Five year follow up of biocolonisable microporous fluorocarbon haptic (BIOKOP) keratoprosthesis implantation in patients with high risk of corneal graft failure.

AIM: To study the anatomical and visual performance following implantation of a model of artificial cornea and to evaluate the postoperative long term complications. METHODS: 11 eyes of 11 patients with bilateral corneal blindness considered as potentially having high risk of failure of penetrating corneal keratoplasty were implanted with biocolonisable Kpro keratoprosthesis (BIOKOP I, FCI, Rantigny, France) in the period between January 1996 and May 1998. Only one eye was implanted in all patients and followed up for a period of 60 months. The visual outcome, anatomical and functional stability, complications, and the general performance of the keratoprosthesis were evaluated. RESULTS: The keratoprosthesis (BIOKOP I) only 36.3% remained in position to date. In the patients' last visit five eyes (45.4%) were blind and one (9.0%) showed a slight improvement in the best corrected visual acuity (BCVA) in comparison to preoperative tests. Six eyes (54.5%) showed improved BCVA before having postoperative complications. Four eyes underwent replacement of a BIOKOP I Kpro with a BIOKOP II as a result of extrusion. The keratoprostheses remained anatomically in situ for a mean of 25.5 months and their functional performance period was limited to a mean of 22 months. CONCLUSION: Corneal keratoprosthesis (BIOKOP I, II) does not provide a stable anatomical relation with the surrounding ocular structures. Its ability to restore vision is limited to a short postoperative period in eyes implanted with severe ocular surface disease.

[Effect of tobramycin with topical diclofenac on arachidonic acid in endotoxin-induced uveitis].

PURPOSE: We studied if tobramycin associated to sodium diclofenac modifies the drug effect of the latter on arachidonic acid metabolism in an endotoxin induced uveitis (EIU) model in albino rabbits. METHODS: Experimental uveitis was induced by intravitreal injection of a 10 ng lipopolysaccharide A Salmonella typhimurium endotoxin dissolved in 5 microl saline solution in the right eye of experimental animals. We have used 48 animals (4 groups of 12 animals each). The control group (G-I) was injected with saline solution (5 microl); the endotoxin group (G-II) was injected with 5 microl of ET; group III and IV were injected with the same amount of ET and treated with topical sodium diclofenac (1%) (Group III) and tobramycin and sodium diclofenac (0.3%) (Group IV) two hours before the administration of endotoxin and then every two hours until 24 hours, when we determined E2 prostaglandin and B4 leukotriene concentration in aqueous humor obtained by paracentesis of the anterior chamber. RESULTS: We observed how the E2 prostaglandin concentration was reduced in the two treatment groups compared with the endotoxin group (p<0.01). However, there were no differences between groups III and IV (p>0.01). There was a mild increase of B4 leukotriene in both treatment groups, which was not significant in relationship to the endotoxin group (p>0.01). No differences were found between the two treatment groups (p>0.01). CONCLUSION: Using the association of tobramycin does not affect the action of sodium diclofenac in arachidonic acid metabolism in endotoxin induced uveitis.

[Retinal detachment surgery after refractive surgery].

PURPOSE: To analyze the characteristics of retinal detachments (RD) in myopic patients who underwent different kinds of refractive surgery (RS). Modifications in patient eyes deriving from RS and how it hindered surgery for treating RD, was also studied. MATERIAL AND METHODS: 14 (6 female/8 male) patients with RD after having been treated with RS (laser assisted in situ keratomileusis (LASIK), photorefractive keratectomy (PRK), radial keratotomy (RK) and phakic anterior chamber lenses). RD was treated by scleral surgery and in two cases by vitrectomy after first RD surgery. RESULTS: 12 RD cases showed slight modification over previous refraction. In 2 cases vitrectomy was necessary since proliferative vitreoretinopathy was evidenced. The appearance of RD cases did not increase after PRK, RK or LASIK. RS did not hinder RD surgery. RD appearance in patients with anterior chamber lenses was higher, with severe difficulty in peripheral exploration and in performing RD surgery. CONCLUSION: After PRK, RK and LASIK, RD surgery does not show more difficulty, while after anterior chamber intraocular lenses implantation there is an increased difficulty for RD surgery.

[Surgical management in subfoveal choroidal neovascularization in high myopia. Analysis of results].

PURPOSE: To analyse results obtained from treatment of choroidal neovascular membranes (CNM) in patients with high myopia after vitrectomy and CNM extraction. MATERIAL AND METHOD: A prospective study including 9 eyes (9 patients) suffering from high myopia (higher than 6 diopters of myopia and/or axial length of more than 26 mm) and subfoveal CNM treated with vitrectomy and membrane extraction. Mean age was 43.11 S.D. 9.02 years of age (range: 32-58). Mean follow up time was 28.44 S.D. 7.65 months (range: 14-39 months). RESULTS: Preoperatory best corrected visual acuity (BCVA) was 0.09 S.D. 0.05 (range: 0.01-0.2). Postoperatory BCVA resulted in 0.15 S.D. 0.09 (range: 0.1-0.4). Differences did not show statistical signification (p= 0.09, Student T test). One case of membrane recurrence was observed, one case developed increased intraocular pressure and cataract requiring extraction; another presented increased intraocular pressure in the early postoperative period. CONCLUSION: Extraction of CNM in high myopia using vitrectomy does not show significant improvements in BCVA.

