Endoscopy: a minimally invasive procedure for diagnosis and treatment of diseases of the salivary glands. Six years of practical experience.

During a 6-year period we did a total of 72 videoendoscopies of the salivary glands and their associated ductal systems. This minimally invasive procedure is associated with little morbidity and discomfort. The main indication was sialolithiasis of the submandibular and parotid glands. Sialoendoscopy was used not only for diagnosis of radiolucent calculi but also for simultaneous removal of calculi. Sialoendoscopy was also of benefit in the diagnosis and treatment of other diseases of the salivary glands. Even patients with chronic sialadenitis could be helped with endoscopic dilatation of the causative sialostenosis, thereby enabling us to conserve the gland.

[Surgery of craniofacial deformities].

Most craniofacial abnormalities are craniosynostoses due to premature fusion of one or more craniofacial sutures. The typical cranial deformities result from growth inhibition perpendicular to the affected sutures. Functional impairment is caused either directly by the pathological growth pattern or indirectly by the increased intracranial pressure. Craniosynostosis is frequently accompanied by dysmorphia or dystopia of the orbits, resulting in possible functional problems. The indications for operation are to increase the intracranial volume and to correct aesthetic impairments. The planning of the surgical procedures has to consider the orbital problems. This review describes the different disease patterns of premature craniosynostosis and specific surgical approaches for their correction.

High-resolution laser surface scanning for patient registration in cranial computer-assisted surgery.

OBJECTIVE: Markerless patient registration is a new procedure that may reduce logistical efforts and possibly also the radiation load on the patients prior to a computer-assisted intervention. Congruent surfaces, such as bone surfaces or skin surfaces, represented in a data set and in the surgical site, can be overlapped using surface-matching. Previous studies describing this kind of markerless registration, however, show inaccuracies of up to 10 mm during computer-assisted navigation. Furthermore, these systems use less than 1000 surface points of the soft tissue surface in order to establish a correlation between the patient and the data set. Previous papers did not answer the question whether it is this scanning resolution that induces these inaccuracies in registration or rather intraoperative skin deformations. STUDY DESIGN: In the present study therefore a new navigation system (SSN++) was used which is able to register up to 180,000 surface points of the surgical site. SSN++ is an infrared navigation system enlarged by a Minolta VI 900 3D volume digitizer. Three different kinds of laser scan-resolution were used for data correlation. An additional congruence analysis was performed in order to assess the geometry of the matched skin surfaces. 22 patients suffering from different cranial diseases (tumors, bony malformations, foreign bodies) were prepared for a computer-assisted intervention. Intraoral titanium-markers, rigidly fixed on the patients by a maxillary splint, were placed as targets while the CT data sets were made. These targets were - after markerless laser scan registration of the patients - supposed to serve for validating the new high-resolution navigation system SSN++. RESULTS: The accuracy of markerless laser scan registration depends on the intraoperative laser scan's resolution. A high accuracy of the data correlation can be achieved if the number of the laser scan cloud points is about the same as the number of voxels of the corresponding surface on the CT data set. A reduction of the laser scan cloud points to less than 10 % compared to the number of voxels of the CT surface, however, leads to a significant loss of accuracy after markerless patient registration. CONCLUSION: The markerless laser scan registration of the surgical site may achieve the same accuracy as a patient registration made by rigidly fixed titanium screws (mean accuracy: 1.2 mm) as long as a high-resolution laser scan is being used.

[New concepts in image assisted surgery: automated patient registration based on the jaw and external ear].

OBJECTIVE: Automatic and marker less patient registration based on natural anatomical interfaces may considerably reduce the radiation load and logistical input prior to computer-assisted surgical interventions, as it is not necessary to place and measure reference markers. The present study was to find out if, apart from the facial skin, also auricles as well as the upper and lower jaw can be used as anatomical interfaces for the intraoperative registration of the patient's position. MATERIAL AND METHOD: Prior to surgical intervention the positions of 20 patients were registered by a high-resolution 3D laser scan and correlated with the preoperative CT data set. Tumors, skeletal malformations, and foreign bodies were indications for surgical intervention. Auricles as well as the upper and lower jaw were used to register the patient's positions. The accuracy of this basically marker less method was clinically evaluated through the additionally placed conventional registration markers. RESULTS AND CONCLUSION: The marker less patient registration based on natural anatomical interfaces was successful in the upper jaw (deviation: 0.8+/-0.3 mm). The tongue and mobile floor of the mouth led to geometric incongruence and inadequate laser registration in the lower jaw. As far as the auricles were concerned, high accuracy could only be achieved as long as the auricles had not been deformed during CT imaging (deviation: 1.9+/-0.9 mm). The usual CT acquisition with a conventional head support, however, led to temporary auricular deformations in more than half of the patients, which made an exact laser scan registration impossible.

