12-year followup study of epidemic Spanish toxic oil syndrome.

OBJECTIVE: To determine the longterm clinical and functional outcome among a large group of patients with toxic oil syndrome (TOS). METHODS: One hundred individuals with onset in 1981 were randomly selected for followup in 1993 from a national TOS database. Clinical and laboratory data for 1981 were collected by retrospective chart review. Ninety-one survivors were reevaluated in 1993 by direct interview, examination, the Health Assessment Questionnaire (HAQ), and the visual analog scale (VAS) for pain. A semiquantitative Total Clinical Score (TCS) was created to assess relative global outcome in 1993, for comparison with the HAQ, and for developing a predictive model based on disease manifestations at onset. RESULTS: Fifty-eight percent continue to have symptoms consisting predominantly of muscle cramping (60%), fatigue (55%), arthralgias (43%), subjective cognitive impairment (44%), psychiatric disease (27%), and soft tissue tenderness (22.5%). Severe neuromuscular sequelae, sclerodermatous skin disease, or pulmonary hypertension were not detected. The most notable laboratory findings at followup were hypercholesterolemia (55%) and hyperglycemia (14.5%). A good correlation was demonstrated between both the HAQ and the VAS for pain with the TCS. A statistical model indicated that alopecia, Raynaud's phenomenon, and sensory neuropathy were predictive of outcome. CONCLUSION: TOS is commonly associated with longterm neuromuscular and articular disease. Multiple factors implicated in the adaptation to chronic disease may contribute to this morbidity.

Pharmacokinetics of oxytetracycline in goats: modifications induced by a long-acting formulation.

The pharmacokinetics of oxytetracycline were studied in goats, after the intravenous and intramuscular injection of a conventional and long-acting formulation. The antibiotic was distributed according to an open two-compartment model. The apparent volume of distribution (Vz) and the central compartment volume (Vc) were 1.443 litres/kg and 0.453 litre/kg, respectively, and the total body clearance was 0.156 litre/kg/hour. The mean half-lives (T1/2 lambda z) of the conventional formulation after intravenous and intramuscular administration were six hours 28 minutes and 10 hours 38 minutes, respectively, whereas the long-acting formulation had half-lives of six hours 36 seconds and 29 hours, respectively, after intravenous and intramuscular injection. From the results of these single administrations two intramuscular dosage regimens can be proposed that achieve minimum concentrations of over 0.5 mg/litre (the minimum inhibitory concentration for most susceptible pathogens): with the conventional formulation by administering an initial dose of 10 mg/kg and a maintenance dose of 8.5 mg/kg every 24 hours, and with the long-acting formulation by administering an initial dose of 20 mg/kg and a maintenance dose of 14 mg/kg every 48 hours.

Ischemic optic neuropathy after intraocular lens implantation to correct high myopia in a phakic patient.

A 33-year-old patient had uncomplicated anterior chamber intraocular lens implantation (Worst-Fechner biconcave myopia lens) to correct high myopia. Immediately after surgery, she developed anterior ischemic optic neuropathy likely associated with increased intraocular pressure and systemic hypotension. To our knowledge, this is the first documented case of anterior ischemic optic neuropathy after anterior chamber intraocular lens implantation to correct high myopia in a phakic eye.

Visual discrimination and recognition of facial expressions of anger, fear, and surprise in 4- to 6-month-old infants.

On the assumption that the ability to discriminate facial expressions has adaptive value to infants during early social exchanges, ethologically based theorists have argued that this ability is innate. Guided by this perspective, we investigated the ability of infants, 4-6 months old to recognize and discriminate facial expressions of anger, fear, and surprise. Results obtained with an infant-controlled habituation-recovery procedure showed that infants both discriminated and recognized these expressions when portrayed by several adult female models. In addition, infants spent more time looking at expressions of anger and surprise than at fear expressions. These results suggest that infants can abstract configurations of features that give affective meaning to facial expressions. It is suggested that the differences in habituation to each expression might be the result of their distinct functional signification for the infant.

The genetic properties of homosexual copulation behavior in Tribolium castaneum: diallel analysis.

The rate of homosexual copulation has been defined as the ratio between the number of homosexual mountings and the total number of mountings (homo and heterosexual) performed by a Tribolium castaneum male during a period of 30 min. In a laboratory population, the average rate when a number of males (m) and females (k x m) are tested together has been estimated in each of the six situations defined by m = 2 and 10 and k = 0.5, 1, and 2, k being the sex ratio among scored individuals. Good agreement was found between the observed rates of homosexual copulation and those expected assuming random contacts between pairs of individuals totally indiscriminate with respect to sex. The genetic properties of the trait have been investigated by means of a diallel analysis of six highly inbred lines derived from the same population and their F1 crosses. Significant general and specific combining ability effects were detected. When noninbred females were used for testing, the rate of homosexual copulation is expected to be higher for inbred than for noninbred males. This prediction, implying the existence of inbreeding depression for the trait, also was confirmed by the data.

Macular edema and vitreous detachment.