Prevention of secondary cataract by antimitotic drugs: experimental study.

To prevent the development of secondary cataract, mitomycin C (MC, 0.2 mg/ml) and 5-fluorouracil (5-FU, 50 mg/ml) were introduced in the irrigating solution (BSS Plus) during an extracapsular lens extraction in albino rabbits. Cellular multiplication in the whole lens capsule was quantified by means of flow cytometry. The total number of residual cells in the capsule after lens extraction together with the mitotic phases of the cell cycle were studied. A statistically significant difference (p < 0.005, Student t test) was obtained between the two groups treated with antimitotics when compared with the control group regarding the inhibition of mitosis in the capsular cell population together with the total cell number. However, 5-FU has shown a more potent inhibitory effect than MC (p < .0.005, Student t test). We did not record any ocular complication during a postoperative period of 2 months. MC and 5-FU could be a promising alternative in the prevention of posterior capsule opacification.

Endothelial cell changes in endotoxin-induced uveitis.

Modifications in endothelial cells density and variations in endothelial pleomorphism during experimental uveitis were studied in 20 albino rabbits after intravitreal injection of 2,000 ng Salmonella typhi endotoxin into 10 of them. The other 10 rabbits were injected with sterile saline serum. Morphologic changes in endothelial cells were also determined, using a method of endothelial vital staining. For the first time, a significant reduction in endothelial density and a significant increase in endothelial pleomorphism during endotoxin-induced uveitis is reported. The control group eyes showed an endothelial density of 3,959 cells/mm2, while the uveitis eyes showed 3,667 cells/mm2 (p < 0.05, Student's t test). A reduction in the percentage of hexagonal endothelial cells was found, from 70.4% in the control group to 64.4% in the eyes with anterior uveitis (p < 0.001, Student's t test). An increase in the percentage of pentagonal endothelial cells was also found, from 14.5% in the control group to 20.1% in the uveitis eyes (p < 0.001, Student's t test). These modifications are correlated with the morphologic appearance of the stained corneal surface, and functional implications are discussed.

Corneal concentration and systemic absorption of cyclosporin-A following its topical application in the rabbit eye.

To ascertain the corneal storage and the possible systemic absorption of 2% cyclosporin-A (CsA) eyedrops, the authors studied, by RIA with monoclonal specific antibodies, the corneal and blood levels of the drug following its topical administration on healthy albino rabbit eyes. When topical CsA was administered following a low-dose application schedule [such as a single dose of 100 microliters every 12 h for 5 days (group 1)], CsA corneal levels of 260.8 +/- 59.8 ng/ml and blood levels of 55.67 +/- 24.4 ng/ml (expressed as means +/- SD) were achieved. These levels significantly increased to 1,111.27 +/- 449.4 ng/ml (p > 0.0001) and 88.66 +/- 59.7 ng/ml (p > 0.05), respectively, when a higher dosage was used [100 microliters every 6 h for 10 days (group 2)]. It is shown that a dose-dependent corneal concentration of CsA, due to a cumulative effect, is achieved in the intact rabbit cornea following topical application of the drug. Systemic absorption of CsA, although irregular, does exist. Therefore, when using topical 2% CsA as in this study, a systemic effect of the drug (especially in low weight animals) should be considered when analyzing the results of topical application.

[Luxation of an anterior chamber lens caused by secondary serous epithelial cyst].

We report the case of a patient in whom luxation of an anterior chamber lens (implanted after extraction of a traumatic cataract) occurred due to the development of an epithelial cyst which caused decentralization of the lens and reduced visual acuity. The physiopathology of this complication of cataract surgery and the various surgical methods of treatment are discussed.

[Treatment of corneal edema using saturation of free radicals].

We report on the results obtained in the treatment of inflammation of the cornea by means of the saturation of free radicals, using chemiluminescence. The experiment was conducted on the Alloxan model of corneal inflammation caused by the freeing of free radicals in the stroma. We compared the action of steroidal and non-steroidal antiinflammatory substances, and also specific saturators of free radicals.

[Morphometry (Sketch PC) of the corneal endothelium after injection of 5-fluorouracil into the anterior chamber].

We made a study of the effect on the corneal endothelium of rabbits of the introduction of 5-Fluorouracile into the anterior chamber. An EECC was performed on 24 rabbits, using BSS-Plus for right eye and 5-Fluorouracile dissolved in BSS-Plus for the left eye as irrigation solutions. We calculated the individual endothelial cell surface and form factor, finding no statistically significant differences between the two groups.
