Influence of climatic and management factors on Eimeria infections in goats from semi-arid zones.

A survey of Eimeria infections was performed in dairy goats and kids (<6 months old) of six farms from a dry desert area of Gran Canaria Island (Spain). The number of oocysts per gram of faeces (OPG) was determined by a modified McMaster technique over a total of 2,616 individual faecal samples taken from the rectum in monthly intervals. Eimeria oocysts were found in 96.1% of the samples with OPG ranging from 1 x 10(2) to 1.4 x 10(6). Kid goats had significantly (P < 0.001) higher OPG counts (46,496 +/- 5,228) than dairy females (2,225 +/- 287). Eight Eimeria species were identified, with Eimeria ninakohlyakimovae (30.0%), Eimeria arloingi (28.6%) and Eimeria alijevi (20.5%) being the most frequent species followed by Eimeria caprina (9.1%), Eimeria christenseni (4.5%), Eimeria jolchijevi (3.4%), Eimeria caprovina (3.2%) and Eimeria hirci (0.7%). Although significant differences were observed among goat groups and herds, the eight species were present in the six farms in both dairy goats and kids. The intensity of oocysts shedding was related to some factors such as the size of the herd and was further influenced by the prevailing climatic conditions of the area. The highest OPG counts were recorded during the hot season in dairy goats and close to weaning time in kids reared in small farms having no prophylactic treatments against eimeriosis.

Comparison of quantitative PCR and antigenemia in cytomegalovirus infection in renal transplant recipients.

UNLABELLED: Cytomegalovirus infection is a common complication of renal transplantation. Antigen pp65 levels serve as indicators of viral load, although the technique is difficult to perform and interpret. We sought to determine whether quantitative PCR had a higher sensitivity and predictive value in CMV infection. METHODS: The study included 100 renal transplant recipients who were screened for IgM and IgG at the time of admission. On days 7, 15, 30, 45, 60, 75, 90, 120, 180, and 360, antigenemia tests were performed on blood (pp65) and urine, and a quantitative PCR on blood. Among 59 patients recruited between November 2003 and August 2004 the mean age was 54.5 +/- 12.9 years. Two patients did not reach 90 days follow-up (3%); four patients have not surpassed 90 days (7%); 22, 120 days (37%); and 31, 180 days (53%). Ninety-three percent of patients showed anti-IgG CMV-positive titers with all being IgM CMV-negative at baseline. The patients at risk for infection were given valgancyclovir as prophylaxis throughout the study. RESULTS: At 474 visits, 8 samples (2.4%) were positive with urine; 5 (1.4%) with pp65, and 15 (4.7%) with PCR. Among the 15 positive samples, two (>100,000 and 3250 copies) revealed agreement of positive IgM and shellvial test on urine; two (15,100 and 5670 copies), antigen pp65 1+; one (17,400 copies) with pp65 2+ and shellvial urine; two (99,400 and 28,300 copies) with pp65 1+ and shellvial urine; and eight remaining determinations, 749, 2250, 686, 928, 2250, 26600, 777, and 2790 copies. The rest of the tests were negative. CONCLUSION: The preliminary results of this study demonstrated that quantitative PCR was a useful rapid tool for diagnosing and monitoring CMV infections.

Occurrence of Pneumocystis jiroveci pneumonia after allogeneic stem cell transplantation: a 6-year retrospective study.

