraviolet stimulation demonstrate the need for standardizing light sources for electro-oculographic testing because the degree of near-ultraviolet irradiance varies considerably according to their design characteristics.

Rhegmatogenous retinal detachment after neodymium-YAG laser capsulotomy in phakic and pseudophakic eyes.

A retrospective study of 18 eyes in 17 patients with rhegmatogenous retinal detachments after neodymium-YAG laser posterior capsulotomy was performed to determine operative settings and to describe anatomic changes after the procedure in an effort to assess their relationship to subsequent retinal detachment. The laser energy required to create a capsulotomy did not appear to be excessive and the capsulotomy openings were not unusually large. The time between YAG capsulotomy and diagnosis of retinal detachment ranged from four to 82 weeks (mean, 28 weeks). The characteristics of the retinal detachments were similar to those after routine cataract extraction. Retinal reattachment surgery was ultimately successful in all 18 eyes. Both YAG laser and knife-needle posterior capsulotomies may increase the risk of subsequent rhegmatogenous retinal detachment as a result of opening the capsule.

Choroidal edema associated with retinal detachment repair: experimental and clinical correlation.

Experimental owl monkey data and prospective clinical data is presented, demonstrating the correlation of serous-choroidal edema with the extent of vortex obstruction and the additive factors of sudden hypotony from drainage and cryotherapy. Older eyes are more susceptible for developing this phenomenon of choroidal edema. The extreme extent of vortex venous stasis is that of venous stasis anterior segment ischemia demonstrated in two clinical cases.

Corneal wick electrode for recording bright flash electroretinograms and early receptor potentials.

We developed a corneal wick electrode that is free of photovoltaic artifact when used with a bright flash strobe and is convenient for routine clinical application. The corneal wick electrode is employed for bright flash electroretinogram (ERG) recordings and for research measurements of the early receptor potential. The wick electrode gives accurate, stable, reproducible recordings with an amplitude comparable to that obtained with standard contact lens electrodes.

Calibration error in dark adaptometer.

We noted that the adjustable fixation device present on some models of the Goldmann-Weekers dark adaptometer has a calibration error that results in a consistent and predictable overestimation of retinal eccentricity. The error occurs because the eye being tested is not in the geometric center of the projection sphere. The true retinal locus may be computed by trigonometric analysis. A conversion table is available for reference.

Bilateral acute anterior ischemic neuropathy in diabetes: case report.

A 51-year-old white man presented with an acute bilateral anterior ischemic optic neuropathy approximately 1 month following hospitalization for a hyperosmolar nonketotic diabetic coma. One eye treated by retrobulbar corticosteroids showed marked improvement. The literature reveals that diabetic anterior ischemic optic neuropathy is often diagnosed in the absence of diabetic retinopathy.

Foveal lesions seen in retinitis pigmentosa.

A total of 110 patients with retinitis pigmentosa were prospectively and consecutively evaluated for the presence of foveal lesions. Of these 69 (63 percent) patients showed one of two types of separate and distinct bilateral foveal lesions. Forty-seven (43 percent) patients had atrophic-appearing lesions of the retinal pigment epithelium within the fovea of both eyes, and an additional 22 (20 percent) patients showed bitalteral foveal cysts or partial-thickness holes. Of the 22 patients in this last group, 16 showed cystoid macular edema evident on fluorescein angiography.
