Method for determining thiocyanate in serum and urine.

We describe a method for rapid and specific measurement of thiocyanate in serum or urine. We separate thiocyanate from interfering compounds by adsorbing it on an anion-exchange resin that has special affinity for thiocyanate, then eluting with sodium perchlorate. The eluted thiocyanate is quantified by a modified König reaction, sodium hypochlorite being used as the chlorinating reagent. Analytical recovery of thiocyanate added to serum and urine was quantitative; the coefficient of variation was 2.3% for both within-day and between-day precision. Cyanide and certain antibiotics interfere, but may be eliminated by including additional washing steps in the usual procedure. The proposed procedure was compared with another method, based on the oxidation of thiocyanate to cyanide. Agreement was satisfactory, both for serum and urine.

Human beta-mercaptopyruvate sulfurtransferase: distribution in cellular compartments of blood and activity in erythrocytes from patients with hematological disorders.

The distribution of mercaptopyruvate sulfurtransferase (EC 2.8.1.2) in human blood cell compartments has been studied. Almost all of the enzyme activity in whole blood was confined to erythrocytes, but the activity per litre cell volume was higher in platelets than in erythrocytes. Leukocytes contained very low concentrations of the sulfurtransferase and plasma was devoid of activity. The enzyme activity of blood hemolysates from normal subjects and patients with various erythrocyte disorders was also determined. Significantly higher sulfur transferase activity was found in male subjects in comparison with females. The enzyme activity per litre erythrocytes was increased in blood from patients with iron deficiency anemia, whereas less clear-cut differences with respect to normals were found in other diseases studied.

Decisive situations influencing spouses' support of patients with heart failure: a critical incident technique analysis.

OBJECTIVE: The aim of this study was to describe decisive situations experienced by spouses of patients with heart failure that could potentially affect their ability to provide social support to the patient. METHODS: A qualitative descriptive design with a critical incident technique was used. Twenty-three informants, 15 women and 8 men, who were spouses of patients with severe heart failure were strategically chosen to ensure maximal variation in sociodemographic data and experiences as a spouse. RESULTS: Decisive situations influenced the experience of spouses of patients with heart failure in a manner that was either positive (involvement with others) or negative (feeling like an outsider). When spouses were given attention and treated like persons of value, they experienced involvement with others. In these cases, spouses had someone to turn to and were included in the physical care. In contrast, when spouses were kept at a distance by the patient, were socially isolated, and received insufficient support from children, friends, and health care professionals, they experienced feeling like an outsider. CONCLUSIONS: By identifying spouses' experiences, health care professionals can assess which kind of specific interventions should be used to improve the life situation of the patient with heart failure and his or her spouse.

Acute dapsone intoxication: a case treated with continuous infusion of methylene blue, forced diuresis and plasma exchange.

A case of massive dapsone poisoning (15 g) in a 26 year old man is reported. The patient exhibited high plasma dapsone concentration, marked methemoglobinemia, and signs of hemolysis. He recovered completely after intensive treatment with methylene blue, activated charcoal, forced diuresis, and plasma exchange. In order to avoid overdosage of methylene blue it is concluded that this substance should be given by continuous intravenous infusion in cases with severe methemoglobinemia. This way of administration caused a steady decrease in the methemoglobin concentration compared to intermittent administration. Plasma exchange was of minor benefit, probably due to the large distribution volume of dapsone.
