EDucation and eXercise for gluteal tendinopathy in an Irish context (EDX-Ireland): findings from the LEAP-Ireland feasibility randomised controlled trial.

OBJECTIVE: To assess feasibility of a randomised controlled trial (RCT) investigating effectiveness of 6 sessions of an EDucation and eXercise intervention delivered over 8 weeks (EDX-Ireland) for gluteal tendinopathy, against usual care. EDX-Ireland was modified from a 14-session EDucation and eXercise intervention (EDX), delivered over 8 weeks, previously evaluated in an Australian RCT. DESIGN: Feasibility parallel RCT. METHODS: Participants were randomly assigned to physiotherapist-led EDX-Ireland or usual care. EDX-Ireland comprised 6 sessions of education, hip abductor strengthening and functional loading over 8 weeks, supported by a home exercise programme. Primary outcomes included success of different recruitment strategies and recruitment/retention rates. Secondary outcomes measured global rating of change and other clinical outcomes. Descriptive statistics (percentage, mean, standard deviations and 95% confidence intervals (CI)) are presented. RESULTS: Of 323 individuals who expressed interest in study participation, 119 completed physical examination screening, and 65 met criteria and consented to participate (recruitment rate 55%). Sixty-five people (89% women; mean age 53.1 ± 9.3 years), were randomised to EDX-Ireland (n = 32) or usual care (n = 33). Eighty-three percent (n = 54) were recruited via social media/community, 3% (n = 2) from general practitioners and 14% (n = 9) from orthopaedic/rheumatology. Retention was 92% (95% CI 82-97%, n = 60) at 8-weeks, and 89% (95% CI 79-96%, n = 58) at 3-months. Feasibility thresholds were met. Effect size estimates indicate that 134 participants would be required for a future RCT. CONCLUSION: Pre-defined recruitment and retention thresholds were met, indicating that a RCT evaluating 6 sessions of physiotherapist-delivered education and exercise against usual care for gluteal tendinopathy is feasible. The kinetics of chick cell population aging in vitro.

We have examined the kinetics of chick cell population aging in vitro using the percentage of labeled nuclei, the number of colonies formed from a low density inoculum and the number of cells/colony to monitor culture age. The results from these studies showed a gradual age-associated decline in each of the parameters which was first detected early in the culture lifespan and well in advance of changes in total cell number at confluency. Our results also indicated that each of the above parameters, in addition to the calendar time cells had been in culture, could be used to estimate the percentage of lifespan completed by the culture. A comparison of the methods used to estimate the remaining culture lifespan indicated that the percentage of labeled nuclei was the most accurate in describing cell age.

Effect of different fetal bovine serum concentrations on the replicative life span of cultured chick cells.

The effect of Eagle's minimal essential medium, containing different fetal bovine serum (FBS) concentrations, on the proliferation and replicative life span of cultured chick cells has been studied. Our results showed that the rate of chick cell proliferation and the cell density at stationary phase increased as a function of serum concentration between 5 and 30% FBS. The replicative life span of cultured chick cells was dependent on the FBS concentration between 5 and 20% in a medium volume of 0.20 ml/cm2. The maximum replicative life span of chick cells was obtained by serially propagating cells in a medium volume of 0.20 ml/cm2 containing 20 or 30% FBS, or, alternatively, in 0.53 ml/cm2 containing 10, 20 or 30% FBS. Cells grown in medium containing 5% serum had a calendar life span of 35 days, whereas cells propagated in medium containing higher serum concentrations had a calendar life span of 50 days. These results reenforce the concept that, although the kinetics of cell population aging can be affected by the culture medium composition, the aging of cells in culture is controlled by alterations within the cell.

Alterations in chromatin functions during aging in vitro.

Age-associated alterations in the chromatin functions of human diploid cells have been observed. These alterations include: (1) a decline in the rate of histone acetylation; (2) a reduction in the rate of RNA synthesis as measured by (a) the rate of 3H-uridine incorporated into the RNA of young and old cells; (b) comparison of the template activity of isolated chromatin from young and old cells using E. coli RNA polymerase and (c) measurement of chromatin template activity using the endogenous RNA polymerase of young and old cells. It is suggested that the nondividing state of old cells may be the result of the inability to synthesize specific RNA molecules (and perhaps specific proteins) necessary for the cell to continue through the cell cycle.

Reiter's aortitis with pericardial fluid, heart block and neurologic manifestations.

A man who presented with Reiter's triad and keratoderma blenorrhagicum followed by cardiac manifestations is reported. He developed mild aortic insufficiency, A-V block and pericardial effusion. Painful ophthalmoplegia occurred 5 months later with exacerbation of the cardiac complications as corticosteroids were being tapered. Retreatment with high dose corticosteroids resulted in a remarkable remission of the neurologic complications and stabilization of his cardiac manifestations now maintained for 3 years.