Different expression of cytokines in survivors and non-survivors from MODS following cardiovascular surgery.

OBJECTIVE: Cardiopulmonary bypass is often associated with pathophysiological changes in form of systemic inflammatory response syndrome (SIRS) or multiple organ dysfunction syndrome (MODS). In the present study, we investigated plasma levels of pro- and anti-inflammatory cytokines in survivors and non-survivors from MODS in the early postoperative course following open heart surgery. DESIGN: Prospective clinical study. SETTING: A University Cardiothoracic Intensive Care Unit. METHODS: Levels of cytokines (IL-6, IL-8, IL-10, IL-18, and TGF- ) and procalcitonin (PCT) were measured at the first four postoperative days in 16 adult male patients with an Apache II-score >24 and two or more organ dysfunctions after myocardial revascularization. MAIN RESULTS: All pro-inflammatory cytokines, except for IL-6, were significantly elevated in non-survivors from MODS, with peak values at the first two postoperative days. The plasma levels of immunoinhibitory cytokines showed no differences between the groups. CONCLUSIONS: The results of our study show a different expression of pro-inflammatory cytokines in survivors and non-survivors from MODS following operations with extracorporeal circulation. In addition to Apache-II score, especially IL-8, IL-18, and PCT may be used as parameters for the prognosis of patients with organ dysfunctions after cardiac surgery.

Treatment of recurrent temporomandibular joint dislocation with intramuscular botulinum toxin injection.

Recurrent dislocation of the mandibular condyle poses a difficult problem for affected patients. In the course of time, dislocations often become more frequent and more difficult to avoid. Even with good patient compliance, conservative treatment is often not sufficient. Operative procedures have also been described for the treatment of temporomandibular joint dislocation. However, these interventions are invasive, involving open arthrotomy with possible complications, and cannot safely guarantee a successful outcome. On the other hand, botulinum toxin injections into the lateral pterygoid muscles offer the option of a predictable and prolonged period without renewed dislocation. We present the results of this treatment carried out in 21 patients with recurrent temporomandibular joint dislocation. Four patients were treated following unsuccessful physical therapy and the use of occlusal splints. The remaining 17 patients were treated for a number of conditions resulting in dislocation, including some with senile dementia and mental impairment in whom compliance with conservative measures was poor or completely absent. Injections were given on a 3-month basis in order to have a sustained effect. Within the study period of 6 months to 3 years, only two of the 21 patients suffered further dislocation. There were no side effects recorded as a result of treatment.

Low plasma glutamine after multiple trauma: relationship with intracellular glutamine in polymorphonuclear neutrophils during prolonged ICU stay.

BACKGROUND: Aim of the study was to evaluate whether low plasma glutamine (GLN) is related to low intracellular GLN in stress-affected cells such as polymorphonuclear neutrophil (PMN). We hypothesized, that because low plasma GLN is assumed to have an impact on clinical outcome, stress-affected cells may also show low GLN contents. METHODS: Thirty-nine consecutive severely injured trauma patients staying at least 10 days at a surgical intensive care unit (ICU) of a university hospital were separated into two groups: group one (n = 16) with low plasma GLN (< 420 micromol/l in average during ICU stay), and group two (n = 23) with normal plasma GLN. Initial blood samples for GLN analyses were collected within 24 h of admission at ICU. Further blood samples were taken on days 5 and 10 at 08:00 hours. RESULTS: Patients in both groups showed no differences regarding demographic data, surgical interventions or infections. Acute physiology and chronic health evaluation (APACHE) II and the sequential organ failure assessment (SOFA) score and mortality rate were also comparable. During the study period, intracellular PMN GLN contents and concentrations did not differ between both groups. On the first day, intracellular PMN GLN content in the low plasma GLN group peaked at 5.01 +/- 3.06 x 10(-16) mol and in normal plasma GLN group at 4.73 +/- 2.57 x 10(-16) mol above the level of healthy individuals. In both groups, content decreased significantly towards the end of the observation period (group one: 2.79 +/- 1.59 x 10(-16) mol and group two: 2.63 +/- 1.71 x 10(-16) mol). A correspondent course could be observed for cell volumes. In contrast, variation of intracellular GLN concentrations remained within the reference range throughout the observation period: group one 836 +/- 510 micromol/l on day 1 and 582 +/- 331 micromol/l on day 10, and group two 788 +/- 428 micromol/l on day 1 and 548 +/- 356 micromol/l on day 10. No correlation between plasma GLN and intracellular GLN was found in either group. CONCLUSION: No association between low plasma GLN and low intracellular GLN in PMN was found in a cohort of severely injured trauma patients with a minimum stay of 10 days at ICU.

