[Treatment of obstructive ileus useful in patients with a malignity in their medical history].

OBJECTIVE: To determine whether treatment of patients with obstructive ileus and a malignancy in the anamnesis is still useful or leads to increased morbidity and reduced quality of the remaining life. DESIGN: Retrospective. SETTING: University Hospital Utrecht. METHOD: Data were collected concerning 89 patients who in the period August 1987 to July 1992 were admitted with the clinical picture of obstructive ileus and who all had a history of a malignancy treated longer than 30 days previously. RESULTS: The patients were 53 females and 36 males. Over one-half of the tumours were of colorectal or gynaecological origin. In all, 56 patients were operated (17 without delay, 39 after failure of conservative treatment), the others were given only conservative treatment. The cause of the obstructive ileus in the operated patients was malignant in 38, benign in 17 and unknown in one case. Of the patients, 22 died in hospital, mostly as the consequence of progression of the original malignancy, and 2 at their own request at home. Major complications occurred in 21 of the 56 operated patients. Nevertheless 65 of the 89 patients could be discharged free of ileus. CONCLUSIONS: Treatment of obstructive ileus in patients with a malignancy in the anamnesis is useful. In most cases, operation is indicated, but initial conservative treatment is justified.

Post-appendectomy wound infection, a solved problem?

Over a 3.5 year period, 550 patients underwent an emergency operation under the diagnosis of acute appendicitis. The appendix proved to be normal in 82 patients, the other 468 patients had signs of acute inflammation with perforation in 142 cases. Management followed a strict protocol with single dose metronidazole as preoperative antibiotic prophylaxis and liberal open wound treatment in case of gross contamination. Consequently, the subcutaneous tissues and the skin were left open in 218 patients. The overall wound infection rate (1.3%) and the intra-abdominal abscess rate (1.1%) were extremely low. Open wound treatment was well accepted by the patients, did not prolong the hospital stay and gave good cosmetic results.

Tube-inlay graft for abdominal aortic aneurysms. Technique and results.

The usefulness of the tube-inlay graft technique for abdominal aortic aneurysms has been tested in 74 consecutive patients. The operative technique is described. In nearly half of the patients this technique could be used. The other patients received a conventional bifurcation prosthesis. Especially in the patient with a ruptured abdominal aortic aneurysm the tube-inlay-graft technique, because of its simplicity, shorter operating time and less blood loss is to be preferred. Since introduction of the method, the mortality in ruptured cases decreased from 46 to 24 percent.

A critical review of the literature on long-term treatment of peripheral arteriosclerosis with oral anticoagulants.

Protracted administration of oral anticoagulant agents to patients presenting symptoms of peripheral arteriosclerosis is a treatment in widespread use in the Netherlands a Germany. However, a critical perusal of the available literature reveals a scarcity of objective data which might confirm the value of this treatment. It may be concluded from the data in the literature that protracted administration of anticoagulants may be useful only in case of stenosis of the superficial femoral artery. It is pointed out that a statistically correct study of this subject, with use of objective parameters, is urgently necessary.