To determine if vitreous changes play a role in macular edema, 197 eyes were selected, 113 with macular edema (68 diffuse and 45 cystoid) associated with different pathologies. We studied the vitreoretinal relationship in all cases. Photocoagulation was done in 21 eyes and vitreous examination before and after treatment. The difference between complete and incomplete posterior vitreous detachment (PVD) between the control and pathologic groups was statistically significant (P less than .0005). This shows that incomplete PVD is related to macular edema. The difference between complete and incomplete PVD and diffuse or cystoid macular edema was not statistically significant. It was significant between incomplete PVD without collapse and with contraction and diffuse and cystoid macular edema (P less than .0005). That is cystoid macular edema is associated with incomplete PVD with vitreous contraction. The vitreous usually collapses with laser treatment. Complete PVD before treatment seems to be associated with a good prognosis after photocoagulation. Cases with incomplete PVD and cystoid macular edema show worsening of the visual field after laser treatment.

Pharmacokinetics of oxytetracycline after intramuscular administration with lidocaine in sheep, comparison with a conventional formulation.

The pharmacokinetic behaviour of oxytetracycline (OTC) was studied in 11 sheep after intravenous and intramuscular administration at a single dosage of 20 mg kg(-1) bodyweight. A conventional formulation was injected by the intravenous route and two different preparations were administered by the intramuscular route: a conventional formulation (T-100) and an aqueous solution of OTC with lidocaine (1 per cent) (OTC-L). The objective was to determine whether there are differences between both formulations in the disposition kinetics of OTC after intramuscular administration to sheep. After intravenous administration of the conventional formulation, plasma oxytetracycline concentrations were best fitted to an open two-compartment model. Mean apparent volume of distribution was 0.77+/-0.02 litre kg(-1) and the harmonic mean half-life was three hours. The OTC transfer process between central and peripheral compartments was fast and that did not influence the elimination process. After intramuscular administrations of both formulations, half-lives were longer than after intravenous administration (mean values of 14.1 and 58.2 hours for T-100 and OTC-L respectively). In both cases, a biphasic absorption, a 'flip-flop' model and a complete bioavailability were found. OTC-L provided therapeutic plasma concentrations over 0.5 microg ml(-1) (the minimum inhibitory concentration for most susceptible pathogens) for a longer period of time than T-100 (72 hours compared with 36 or 48 hours).

[A study of the motor and sensory cortex using functional magnetic resonance: tasks of active and passive movement].

INTRODUCTION AND OBJECTIVE: The objective of this study was to locate the rolandic area (pre- and post-central) by means of functional magnetic resonance imaging (FMRI) and define its correspondence on a Talairach map, whilst active and passive movements of the dominant hand were performed. MATERIAL AND METHODS: Ten healthy volunteers were found, 6 men and 4 women, of an average age of 26 years (range 22-33). Two appropriate tasks were designed: one involving active and one passive movement. The examination was carried out using a 1.5 Tesla (General Electric) MRI apparatus. An echo-sequence of planar echo-gradient (BOLD technique) was used, making sagittal and axial planes, parallel to the AC-PC line (anterior commissure-posterior commissure). Subsequently an anatomofunctional Talairach map was drawn for each subject, to include the information obtained on FMRI. RESULTS: In all subjects central activity was detected in the rolandic area during the tasks involving selected active and passive movements. Overlap was seen between the pre- and post-rolandic areas with both types of tasks. CONCLUSION: There is good correlation between the image obtained of motor-sensory activity in the rolandic zone and the Talairach anatomofunctional map.

[Hemispheric dominance for language and functional magnetic resonance: a comparison of three tasks].

OBJECTIVE: The object of this study to analyse and compare the efficacy of the classic language tasks (repetition, phonological fluency and lexical decision), for the purpose of identifying the dominant hemisphere for language by means of functional magnetic resonance. SUBJECTS AND METHODS: The three tasks were performance with a groups of 10 right handed subjects with ages of 22 to 40 years. The examination was carried out using a 1,5 T MRI apparatus. An eight sequence of planar echo gradient (BOLD technique) was used, making the oblique axial plane coincide with the line between the anterior and posterior commissures (CA CP line) and covering the entire brain. RESULTS: Dominance was calculated by means of the lateralization index, comparing the activation in each hemisphere in two locations: first, taking account the temporal lobe together with the right and left frontal lobes, and second, taking into account only the right and left frontal lobes. CONCLUSION: The result indicate that the most effective test for obtaining the dominant hemisphere was the one for phonological fluency, this task activated the frontal areas, which showed greater participation in the left hemisphere.

[Triple ureter with inverted ureteral branch in "y" with ectopic ending and calculi inside].

OBJECTIVE: To report an uncommon case of ureteral duplication with a single intramural trajectory and a third ureter arising from the medial ureter, corresponding to the inferior pyelon, opening at the level of the bladder neck and containing a calculus measuring 10 mm along its longest axis, lodged in a saccular dilatation. The etiopathogenesis of this rare anomaly is briefly reviewed and discussed. METHODS/RESULTS: Diagnosis was established endoscopically and pyelographically. Endoscopic resolution was not possible, but stone removal was successfully achieved by conventional surgery using the least invasive approach possible. The patient is asymptomatic 24 months postoperatively. CONCLUSION: Although infrequent, this condition should be suspected in those "unclear' cases seen in day-to-day urological practice. Symptoms of the associated pathology are more common than those arising from the anomaly. Treatment should be specific to each case, as least invasive as possible and should aim at symptomatic resolution.
