[A study on the relation between cerebral lesion and cognitive defect in multi-infarct patients].

240 cases of multi-infarct patients with more than two cerebral infarct lesions in CT findings were collected successively from 1991 to 1992. 42.92% of them were shown to have cognitive defect with a series of neuropsychological tests. Correlation of the occurrence of cognitive defect with the CT findings such as the location and extent of focal lesions as well as the presence of cerebral atrophy or subcortical degeneration were carried out. According to the results by multiple regression and factor analysis, the location and extent of the focal infarct lesions were not the primary determining factor for the occurrence of cognitive defect. It is, therefore, suggested that the term of ischemic or vascular dementia be used clinically instead of the so-called multi-infarct dementia.

CSF cholinesterase in early-onset and late-onset Alzheimer's disease and multi-infarct dementia of Chinese patients.

Using Ellman spectrophotometric method we measured the total cholinesterase (ChE) activity in lumbar cerebrospinal fluid (CSF) of 13 persons without neurological disorder, 10 non-demented patients with cerebral infarcts, 17 patients with dementia of Alzheimer's type (DAT) (11 presenile, 6 senile cases), 10 patients with multi-infarct dementia (MID), 1 patient with Parkinson's disease associated with dementia. The ChE activity in CSF was significantly lower in the DAT group compared with age-matched control subjects (p < 0.001). This paper also analyses the possibility of using CSF ChE activity as a marker of DAT, and the relationships between its level of activity and the age of the patient at onset, stage of illness and severity of dementia as well as discrepancies in the data published so far. Previous work has shown that ChE activity in the brain tissue and CSF of MID is normal: therefore, if low ChE activity is found in the CSF of MID patients, as was obtained in 8 out of 10 cases in our series, the diagnosis of mixed dementia should be considered.

[Factors related with stroke recovery].

81 patients with an average of 61 who had a hemispheric stroke for the first time and survived more than 12 months were followed in order to study the factors related with recovery. The results are: (1). The severity of the initial attack and the presence of depression and other complications in recovery period are the main relating factors with both the quality of life and recovery of neurological deficits. (2). The size of the lesion shown in CT is the main relating factor with the recovery of neurological deficits. (3). function of affected hand is a main relating factor with the quality of life. According to these results, it is essential to have a preliminary consideration to facilitate recovery at the initial stage of stroke and to organize the rehabilitation programs for the patients.

[Alteration of erythrocyte biophysical properties in patients with ischemic cerebrovascular disorders].

Several erythrocyte biophysical properties (erythrocyte deformability, erythrocyte aggregation and erythrocyte electrophoretic mobility) were investigated in patients with cerebral infarction and in individuals with risk factors of stroke (RFS population; RFSP). Blood viscosity, plasma viscosity, hematocrit, plasma fibrinogen level and yield shear stress (YSS) were also tested. In comparison with the results in a control group, erythrocyte deformability (erythrocyte length under a certain constant shear stress and erythrocyte filterability) was less in both the patient group and the group of RFSP and there is an accompanying increase of erythrocyte aggregation. Blood viscosity and fibrinogen level were higher in the patient groups. There was no correlation between erythrocyte deformability and other hemorheological parameters. Our results suggest that significant alteration of erythrocyte biophysical properties may be implicated in the pathogenesis of ischemic stroke. These abnormalities are associated with some of the risk factors of stroke such as hypertension, atherosclerosis and cardiovascular disorders.

[Dot-ELISA in the diagnosis of neurocysticercosis].

The dot-enzyme-linked immunosorbent assay was used to detect Cysticercus cellulosae antibodies in sera of patients with neurocysticercosis. Among 108 confirmed cases of neurocysticercosis 81.6-96.1% showed positive reactions. Two out of 54 normal control sera reacted at a serum dilution of 1:20, but none at a 1:40 (range 40-640). No cross reactions were observed with sera from cases of paragonimiasis and clonorchiasis, but it did occur to some extent with sera from cases of echinococcosis and cerebrovascular diseases. The results indicated that the dot-ELISA was sensitive, specific and economic for the diagnosis of neurocysticercosis.