[Retrospective analysis of facial fractures related to falls in 260 Japanese patients].

OBJECTIVE: Numerous epidemiological studies concerning fall-related injuries have been performed in the geriatric and orthopedic field to clarify the relation between the type of fall and fracture patterns. Although falls are a common cause of facial fractures, there is no study describing these relations. This study thus investigates the relationship between the type of fall and fracture pattern. MATERIALS AND METHODS: We analyzed 260 patients with fall-related facial fractures treated between 1995 and 2000 at the First Department of Oral and Maxillofacial Surgery, Osaka University Dental Hospital, Japan. These patients were classified into two groups: (1) those who suffered fractures by a fall from a standing position or lower and (2) those who suffered fractures by a fall from a level higher than the standing position. RESULTS: Fractures caused by falling from a standing position or lower was more common in older patients, especially in women beyond 70 years. The localization of mandibular fractures was not significantly different between the two groups. However, a high incidence of midface fractures was observed in the patients who fell from a higher position. These patients tended to suffer from central midface or dislocated fractures. CONCLUSION: The results were consistent with recent orthopedic studies, which suggest a relation between fractures in old patients and osteoporosis. A strong force caused by falls from a high position tends to act on the high portion of the midface. Our classification based on the height of the falls was able to demonstrate clearly the relation between etiological factors and fracture patterns. Therefore, it seemed reasonable to analyze patients treated in the Department of Oral and Maxillofacial Surgery, Heidelberg University, Germany, according to the same classification in order to compare both collectives.

[Digital volume tomography--an extension to the diagnostic procedures available for application before surgical removal of third molars].

BACKGROUND: Trauma to the inferior alveolaris nerve is one of the worst complications of the surgical removal of wisdom teeth. With digital volume tomography (DVT), a new imaging technique has been introduced to dentistry and oral and maxillofacial surgery, offering the same options for reconstruction as computer tomography (CT). Since mid-2000 we have been performing imaging via DVT in the case of deeply dislocated third molars sited very close to the nerve canal. METHODS AND RESULTS: In the present study DVT images were made for a total of 81 lower and 11 upper wisdom teeth; the existing orthopantomogram of these teeth had not been sufficiently clear to allow a diagnosis in these cases. Detailed visualization of the wisdom tooth concerned in all planes was possible in each case, and exact reconstruction of the course of the inferior alveolar nerve was successful in 93%. DISCUSSION: In summary, DVT is an advantageous and practical imaging technique that can be used for to extend the diagnosis in surgery on third molars.

[Opportunities and chances for tissue chip microarrays in head and neck surgery. A novel technique for the rapid evaluation of potentially novel biomarkers].

Oral squamous cell carcinomas (OSCC) are malignant tumors with a poor prognosis and low long-term survival rates, even when using modern adjuvant and neoadjuvant therapy forms in addition to surgery. For the clinical estimation of each tumor, it is necessary to define stage-dependent molecular and/or cellular parameters as it is known that OSCC develop along a multistep pathway including the loss of tumor suppressor genes and the amplification of oncogenes which result in changes in protein expression. In order to establish a reliable pattern of molecular and cellular biomarkers, a large number of tumor specimens from different stages of the disease need to be analysed. In this study, biopsies of a collective of 293 OSCC in different stages were screened with the novel technique of tissue chip microarrays by fluorescence in situ hybridization (FISH) and immunohistochemistry (IHC). FISH-analysis was performed on the oncogene cyclin D1 and IHC-analysis on the proteins cyclin D1, p53, p16, cdk4, bcl2, mdm2 and rb. Tissue chip technology was shown to facilitate rapid screening for molecular and cellular alterations in different stages of OSCC and revealed reliable and reproducible results that may allow the definition of a multistep pathway model for tumor progression in OSCC.

Effects of arginine, L-alanyl-L-glutamine or taurine on neutrophil (PMN) free amino acid profiles and immune functions in vitro.