Pneumocystis jiroveci pneumonia (PCP) has become a rare opportunistic infection due to the efficacy of prophylactic regimens. We conducted a 6-year retrospective study at our institution. A total of 13 cases of PCP were diagnosed among 519 patients undergoing allogeneic hematopoietic stem cell transplantation (HSCT) (2.5%). In three patients, PCP occurred within the first 5 months following HSCT. These severely immunocompromised patients were receiving prophylaxis and had concomitant aspergillosis that caused rapid death in two of them. In 10 other patients, PCP occurred a median of 14.5 months after HSCT. In all these patients, PCP prophylaxis had been discontinued, mainly because of the suspected bone-marrow toxicity of the prophylactic regimen. Median CD4+ T cell count was 131/microl at diagnosis. Seven of these 10 patients were receiving immunosuppressive therapy for chronic graft versus host disease and three had a relapse of their hematological malignancy. One patient died from PCP despite high doses of cotrimoxazole. We conclude that PCP is still occurring after allogeneic HSCT, mainly as a late complication in patients in whom PCP prophylaxis had been prematurely discontinued. Long-term PCP prophylaxis should be maintained in patients receiving immunosuppressive drugs, and in those with low CD4+ T cell counts or a relapse of their hematological malignancy.

[Immunomodulatory treatment with beta-interferon in patients with cervical intraepithelial neoplasia and human papillomavirus infection: long-term follow-up].

The aim of this study was to evaluate the long-term efficacy of beta-interferon treatment in the management of cervical intraepithelial neoplasia associated with human papillomavirus (HPV) infection in a selected group of patients. Thirty-nine patients who had histologically proven cervical intraepithelial neoplasia I or II with concurrent HPV infection were administered 27,000,000 IU of intramuscular beta-interferon. Periodical controls were carried out based on Papanicolaou smears and/or colposcopic examination and DNA HPV determination was performed by hybridation test, for a maximum of 8 years' follow-up. An initial complete response rate of 75% was obtained, with a recurrence rate of 25%, cervical intraepithelial neoplasia with late manifestation was of higher grade. Treatment interruption due to side-effects was not necessary. Associated viral infections appeared to be a recurrence risk factor, but not smoking or parity. While immunomodulatory treatment with beta-interferon has good long-term results in cervical intraepithelial neoplasia I/II treatment, wider randomized studies are required to obtain conclusive results.

[Cytomegalovirus viral load and antigenemia in the monitorization of prophylactic treatment with valganciclovir in renal transplant recipients].

Cytomegalovirus (CMV) disease is one of the most relevant infectious complications in solid organ transplant, and we must perform an appropriate prophylactic intervention. The goal of this study was to evaluate the effectiveness of prophylactic treatment with valganciclovir in renal transplant recipients in the first three months post transplantation by shell vial urine culture assay, and by measuring antigenemia (pp65) and CMV viral load, the latter by PCR. The population of the study included 100 renal transplant recipients. We analyzed the results of 36 patients recruited between November 2003 and July 2004 who were receiving a prophylactic oral treatment with valganciclovir, and who had finished the follow-up period of 90 days. The three tests mentioned above were performed on days 7, 15, 30, 45, 60, 75 and 90. No positive antigenemia was detected, the virus was cultured in a urine specimen and, in one patient, three measurements of viral load in serum were positive. Preliminary results of the study suggest that universal chemoprophylaxis with valganciclovir is effective for the prevention of CMV infection in renal transplant recipients and that, although all three tests used were useful, the measurement of CMV viral load seems to be the most appropriate method for monitoring these patients.

Ninety-six-week efficacy of combination therapy with lamivudine and tenofovir in patients coinfected with HIV-1 and wild-type hepatitis B virus.

We describe 6 patients who were coinfected with human immunodeficiency virus (HIV) type 1 and wild-type hepatitis B virus (HBV), in whom complete and sustained antiviral activity against wild-type HBV strains was attained during 96 weeks of combination therapy with lamivudine and tenofovir. The use of combination therapy with lamivudine and tenofovir for the treatment of HBV infection is very promising in the treatment of HIV/HBV coinfection.

Genetic variability in cysteine protease genes of Haemonchus contortus.

