Multinucleate cells in thymus of C3H mice.

Multinucleate epithelial cells occur in the thymus of C3H mice. They are poorly differentiated and scarce, but are more numerous in the medulla than in the cortex. Their increase in number with age is particularly significant between the first and the third months especially for cells with a large number of nuclei, and may be related to thymic involution. Viral particles of type C, similar to those described in murine leukemias, are found in mono- and multinucleate medullary epithelial cells.

Chronic lymphocytic leukemia: changes in bone marrow composition and distribution assessed with quantitative MRI.

The purposes of this study were (a) to determine the prevalence of bone marrow abnormalities in patients with chronic lymphocytic leukemia (CLL) using quantitative MR assessment of axial marrow composition and peripheral marrow distribution; (b) to assess the agreement between both quantitative MR methods and compare their sensitivities to detect marrow alterations; and (c) to correlate MR findings with clinical and laboratory parameters. Twenty-nine consecutive patients with biopsy-proven CLL were investigated on a .5-T MR imager to determine bulk T1 relaxation times of the vertebral bone marrow and proportion of proximal femur surface area occupied by nonfatty marrow on coronal T1-weighted MR images of one hip. Of the 29 patients, 12 (41%) had abnormal increase in lumbar marrow T1 values (>600 msec) and 16 (55%) had increased proportion of surface area occupied by nonfatty marrow in the proximal femur (>+1 SD compared to normal values determined in sex- and age-matched healthy subjects). The results of both quantitative MR methods were normal in 12 patients and abnormal in 11 patients (agreement, 79%). Patients with alterations in peripheral marrow distribution had significantly higher T1 relaxation times (P = .001) than those with normal peripheral marrow. Patients with abnormal marrow composition or distribution at MRI had significantly higher blood and marrow lymphocytosis than patients without these features. In conclusion, the agreement between both quantitative MR methods suggests a parallelism between changes in axial marrow composition and in peripheral marrow distribution in patients with CLL. The limits of quantitative MRI in CLL must be kept in mind, because quantitative MR methods failed to detect leukemic marrow infiltration in 41% of patients.

[Followup of pregnant women in the context of malaria: two case reports].

Whether or not a pregnant women should travel to regions where malaria is highly endemic will always be open to question as no prophlaxis can guarantee complete protection in every case. No chemoprophylaxis is 100% effective or entirely without side-effects, particularly for pregnant women whose immune status provides a favourable environment for the emergence of this parasitosis. The advice given will depend on the country and the region, a possible resistance of the parasite to treatment, the season, the length and circumstances of the stay and individual factors. The choice of treatment is very limited as many therapies are contra-indicated in pregnancy; in addition it is extremely important to give a sufficiently clear explanation concerning precautions and prophylaxis as well as of how a possible bout of malaria should be treated, particularly in case of extended stay.

Grey platelet syndrome: evidence for alpha-granule localization of the platelet plasminogen activator inhibitor-1 pool.

The case of an 11-year-old boy with grey platelet syndrome is described. Platelets had the typical grey and ghostly appearance on May-Grünwald/Giemsa staining, caused by the absence of alpha granules confirmed by electron microscopy. Alpha granule protein content, i.e., beta-thromboglobulin and platelet factor 4, was less than 3% of normal and alpha granule secretion in response to thrombin was not detectable photometrically. The plasminogen activator inhibitor-1 pool in the patient's platelets was 5% of normal, confirming previous indirect evidence for the storage of this protein within the alpha-granule. Dense body secretion of adenosine triphosphate and 5-hydroxytryptamine was normal. Aggregation occurred normally in response to adenosine diphosphate and there was a slight delay in response to collagen.

Leiomyoma of the suprarenal gland in a child with ataxia-telangiectasia.

We report the occurrence of a leiomyoma of the suprarenal gland in a 10-year-old girl with ataxia-telangiectasia (A-T). Muscle cell tumors are very uncommon in this gland as they are in A-T. Possible reasons for developing nonhematologic tumors in this syndrome are reviewed. A defect in DNA repair mechanisms probably favors, in young children, the expression of tumors normally expected in the aged.

Thymus histogenesis in C3H mice.

The histogenesis of the mouse thymus was studied by means of light and electron microscopy in an attempt to clarify the simultaneous development of stromal and lymphoid cell populations. On the twelfth day of embryonic life, the thymus primordium was composed principally of undifferentiated epithelial cells and some lymphoblasts. In the developing cortical regions, lymphoblasts accumulated rapidly, stretching the epithelial cells which became stellate in shape. The latter contained multivesicular bodies and, from the sixteenth day on, also typical clear vacuoles. Medullary regions were prefigurated as soon as day 13 by several areas wherein lymphoblasts were sparse and epithelial cells were closely associated, with numerous desmosomes and abundant tonofilaments. On the sixteenth day, some epithelial cells in these regions were differentiated into globular cells, or formed Hassall's corpuscles and intra- or extracellular cysts. On the seventeenth day, the presence of interdigitating cells in the medullary areas completed cortico-medullary differentiation. On the eighteenth day, small cortical thymocytes differentiated and the thymus possessed all characteristics of an adult thymus. Thus, at birth, the histogenesis of the mouse thymus was achieved and the only further modification consisted in a gain of weight.
