Osteonecrosis-like syndrome of the medial tibial plateau can be due to a stress fracture. MR findings in 13 patients.

OBJECTIVE: To demonstrate the contribution of magnetic resonance imaging to the elucidation of mechanisms involved in "osteonecrosis-like syndrome of the medial tibial plateau". PATIENTS AND METHODS: A magnetic resonance study with sagittal and coronal sections was done in 13 patients (age range, 57-95 years) two weeks to four months into a painful syndrome meeting the definition of "osteonecrosis-like syndrome of the medial tibial plateau". Gadolinium injection was used in nine patients. Clinical symptoms resolved within a few weeks in all 13 cases. RESULTS: T1-weighted images without gadolinium showed diffuse low signal from the epiphysis (n = 12) containing an area of even lower signal seen either as a crescent-shaped subchondral image (n = 3/12) or as a linear image (n = 9/12). On postgadolinium images, the low signal was abolished except for a line of low signal parallel to the subchondral bone. T2-weighted images demonstrated diffuse high signal from the medial tibial plateau with persistence of the line of low signal (n = 8/12). CONCLUSION: Magnetic resonance imaging allows to analyze the anatomic lesion responsible for "osteonecrosis-like syndrome of the medial tibial plateau". Our magnetic resonance findings were similar to those seen in stress fractures at other sites.

[Occult fractures of the internal tibial plateau].

Insufficiency bone fracture of the medial tibial plateau occurs in elderly patient. Pain and disability are important. Physical examination and initial X-Ray are note contributive. Insufficiency fracture appeared on bone scan as localised hyperfixation and as a slow intensity image of the medial tibial plateau on MR Imaging. Outcome is favorable with discharge.

[Clino-static syndrome].

The clinostatic syndrome described in 1953 by Pierre Lehman is defined by weakness of the lower limb in the supine position contrasting with absence or only slight disorders on standing, normal neurological examination and passive mobility of the hip. The aetiology generally corresponds to a malignant lesion of the base or roof of the acetabulum and very rarely a fracture of the acetabulum or aseptic necrosis of the acetabulum, detected by bone scan or plain X-rays. An inverse clinostatic syndrome of the lower limb due to lysis of the ischium has also been described.

Case report 728: Desmoplastic fibroma.

A 69-year-old woman with a 25-year history of recurrent sciatica presented with an expanding lytic lesion of the right side of the first sacral vertebra. Histological examination proved the lesion to be a DF. DF is usually diagnosed in young patients and is very rarely located in the spine and sacrum. The growth rate of the tumor was presumed to be very low because it had not progressed from radiological studies performed 9 years previously. The clinical, radiological, and pathological features of DF are described, and the differential diagnoses are discussed. The most difficult problem is to distinguish DF from a low-grade fibrosarcoma or chordoma.