To increase the existent genetic variability in cysteine proteases, a polymorphism study was performed in Haemonchus contortus by comparing 2 different strains of the parasite: North American (NA) and Spanish (SP) strains. For this purpose, the polymorphism of 5 previously reported genes (AC-1, AC-3, AC-4, AC-5 and GCP-7) were analysed by PCR-SSCP and sequencing procedures. Based on the SSCP results, a total of 20 different alleles were identified for the 5 loci assessed. Except locus AC-5, all the loci were polymorphic. Loci AC-1, AC-3, AC-4 and GCP-7 showed 5, 8, 2 and 4 alleles, respectively. The allelic frequencies ranged from 0.0070 to 0.8560 and were significantly different between strains. In addition, nucleotide diversity analyses showed a significant variation within and between strains. The variations in the nucleotide sequence of the different alleles were translated in some cases into changes in the amino acid sequence. Evidence of genetic variability in cysteine proteases from two different strains of H. contortus for the same set of genes had not been previously reported.

[Haemophilus influenzae and betalactam resistance: description of bla TEM gene deletion].

The resistance to betalactam antibiotics in a total of 177 clinical isolates of Haemophilus influenzae, mostly from respiratory tract samples and characterized by their betalactamase production, was studied using the cephalosporin chromogenic assay and by detecting bla(TEM) and bla(ROB) genes. A substantial number of clinical isolates carrying the bla(TEM) gene, which presented a mutation consisting of the absence of a fragment of 136 base pairs, located upstream from the coding region including the 35 but not the 10 region of the promoter were found. This suggests that a new bla(TEM) promoter exists in these strains. This finding was associated with increased resistance to the antibiotics cefaclor and loracarbef compared with normal isolates. It was also found that 3.9% of the isolates carried the bla(ROB-1) gene.

[Prognostic significance and clinic utility of serum and immunohistochemical cathepsin B levels in colorectal cancer].

BACKGROUND: Aproximately one third of node-negative colorectal cancer recur suggesting the presence of micrometastasis not detected by conventional histopathologic methods. We think that the role of enzymes like Cathepsin B play in the process of invasion and metastasis in colorectal cancer might identify at earlier stages patients with high risk of shorter survival and who need more aggressive treatment. Our porpuse is to evaluate the prognostic significance of preoperative serum and inmunohistochemical levels of cathepsin B to identify colorectal carcinomas with worse prognostic. METHODS: Fifty five patients undergoing surgical treatment for colorectal cancer from 1998 to 2000. As a control group sera from 23 patients with acute appendicitis. Serum levels of cathepsin B were obtained preoperatively (KRKA, Novo Slovenia;ng/ml); cathepsin B inmunoreactivity was determinated after surgical treatment, (C-19, Santa Cruz Biotechnology). Serum levels of CEA (Inmulit 2000 CEA), and CA 19,9 (Inmulite Gi-Ma, Diagnostic Products Corporation, Los Angeles, CA), and p53 expression (Dako) were determinated in patients with colorectal cancer. Survival analysis was realized using Cox and Kaplan-Meier methods (SPSS 10.0 for Windows). RESULTS: The mean age of patients with colorectal cancer was 68 years (range 39-87 years). 29 males and 26 females. Tumor size was 4.6 cms., range 1-12. Rectal localization, 32.2%. Moderately differentiated, 49.1%. The median serum and inmunohistochemical levels of cathepsin B were 5.74 ng/ml and 29.56% in patients with acute appendicitis respectively. Preoperative serum levels in patients with colorectal cancer were: CEA, 46.04 ng/ml (range 0.21-7.32 ); CA 19,9, 110.52 UI/ml, (range 2.5-1920); and Cathepsin B, 6.94 ng/ml, range 3.57-11.6). Inmunohistochemical results were: p53, 44.36%, (range 0-95); Cathepsin B, 66.9% (range 10-90). Serum and inmunhistochemical values were significantly increased in patients with colorectal cancer when compared with control group, p=0,011 and p=0,000. High serum levels of cathepsib B were significantly associated wiyh shorter survival of patients with colorectal cancer in univariate and multivariate methods, p=0.041;HR 1.281 95%CI (1.043-1.716) and p= 0.022; HR 1.338.955 CI (1.043-1.716). CONCLUSIONS: Cathepsin B can be used like an independent prognostic tumoral marker in colorectal cancer. Preoperative serum levels over 6.94 ng/ml, are associated with worse prognostic and shorter survival.

