[Determination of the ejection fraction of the left ventricle by videodensitometric analysis of digital angiography. Preliminary results].

Digitalisation enables angiocardiography to be performed by a peripheral intravenous injection. Computer-assisted analysis of the date widens the possibilities of quantification. The authors have developed a videodensitometric method of studying the left ventricular ejection fraction. The research was performed on an experimental model and the technique validated in a series of 10 patients. The experimental model consisted of a series of balloons which, when inflated with contrast medium assumed an allipsoid shape resembling a left ventricle. The balloons were blown up in two stages with an automatic injector to simulate systole and diastole. The images were recorded in the same way as during ventriculography. Videodensitometric measurements showed 3 to 5% variations from the true values. The method was then applied to the calculation of the left ventricular ejection fraction in 10 patients: left ventricular function was also quantified by geometrical methods (Dodge) from the same angiogrammes and the 2 sets of results were then compared. The correlation coefficient between the two methods was 0.97, so validating the new technique. Videodensitometry opens up new perspectives in the study of left ventricular function. On the other hand it can be used to monitor the ejection fraction in severely ill or recently operated patients, and, on the other hand the principle of videodensitometry eliminates the geometrical approximations inherent in the classical methods of angiographic analysis and would therefore seem to be more suitable for the study of pathological left ventricules (aneurysm...). Finally, the technique of videodensitometry represents a new step towards the measurement of true volumes and flow rates.

[Effects of the argon laser on atheromas. Preliminary study on post-mortem arterial samples].

The effects of argon laser radiation of human atheroma were studied in vitro. Lesions produced were craters from total tissue volatilization surrounded by a thin zone of coagulation. The degree of tissue destruction was related to the energy used and the direction of the laser beam in relation to the atheromatous stenotic lesion. These findings confirm that it is possible to destroy the atheroma under controlled conditions, but at the non-negligible risk of perforation. By allowing visual control, the angioscope should ensure increased safety in use and provide hope for future applications of this method by the percutaneous route in the treatment of established atheromatous disease.

[Results of in situ arterial thrombolysis by the combination of urokinase and lysyl plasminogen in acute arterial occlusive diseases of the lower limbs].

35 patients with acute arterial occlusions [27] and graft thromboses [8], responsible for severe and recent ischemia, were treated by fibrinolytic therapy (Urokinase: 1 000 units/kg/hour, and Lys Plasminogen). These drugs were delivered at the site of occlusions using a 5 French catheter. Angiographically, initial success was obtained in 30 patients (85%) and a significant clinical benefit persisted 5 months later, in 20 patients (57%). 4 distal embolisms during the treatment were noted, and one woman died a few hours after the withdrawal of an axillary catheter of a cerebellar infarction. Only two minor (6%) and one severe (3%) groin hematoma were encountered. No patient had at any moment a fibrinogen concentration lower than 1 g/l. Thus, the thrombolytic treatment used in the study appears as effective as locally administered Streptokinase but better tolerated.

[Percutaneous endoluminal angioplasty of the supra-aortic arterial trunks. Immediate and remote results].

Twelve tight stenoses (9 of the subclavian artery, 1 of the axillary artery and 2 of the brachio-cephalic artery) were dilated with immediate satisfactory results in all cases. Only one complication (regressive lesion of the median nerve) was recorded. Eight patients were followed-up for a mean period of 13 months: all were symptom-free, and digital angiography by the intravenous route showed a stable image of patency. From a review of published dilatations of the subclavian artery it appeared that the method is 95% effective, that its mortality is nil and that no complication, notably embolism, has been reported. In view of the risks involved in surgical procedures and of the advantages of percutaneous transluminal angioplasty, at least in theory, this technique seems to be a suitable alternative to surgery in stenoses of the subclavian arteries.

[Intra-arterial thrombolysis with the combination of urokinase and lysyl-plasminogen. 27 cases of acute arterial obliteration of the lower limbs].

Twenty-five in situ thrombolysis using Lysyl Plasminogen and low doses of urokinase were performed on 25 acute, recent and severe arterial occlusion of lower limbs. Early success were 56% and 44% with a follow up of 5 months. Complications were very limited. Thus the thrombolytic treatment used in this study appears as effective as locally administered streptokinase but higher tolerated. It seems to be able to win one of the best places in the treatment of the arterial disease of the lower limbs.

[Contribution of digital intravenous angiography in the assessment of the postoperative results after surgery of the vessels of the neck].

Forty-six patients who had undergone neck vessel surgery were investigated postoperatively by intravenous digitalised angiography. In 30 patients an endarterectomy had been performed and in one case the examination did not provide satisfactory opacification of the carotid vessels. In 29 cases the result was of satisfactory quality: the appearance of the operative site was normal in 21 patients, dilated in 3, irregular in 7 and stenosed in 1. Twenty patients had undergone a bypass. There was one failure and 19 results suitable for analysis. The bypass was normal in appearance and permeable in 14 cases, irregular in 1, dilated in 2 and obstructed in 2. Overall, intravenous digitalised angiography provided satisfactory assessment of the operative result in 96% of patients.

[Value of digital angiography via a vein in the diagnosis of an atheromatous aneurysm of the subrenal aorta ruptured into the inferior vena cava].

Fistulisation of an atheromatous aneurysm of the aorta into the inferior vena cava is a rare type of rupture. Emergency surgery is required following its diagnosis. A case of this type is reported where a diagnosis was obtained without delaying surgery using a new radiological technique: numerical angiography via a venous approach. By virtue of its simplicity, reliability and safety, this technique is of great value in the emergency investigation of rupture of an atheromatous aneurysm of the aorta.