The objective of this study was to determine the effects of arginine, L-alanyl-L-glutamine (Ala-Gln) or taurine on polymorphonuclear leucocyte (PMN) free amino acid profiles, superoxide anion (O2-) generation, hydrogen peroxide (H2O2) formation and released myeloperoxidase activity (MPO). Arginine led to significant increases in PMN arginine, ornithine, citrulline, aspartate, glutamate and alanine concentrations as well as increased H2O2-generation and MPO activity while O(2-)-formation was decreased. Ala-Gln caused significant increases in PMN free glutamine, alanine, asparagine, aspartate, glutamate, ornithine, arginine, serine and glycine concentrations and increased PMN immune functions. Taurine significantly increased PMN free taurine profiles, reduced PMN neutral amino acid content and decreased H2O2- and O(2-)-formation while MPO was increased. Altogether, the pharmacological regimens which enhance the supply of arginine, Ala-Gln or taurine in whole blood significantly affect PMN "susceptible free amino acid pool". This may be one of the determinants in PMN nutrition considerably influencing PMN immune functions. Effect of dose reduction in digital dental panoramic radiography on image quality.

OBJECTIVES: To investigate the effect of dose reduction in digital panoramic radiography on subjective image quality and diagnostic performance. METHODS: Two digital panoramic radiographs were obtained with the Orthophos DS(Sirona, Bensheim, Germany) of patients (n=100) receiving dental care. The first image was taken at the standard exposure setting. For the second image the tube current was reduced by between 48 and 53%, 63 and 69%, 75 and 80% and 80 and 81% respectively. Ten observers rated both images for 21 specific anatomical details and 30 pathological findings. RESULTS: All radiographs taken at reduced mA levels had a significantly inferior score (P<0.01) for anatomical details. There was no difference in the scores for pathological findings. CONCLUSION: Radiographs obtained at reduced mA had inferior subjective image quality, but there was no difference in diagnostic performance. Thus, a reduction in tube current of approximately 50% is recommended. In certain circumstances such as follow-up radiographic examinations, a reduction of up to 65% should be considered.

[3D-ultrasound: a valuable adjunct for therapy planning and follow-up of head and neck tumours].

AIM: The three-dimensional visualization of orofacial tumours and adjacent organs at risk of infiltration is an important requirement for staging, therapy planning and follow-up. Artifacts from the mandible or metal implants often reduce the diagnostic power of computed tomography (CT). The value of 3D-ultrasound (3D-US) in respect to the standard methods CT and magnetic resonance imaging (MRI) was therefore analyzed. METHOD: Between 9/97 and 10/99 the visualization of orofacial tumours in 243 patients by 3D-US was examined, classified on a five-point scale and compared to the corresponding CT and MRI scans. RESULTS: Complete visualization of the tumours was possible in 85.6 % by 3D-US, whereas the rates of MRI and CT were lower with 77.4 % and 61.3 % respectively, mainly because of artifacts. The best combinations of methods, 3D-US + CT (96.7 %) and 3D-US + MRI (95.1 %) gave almost equivalent results, whereas CT + MRI (83.5 %) was inferior. In 2.5 % of the cases none of the methods produced adequate results. CONCLUSION: By free selection of sectional planes and direct correlation to the clinical findings 3D-ultrasound can improve staging, therapy planning and follow-up of orofacial tumours, especially in the case of small tumour size, dental crowns/inlays, metal implants or contraindications to MRI.

Symmetry considerations for volumetric implant-planning.

Symmetry Considerations can be used not only to plan the desired shape of reconstructured bone structures, but also to generate prototypes for soft tissue implants. The paper describes a system which allows to calculate a symmetry plane in the facial area automatically and computes proposals for implants or transplants. This is done by using a Simulated Annealing algorithm to determine the best reflection and morphologig operators to calculate the implant. The system presented has been used to calculate soft tissue implants in the facial area and a replacement for parts of the lower jaw.

["Computer- and Sensor-Assisted Surgery" Special Research Area 414--goals and concepts].

This paper presents an overview of our research in computer assisted surgery within the "Sonderforschungsbereich 414--Information Technology in Medicine: Computer and Sensor Supported Surgery". The overall goal is to achieve improved operation methods including higher quality, more safety and also more economy due to shorter operation time and less postoperative treatment. The SFB 414 is divided into three different sections focusing on projects concerning the heart, projects supporting cranio-facial interventions and interconnecting projects that serve both categories.
