[A new instrument in eyelid surgery: ptosis forceps with a protected lock].

We report on a new instrument in eyelid surgery: the ptosis forceps with a protected lock. This new device helps the surgeon to dissect the levator muscle to facilitate any type of levator surgery. It is particularly useful in the adjustment of the superior lid margin level towards the corneoscleral area in ptosis surgery with levator resection.

[Analysis of the efficacy of early probing in the treatment of symptomatic congenital lacrimal duct obstruction in infants. Apropos of 92 cases].

INTRODUCTION: Nasolacrimal duct obstruction (NLDO) is a common clinical problem (5 to 6% of newborns). Dacryostenosis is a partial or complete block in the nasolacrimal duct. PATIENTS AND METHODS: The records of 68 patients (92 cases) of NLDO with/without mucocele were reviewed. The probing was performed early and according to the same technique. RESULTS: The age at the time of probing ranged from 0 to 7 months (mean 4.6 months). The cure rate was 91.3% and the failure rate was 8.70%. DISCUSSION: Our results are consistent with other reports in the literature. The cure rate was higher with a earlier probing and was good after a second tentative of probing. A multiple obstruction of nasolacrimal system was cause of a failure. This was not the case with mucocele. The optimal timing of the probing is discussed in this report and we preferred to perform the probing to 4 at 6 months. CONCLUSION: We showed a high cure rate with early probing in our series. These results should be confirmed by a prospective randomised study (in process in our institution).

[Fetal anophthalmos. Problem of the prenatal diagnosis: a case report].

We report a case of anophthalmia discovered after birth and discuss the need to resolve diagnostic difficulties and determine prognosis. This is an exceptional malformation which is particularly difficult to manage. It often occurs in the context of a complex malformation syndrome. Recent imaging techniques including magnetic resonance imaging help determine the degree of malformation and provide useful information for giving genetic advice to parents.

[Postoperative ptosis: etiopathogenesis, clinical analysis, and therapeutic management. Apropos of a series of 43 cases].

PURPOSE: Acquired postoperative ptosis (PP) are difficult to situate in the current classification of ptosis. Assessement of the mechanisms, the clinical features and the possible treatments of these PP would suggest a new classification of ptosis. MATERIAL AND METHODS: Among 260 cases of surgically corrected ptosis, 43 cases of PP (16.5%) were detected and analyzed. RESULTS: Forty cases of PP were eligible for this study. Their responsible mechanisms were aponeurotic (57.5%), mixed (aponeurotic and/or myogenic and/or neurogenic) (27.5%) and myogenic (15%). PP was assessed in most cases as being mild (77.5%) and the levator's muscle contraction was most often mildly impaired (77.5%). In these cases, surgical procedure was performed: levator aponeurosis disinsertion repair (85%), Fasanella-Servat procedure (2.5%), frontalis sling (2.5%) and other surgical procedure with synthetic materials (10%). Postoperative complications included 1 case of persistent lid edema and 4 cases of spontaneous suture rupture. Six patients (15%) were secondarily reoperated: 2 for overcorrection (5%) and 4 for undercorrection (10%). The general outcome was good in 90% of cases, insufficient in 5% of cases and unsatisfactory in 5% of cases. CONCLUSION: This study confirms the previously described features of the PP: onset after anterior surgery procedures of usually moderate ptosis, induced by an aponeurotic defect mechanism in most cases. The treatment was exclusively surgical: anterior reinsertion of the levator aponeurosis. For better management, we suggest a new ptosis classification: aponeurotic, myogenic, neurogenic and mixed (aponeurotic and/or myogenic and/or neurogenic) and false or pseudo-ptosis.

[Acute ocular dryness].

Acute ocular dryness is a morbid condition which occurs when the cornea is suddenly deprived of its natural protection by the eyelids. The reasons for acute ocular dryness are numerous, but superior eyelid trauma often leads to the development of such a pathology. Exposure keratitis is subsequently observed if antibiotics and trophic eyedrops are not prescribed rapidly. An adaptated treatment according to the aetiology must be provided in a department specialized in ophthalmology.

[Analysis of the efficacy of the large mono-canalicular intubation stent in the treatment of lacrimation caused by congenital obstruction of the lacrimal ducts in infants].

