Post-necrotising enterocolitis pseudo-obstruction treated with Cisapride.

We report on one patient in whom segmental colic intestinal pseudo-obstruction (IPO) following the surgical treatment of a grade III necrotising enterocolitis (NEC) was responsible for a severe failure to thrive. Further intestinal resection in an already short gut was avoided by using Cisapride, a new intestinal prokinetic agent (1 mg/kg/d in 4 doses, orally), which dramatically improved the symptoms and allowed weight gain and intestinal adaptation. After 6 months, Cisapride was withdrawn. IPO did not recur after 2 years of follow-up, although proximal distention persisted.

The persistent Müllerian duct syndrome: a molecular approach.

A rare form of male pseudohermaphroditism is characterized by the persistence of Müllerian derivatives in phenotypic males. To determine the etiology of this syndrome, we studied the expression of anti-Müllerian hormone (AMH) in six boys, including three brothers, with the persistent Müllerian duct syndrome. All except one presented with an inguinal hernia containing the Müllerian derivatives, and in two boys the hernial sac contained the contralateral testis. AMH was normally expressed in the testicular tissue of two patients, as shown by bioassay of anti-Müllerian activity and immunocytochemistry. The testicular tissue of the other patients had no detectable bioactive or immunoreactive AMH, yet they expressed AMH mRNA with a normal transcription initiation site and in the amount expected for their age. These results prove the heterogeneity of the persistent Müllerian duct syndrome and suggest that it may sometimes involve peripheral insensitivity to AMH.

[Ureteral stenosis of appendicular origin, observations apropos of the value of imaging].

A 73-old man undergoes exploration for right nephrotic colic. IVP demonstrates a dilatation of the upper tract up to the iliac vessels. Extensive exploration is not able to precise the cause of the extrinsic stenosis. At the operation, a subacute appendicitis perforated in the retroperitoneum is assessed. The contribution of new imaging techniques to the aetiologic diagnosis of ureteric stenoses is discussed.

Role of nitric oxide in gastrointestinal function and disease.

In the digestive tract, nitric oxide (NO) is extensively distributed, from the mouth to the anus, not only in mammals but also in lower vertebrates and even in invertebrates. It is involved in splanchnic and systemic hemodynamics, in mucosal protection, in immune mechanisms, in hepatic function, in endocrine secretion and in several other roles. It is also involved in the organization of the peristalsis as an inhibitory non-adrenergic non-cholinergic neurotransmitter and relaxant of the smooth musculature. The abundant literature dealing with NO in hepato-gastro-enterology is reviewed and discussed in a pragmatic approach for the gastroenterologist, with emphasis on the involvement of NO in pathophysiology and in the perspective of future therapeutics.
