[Measurement of flow in the Cordis-Hakin type ventricular shunt valve, used in the surveillance of hydrocephalic patients with shunts. An experimental study (author's transl)].

Using isotopes, Benabib et al. were able, in 1970 and following the work of Descuns et al., to evaluate the function of a ventricular shunt system be measuring the flow passing through it. The experimental study carried out confirmed the applicability of the method described by Benabib et al. to the Cordis-Hakim valve after substitution of the 131I used by those authors by 99mTc O4. Measurement of flow is valid only if true flow lies between 0.085 ml/mn and 1.7 ml/mn. Although with flow rates of less than 0.07 ml/mn the values obtained are overestimates, the distinction between no flow and slight flow remains definite.

[Role of folic-acid deficiency in deficiency diseases of the nervous system. Apropos of 12 cases including an anatomo-clinical case].

The authors report 12 cases of neurological syndromes due to folic acid deficiency, due in 8 cases to chronic alcoholism. In 5 cases there was polyneuritis, 3 cases had cerebellar atrophy, whilst 4 patients had subacute combined degeneration of the cord. Folic acid deficiency occurred alone in five cases out of twelve, as in 3 cases vitamin B1 deficiency was associated, and in four cases there was malabsorption of vitamin B12. A neuropathological study of these cases showed: 1) moderate involvement of the mamillary tubercles as observed in deficiency encephalopathies. 2) severe peripheral nerve involvement especially of axonal type. 3) involvement of the anterior horns of the spinal cord with appearances of central chromatolysis and a few atrophic neurones.

[Alexia without agraphia, colour "agnosia" and right visual field defect: a syndrome of the posterior cerebral artery (author's transl)].

Two cases of alexia without agraphia are reported, with differences in the characters of the alexia and in the accompanying signs. The visual field defect was limited to quadranopia in both cases which was upper quadrantic and mild in one, lower quadrantic and severe in the other. These differences make it unlikely that a similar anatomical location of the lesion and a unique mechanism are responsible of all cases of alexia without agraphia. Persistance of visual inputs in the right visual field is partial in our cases, but alexia without hemianopsia does exist. These facts are against the mechanism of "splenio-cortical" disconnection proposed by Geschwind as the only explanation. A "cortical", or agnosic mechanism has been also claimed, with lesions of the lingual and fusiform gyri on the dominant side, as well as "subangular", mechanism, with intrahemispheric disconnection by a lesion located between the visual associative areas and the angular gyrus: in these last two situations an hemianopsia is not necessarily associated to the alexia without agraphia.

[French epidemiological bases for the treatment of dementia syndromes and cognitive impairment in the elderly].

Epidemiological studies of Dementia have major methodological problems, particularly in France: selection of the studied sample, difficulties for contact with elderly people, heterogeneity of place of residence, inaccuracy of the diagnostic procedures. In spite of these problems, the Paquid Research program allowed to give estimations of prevalence and incidence rate of dementia in the French population. On the basis of these results, one can estimate that there are actually about 360,000 prevalent cases of dementia in France, of whom 270,000 are of Alzheimer type. There were also 150,000 annual-incident cases of dementia. However, many of these cases were not considered as demented by the medical system because of the absence of cognitive complaints.

[Epidemiology of memory disorders].

To study the predictive value of memory complaints resulting in consultation with a GP as to the risk of subsequent dementia, we analysed the data from the follow-up of the Paquid cohort study carried out around Bordeaux. A sample of 1503 elderly people over 65, living at home and non-demented at the baseline screening, was considered. The risk of dementia was measured two and four years after the baseline screening: 60.8 per cent of subjects themselves perceived a memory impairment and 15.6 per cent expressed this complaint to their GP. Forty-eight developed a subsequent dementia. Taking the group without memory complaint as the reference set, three groups of elderly people could be recognized as at high risk of dementia: subjects with self-perceived memory impairment, consulting a GP with low memory performance or with normal memory performances, and subjects with memory complaints not resorting to a GP but with low memory performance. In non-demented elderly people, memory complaints expressed to the GP may be a strong predictor of dementia and should not be neglected.