Transluminal angioplasty of failing infrainguinal arterial by-pass grafts: initial and long-term results in 13 patients.

Thirteen stenotic infrainguinal arterial bypasses (12 venous, 1 Gore-tex graft) were treated by transluminal angioplasty, either percutaneously (10 patients) or surgically (3 patients). Eleven procedures were immediately successful (two at the proximal portions of femoropopliteal grafts, six near the distal anastomoses, and three at the distal parts of femoroinfrapopliteal grafts) and dilated stenoses are still patent with a mean duration of 24 months in all patients except 2 who died during the follow-up period. The calculated cumulative patency rate is 85% at 36 months. Two procedures were followed by immediate disruption near the distal end of an in situ saphenous bypass graft where balloon inflation was performed. These required immediate surgical repair. Dilatation of the distal ends of in situ saphenous femoropopliteal bypasses may not be as safe as in other locations.

Relief of buttock claudication by percutaneous recanalization of an occluded superior gluteal artery.

We report a case of a woman presenting with right severe buttock claudication and normal neurological and osteoarticular examination, in whom a guidewire recanalization and percutaneous transluminal angioplasty (PTA) of an occluded right superior gluteal artery (SGA) has provided relief of her symptoms. To our knowledge, this is the first report of percutaneous recanalization of the SGA. PTA can be considered the treatment of choice for buttock claudication caused by SGA stenosis or occlusion.

Feasibility, safety, cost-effectiveness and 1 year follow-up of coronary stenting without predilation: a matched comparison with the standard approach.

BACKGROUND: We evaluated the feasibility, safety, procedural cost-effectiveness, radiation dose and time and 1-year target vessel revascularization rate of direct unprotected mounted stenting without previous balloon dilatation (DS) in native coronary artery lesions. METHODS: DS was attempted in 119 patients; 39 had a recent myocardial infarction, 62 had unstable angina, and 18 had stable angina. The clinical follow-up was obtained at 14+/-5 months (range 6 to 24 months). These results were compared with those for a consecutive group of 160 patients matched for type and length of lesions and who had a stent only if the post-balloon residual stenosis was >30%. RESULTS: The feasibility of DS was 112/119 (94%). The number of inflations, the length of the stent/length of the lesion ratio, the time and the dose of radiation were dramatically lower in the DS group (P<0.001). DS conferred a slight reduction in procedure-related cost [$820+/-157 for DS vs. 894+/-427 for standard dilatation (SD) per patient]. The 1-year target vessel revascularization rate was similar in both groups [nine (8%) for DS vs. 17 (11%) patients for SD, ns]. CONCLUSIONS: DS is feasible and safe in selected coronary lesions. This method provides a low rate of repeat revascularization and reduces the time and the dose of radiation compared with the standard approach.

MR evaluation of chronic aortic dissection.

Thirty patients with suspected or known chronic aortic dissection were imaged with magnetic resonance (MR), CT, and angiography. Five of these patients had previously undergone surgical repair of the ascending aorta for a type A dissection. Magnetic resonance demonstrated an intimal flap and a double lumen in 25 cases. In four cases with a thrombosed false lumen, proved angiographically, an intimal flap and double channel were not seen. In two of four aortic dissections with a thrombosed false lumen, CT made the diagnosis by showing displaced intimal calcifications not visualized on MR. In one case the aortic dissection was made on CT and angiography but was not supported by MR which showed an aortic aneurysm, subsequently confirmed at surgery. Magnetic resonance, CT, and aortography differentiated between type A (nine patients) or B (20 patients) dissection in all cases and demonstrated extension into the abdominal aorta. Extension into the iliac arteries was seen on MR in three patients but missed in nine patients. Magnetic resonance differentiated the true and false lumen in all but one case. Thrombosis of the false channel was identified in four cases by a decrease in signal intensity on the second echo image. Cardiac gating and longitudinal contiguous sections seemed to be more suitable for appreciation of the relationships with arch vessels. Transverse contiguous slices allowed determination of the origin of celiac, mesenteric, and renal arteries from either the true or the false lumen. This study confirms that MR is an accurate and noninvasive method for the evaluation and follow-up of chronic aortic dissection, obviating the need for iodinated contrast media.

[Comparative study of the carrying out of iliac angioplasty with digital and conventional angiography. Impact on real (consumable) costs].

Differences in time and cost were evaluated between the performance of iliac angioplasty with conventional (AC) and digital subtraction (AN) angiography, after a total of 27 angioplasties (13 with AC and 14 with AN). Excluding amortization of material and personnel costs, findings confirmed a certain number of advantages for AN: gain in time of about 34%, decrease of about 14% in charges, and notably of 83% in expenditure on films and 50% on contrast media.

[Anatomo-histological consequences of arterial dilatation using the balloon catheter. Preliminary studies in the rabbit].

Histopathologic changes were evaluated after arterial dilatation in a group of 8 rabbits. Rabbits were given an atherogenic diet and then aortic disendothelialization performed using a Fogarthy catheter. This was followed three to five months later by percutaneous angioplasty using a balloon catheter. Animals were sacrificed and histology of arterial segments determined. Findings showed thickening of intima with lipophagic infiltration. In contrast, the media was greatly altered with rupture of internal elastic fibers and a true cleavage plane suggestive of subadventitial dissection: the adventia was not modified.