[Resistance of HIV-1 to antiretroviral drugs in Valencia (Spain): mutations and susceptibility].

Resistance of HIV to antiretroviral drugs was studied in 210 samples taken in the last two years from patients at the Molecular Biology Unit of the Microbiology Department of the Hospital La Fe in Valencia, Spain. Once the viral load in plasma was determined, resistance was detected using complete gene sequencing for protease until position 3464 of the HIV-1 inverse transcriptase gene. The results were analyzed using the programs Omiga 1.2 (Oxford Molecular Group) and HR-ASAP 1.0 (Stanford University). The protease inhibitors the least affected by the presence of mutations leading to resistance were amprenavir (68.96% activity), and lopinavir (70.69% activity), and of the inverse transcriptase inhibitors, tenofovir (94.02% activity), D4T (74.62% activity) and 3TC (76.12% activity). The treatment combination with the greatest activity, based on the different mutations, was D4T + 3TC + NNRTI. To justify the persistence of viremia with relatively low genotypic resistance to antiretroviral drugs other variables must be considered, such as treatment compliance and the pharmacokinetics of the drugs.

Use of voriconazole to successfully treat disseminated Trichosporon asahii infection in a patient with acute myeloid leukaemia.

Trichosporon spp. is an emerging fungal pathogen in immunocompromised hosts, and disseminated infection is often fatal in neutropenic patients. Reported here is a case of disseminated infection in a neutropenic patient with acute leukaemia. After failure of amphotericin B and fluconazole therapy, the course of infection dramatically improved with voriconazole treatment. A literature search revealed 69 additional cases of disseminated Trichosporon spp. infections in neutropenic patients, and these are also reviewed. Clinical symptoms that suggest infection include fever, disseminated papulopustular cutaneous lesions and pulmonary involvement. Despite treatment with antifungal agents (amphotericin B, fluconazole), 78% of patients died. Voriconazole may represent a promising therapy for this life-threatening infection.

Detection of Microsporidia, cryptosporidia and Giardia in swimming pools: a one-year prospective study.

In order to estimate the rate of microsporidia, cryptosporidia and giardia contamination of swimming pools, sequential samples of water were collected during a one-year period in six different swimming pools in Paris, France. Fourty-eight samples were submitted to filtrations. Eluates were examined for microsporidia using polymerase chain reaction (PCR) and for cryptosporidia and giardia using immunofluorescence staining. One of 48 specimens was positive for microsporidia. Using DNA sequence analysis, unknown microsporidia species were identified, which were close to an insect microsporidia Endoreticulatus schubergi. One sample was positive for cryptosporidia and none were positive for giardia. This study shows a low level of swimming pool water contamination by microsporidia, cryptosporidia or giardia, demonstrating the efficacy of cleaning filtration and disinfection procedures used in French swimming pools.

[Severe sickle cell anemia revealed in an adult by medullary necrosis].

INTRODUCTION: Drepanocytosis, even severe, may only be discovered in adults further to severe complications. OBSERVATION: A 44 year-old woman was hospitalised for a severe vaso-occlusive crisis, revealing a drepanocyte syndrome with heterozygote S-beta zero thalassemia. Evolution was marked by severe anaemia, the aregenerative nature of which, uncommon during vaso-occlusive crises and the absence of Parvovirus B19 infection, led to the diagnosis of medullar necrosis. Evolution was rapidly improved after transfusion of erythrocyte concentrations and symptomatic treatment of the pain. COMMENTS: Medullar necrosis is a rare entity with multiple causes. In severe drepanocyte syndromes it is concomitant to a severe vaso-occlusive syndrome, resulting from medullar ischemia due to specific microvascular damage.
