Absence of testicular steroid sulphatase activity in a boy with recessive X-linked ichthyosis and testicular maldescent.

We present a 14-year-old boy with recessive X-linked ichthyosis in whom only one testis could be found. In this apparently normal testis, a lack of activity of the enzyme steroid sulphatase was demonstrated. Several male patients with recessive X-linked ichthyosis have been reported to have testicular diseases, and it is suggested that this may be related to the absence of testicular steroid sulphatase activity.

Carcinoma in situ of the testis in children with 45,X/46,XY gonadal dysgenesis.

The frequency of gonadal tumors in intersex patients with a karyotype including a Y chromosome is very high. In other at-risk groups, testicular germ cell tumors have been shown to be preceded by carcinoma in situ (CIS) changes. We investigated gonadal tissue from four children, aged 1 month to 18 years, with 45,X/46,XY gonadal dysgenesis, and with male or ambiguous genitalia, for the presence of CIS germ cells. Twelve gonadal biopsies and gonadectomy specimens were analyzed by means of conventional histology and densitometric DNA measurements. CIS changes were detected in specimens from all four patients, and aneuploid DNA distributions of the CIS germ cells confirmed the malignant potential of these cells. In one case, electron microscopic analysis revealed the same ultrastructural features of the CIS germ cells as previously described in seminoma cells. These observations indicate that in all patients with 45,X/46XY gonadal dysgenesis and a male phenotype, gonadal biopsies should be considered as soon as the syndrome is diagnosed. We believe that the finding of CIS warrants gonadectomy.

Initiation of spermatogenesis during chemotherapy for leukemia.

Although the information is sparse, cytotoxic drugs have been suspected to cause serious damage to the immature testis, especially when the treatment is given during puberty. In a prospective, longitudinal study, we have investigated the effect of combination chemotherapy on the maturation of the germinative epithelium in 22 testicular biopsy specimens from 10 pubertal boys with acute lymphoblastic leukemia. Five of the boys developed complete spermatogenesis including spermatozoa during the treatment, the biopsy specimens of two patients showed partial maturation of the seminiferous epithelium, while the germ cells of two boys remained immature. In one patient, who received large doses of cyclophosphamide and cytosine arabinoside, all germ cells disappeared during the therapy. We conclude that a significant proportion of boys with acute lymphoblastic leukemia may develop potential fertility even while on treatment with cytotoxic drugs during puberty.

[Immunohistochemistry: theoretical potentials and practical application].

Immunhistochemical methods are increasingly used and their application in surgical pathology is obvious. Nine different immunohistochemical techniques are compared in this review article. The peroxidase anti-peroxidase method of Sternberger et al. (1970), the avidin biotin complex method of Hsu et al. (1981) and the labeled avidin biotin technique of Guesdon et al. (1979) are to be preferred. Two step methods using labeled antibodies such as alkaline phosphatase or peroxidase-labeled second antibodies are less sensitive. However, two-step methods using alkaline phosphatase-labeled antibodies offer the advantage of double staining procedures since alkaline phosphatase reacts with different azo dyes and results in a wide spectrum of colours in the final reaction product. At present, the major problems of immunohistochemistry in daily work are the selection of the fixative and of appropriate controls. Though both problems remain unresolved, immunohistochemical methods should be used for special problems in surgical pathology.

[Flea detection (Siphonaptera) in dogs at the intake area of the Magdeburg polyclinic for small house and zoo animals].

Two random investigations carried out in 1981/1982 established 203 fleas of 5 species in a sex incidence of 1:2.4 (60 males/143 females) on 48 dogs: Ctenocephalides felis occurred most frequently in an extent of infestation of 13 (1981) and 9 (1982) in 24 dogs with a population density of 1.1 to 1.9 specimens/animals, followed by C. canis with 8 (1981) and 11 (1982) from 24 dogs in a greater density of 2.3 to 5 per host. In this C. felis occurs more frequently in "town dogs": 15 identifications from Magdeburg and 3 from the county towns of Quedlinburg, Schönebeck and Burg as opposed to only 2 from country communities. Pulex irritans in an extent of infestation of 5 out of 24 dogs in 1982 reached a density of 5 per animal. The results confirm the fact that the range of species of fleas on dogs depends or the keeping conditions and the specific breed activity of the hosts.

[Secretin and bicarbonate secretion in probands and ulcer patients following bolus administration of hydrochloric acid into the duodenum].

Various amounts of hydrochloric acid (50, 100 and 200 ml) of two different concentrations (70- and 140 mmol/l) were rapidly instilled into the duodenum of eight healthy subjects and five duodenal ulcer patients. Secretin, gastrin and bicarbonate output were measured. Secretin release and bicarbonate secretion are dependent on acid load and not on acid concentration. There is a linear correlation between the amount of acid instilled into the duodenum and pancreatic bicarbonate output. After duodenal acidification duodenal ulcer patients released equal amounts of secretin and secreted as much bicarbonate as healthy subjects.

Cryptorchidism and testis cancer. Atypical infantile germ cells followed by carcinoma in situ and invasive carcinoma in adulthood.

Atypical germ cells closely resembling carcinoma in situ (CIS) cells of the adult testis were found in a biopsy specimen from an undescended testis of a 10-year old prepubertal boy who was admitted for orchiopexy. The atypical cells were distributed as normal infantile germ cells in the seminiferous tubules, but in successive postpubertal biopsies an adult CIS pattern developed. Microspectrophotometric DNA measurements showed aneuploidy of both the prepubertal and postpubertal atypical germ cells, and the malignant potential of these cells was proven as small foci of invasive germ cell carcinoma in addition to widespread CIS were found when an orchiectomy was performed at the age of 20 years. Thus, premalignant changes in the testis may be found before puberty, and may precede a clinically detectable tumor by more than 10 years.
