[In vitro resistance to fluconazole and itraconazole in clinical isolates of Candida spp and Cryptococcus neoformans].

An in vitro susceptibility testing of 181 strains of six species of Candida and 21 strains of Cryptococcus neoformans was carried out in order to investigate the resistance to new antifungal drugs. We have studied clinical isolates from 200 different patients of Hospital del Mar (Barcelona) and Hospital La Inmaculada (Almería). An agar diffusion method (NeoSensitabs, Rosco, Taastrup, Denmark), was employed with fluconazole, itraconazole, and reference drugs amphotericin B, flucytosine, tioconazole and ketoconazole. A high level of susceptibility was found for amphotericin B in C. neoformans strains while 19% of them were resistant to flucytosine. All the strains of C. neoformans and Candida guilliermondii were susceptible to the new azoles derivatives and also Candida parapsilosis and Candida albicans had a great susceptibility to this antifungals. A greater level of resistance was found for Candida krusei, Candida tropicalis and Candida glabrata to fluconazole, itraconazole and ketoconazole, but resistance to fluconazole and itraconazole is not always linked because ten resistant strains for fluconazole were susceptible to itraconazole, and two other resistant to itraconazole were susceptible to fluconazole.

[Isolation of Cryptococcus neoformans from environmental samples in Alicante].

A study of the incidence of Cryptococcus neoformans in Alicante was carried out in environmental samples from the cities of Alicante and Santa Pola. The samples were pigeons faeces and Eucalyptus camaldulensis tissues. This study shows that the prevalence of the yeast in faeces from captive pigeons is higher (81.5%) than in the same samples from urban pigeons (16.3%). Regarding the biotype and varieties, a 79.3% of the isolates belonged to C. neoformans, and all of them were C. neoformans var. neoformans. None of the E. camaldulensis samples showed C. neoformans growth, although some Cryptococcus laurentii strains were isolated from flowers.

[Candidiasis in an intensive care unit].

The risk of being infected by candidiasis in an Intensive Care Unit (ICU) is evaluated, using an algorithm which allows the establishment of an early fungicidal treatment. This is a study which includes 34 patients with a mortality of 35%. Yeasts are detected from the second week at ICU, related with the long stay of patients at ICU and the relationship between mortality and age. The first positive sample usually does not indicate disseminate candidiasis (9%) or a positive blood culture (6%). Population defined as high risk for disseminated candidiasis (HRDC) with a negative blood culture has the worst prognosis (18 cases, 50% mortality rate). The six cases with HRDC with positive blood culture showed a mortality of 17%. In seven cases there were no HRDC criteria (mortality of 14%). With said study the existence or not of a HRDC could be determined, establishing the adequate antifungal treatment.

Assessment of a new hub design and the semiquantitative catheter culture method using an in vivo experimental model of catheter sepsis.

An in vivo model of hub-related catheter sepsis in rabbits is reported. The model was used to investigate the protection offered by a new hub design against external contamination by Pseudomonas aeruginosa or Staphylococcus epidermidis and to reassess the diagnostic value of the semiquantitative culture method in bacteremia of endoluminal origin. Contamination of conventional Luer-Lock connectors was followed by clinical sepsis, positive catheter segment cultures, or both, whereas contamination of the new hub was followed by complete protection. Clinical and bacteriological discrepancies observed between contamination with P. aeruginosa and S. epidermidis suggest that the virulence of microorganisms may account for differences in the natural history of hub-originated catheter sepsis. The semiquantitative extraluminal method for catheter culture yielded less than 15 CFU in three animals with proven bacteremia and should not be used as the "gold standard" to define catheter-related bacteremia.

[Correlation between the prick test and specific IgE against allergens of house dust and Dermatophagoides sp].

Of the techniques which are usually used for the in vitro etiologic diagnosis of determined allergic processes, it is the RAST technique which is most widely used for determining specific antibodies of the IgE class. On the other hand, also of interest is the quantification of total IgE in these patients. We have studied 80 allergic and non allergic patients in order to evaluate the possible correlation between the Prick and the RAST tests applied to different allergens such house dust. Dermatophagoides pteronyssinus and Dermatophagoides farinae. The patients were distributed in four groups in relation to the clinical history and serum levels of total IgE, as follows: Group I: Patients with positive clinical history and increases levels of total IgE. Group II: patients with positive history and normal values of total IgE. Group III: patients with negative history and increased levels of serum IgE, and Group IV with non atopic subjects with normal values of IgE. The results show a high correlation between both tests for house dust mite allergens in the group I. The relation to inferior in the group II. As far as house dust is concerned, there is no correlation between the cutaneous test in the RAST test even in the group with positive clinical history. The data observed with house dust mites suggest a relation between the clinical history and the cutaneous test and also between the RAST and the levels of total serum IgE. These relations are not fund with house dust. It is discussed the validity and applications of both tests to the diagnosis of immediate hypersensitivity reactions to house dust mites and house dust.