BACKGROUND: Bicanaliculo-nasal intubation (BCNI) is considered as the method of choice in the treatment of naso-lacrimal duct obstruction (NLDO) in children. Side effects of BCNI are possible and monocanaliculo-nasal intubation (MCNI) provides the same results with fewer complications. Nevertheless, most of the short duration monocanaliculonasal stent complications concern the migration of the silicone stent in the canaliculus. We thought that this fact may be linked with the shape of the head of the monocanaliculonasal stent and decided to alter it. METHODS: Monocanaliculo-nasal intubation was performed in 52 cases, in 41 children, using a modified silicone mono-canalicular stent (large collarette stent). RESULTS: The results were compared with a reference series of 52 cases treated with the classical silicone mono-canalicular stent intubation. The rate of success was comparable in both series, but fewer complications were observed with this new device. In particular, no migration of the silicone in the canaliculus occurred. CONCLUSION: The authors state that this modified mono-canalicular stent decreases the lacrimo-nasal intubation morbidity without altering its effectiveness.

[The eyelid of the competitive swimmer].

Two cases of pseudo-baggy eyelid localized to the medial part of the superior lid are described. This peculiar feature occurred in two teenagers girls who regularly wore hard plastic protective goggles during swimming competition. Wearing goggles is probably responsible for the development of this new, and to our knowledge, previously unreported syndrome. Clinicians should be aware of the typical signs and require about swimming practices in young women who present with an unusually baggy eyelid.

[A new biomaterial in surgery of ptosis with frontalis suspension: wide pore PTFE].

BACKGROUND: In brow suspension, there is agreement that fresh tissue (autogenous fascia lata or temporalis fascia) provides the best results in terms of low complications and duration of the effect. Yet, the morbidity of fresh tissue harvesting is not negligible. Many alternative materials have disadvantages in terms of duration of the procedure and side effects. METHODS: We have used a new material: wide porous expanded polytetrafluoroethylene (eP.T.F.E.). This material is an inert vitreous teflon alloplast with a high biocompatibility. It is close to Gore-Tex, but differs from it by a higher porosity (over 90%) and wider diameter of its pores (over 50 mus). We performed a series of 60 brow suspension from February 1992 to March 1994, using this new material. RESULTS: We did not encounter any significant complications due to the material and in circumstance, we did not deplore any migration, infection or extrusion of the ePTFE. The biocompatibility seems to be increased, as it has been demonstrated by light and electron microscopy of the implanted material who revealed that fibrovascular ingrowth was significantly achieve two months after surgery. CONCLUSION: The first results of this series are very encouraging. They can favourably be compared with the results of other series using different types of materials available in brow suspension. Though our longest follow-up is 30 months, further study is necessary to evaluate the long term results and eventual long term side effects of ePTFE, which avoid harvesting fresh tissues, thus reducing the brow suspension procedure's morbidity.

[Role of nasolacrimal intubation in the treatment of lacrimation caused by congenital lacrimal duct obstruction in infants].

We report a study of a series of 83 cases of congenital nasolacrimal duct obstruction treated with monocanaliculonasal or bicanaliculonasal silicone intubation. Efficacy, complications, as well as the duration of the placement of the probes were studied. The analysis of the various factors of this series (age of the infants at the time of intubation, duration and type of intubation, complications) demonstrated the superiority of short-term monocanaliculonasal intubation (about four weeks) compared to longer intubation.

[Large orbito-palpebral cavernous hemangioma].

Cavernous hemangiomas are benign vascular orbital lesions that exert a slowly progressive mass effect. They usually occur in adults. The authors report an unusual case of a large orbito-palpebral cavernous hemangioma with peculiar history and symptoms. The main clinical and radiological characteristics of this tumor are recalled together with the basis of surgical management.

[A conjunctival tumor of dental origin. Apropos of a case].

One case of connective-tissue tumor with a lymphomatous appearance, secondary to chronic sinusitis with a dental origin, is reported. The main ophthalmological syndromes associated with a dental and/or sinusal involvement are described.

[Supra-tarsus sulcus. Treatment by silicone prosthesis].

In the anophthalmic socket, the deep superior eyelid sulcus is a quite common syndrome; sometimes with ptosis of the upper eyelid and laxity of the lower eyelid. The treatment by a silicone prosthesis in the deep sulcus fills eyelid volume. The main advantages of this procedure are simplicity and relative harmlessness. The authors relate their technique and the results about 15 cases. The indications are discussed.
