Medullary and mixed medullary-papillary carcinoma of the thyroid gland.

Concurrent medullary and papillary carcinomas of the thyroid gland are rare and occurrence of the medullary and medullary-papillary carcinoma, as far as we know, has not been previously described. The medullary carcinoma was found in the upper part of the right lobe and the mixed medullary-papillary carcinoma in the left lobe of thyroid gland. Immunohistochemistry revealed calcitonin positivity in the medullary parts of the carcinoma and thyroglobulin positivity in papillary structures. Both light microscopy and immunohistochemistry observation distinguished these two cell lineages. The patient was treated with radiotherapy but died 18 months after first symptoms. Metastases were identified in the liver, mediastinal lymph nodes and soft tissues and micro-metastases in the lungs. The authors discuss histogenesis of mixed thyroid carcinomas.

[Dedifferentiated liposarcoma of the omentum simulating malignant peritoneal mesothelioma].

Liposarcoma of the omentum is a rare tumour which can simulate mesothelioma by naked eye and in histology as it was in the described case: A diffuse tumorous infiltration of parietal and visceral peritoneum was found in a male autopsy combined with two secondaries in the liver and one renal. The omentum was increased and quite infiltrated by the tumour. Single tumour foci consisted of structures reminding of a malignant neurogenous tumour, malignant fibrous histiocytoma and haemangiopericytoma. Some areas with a rather oval tumour cells with poor cohesivity reminded of a malignant lymphoma but were without any immunohistochemical positivity with markers proper for ML. A series of other tumours were to be eliminated in differential diagnostic consideration, i.e. sarcomatoid carcinoma, GIST, secondaries of a sarcomatoid variant of melanoma, malignant schwannoma, malignant fibrous histiocytoma, peritoneal leiomyomatosis. The prevalence of dedifferentiated structures can mostly veil the diagnosis of liposarcoma and immunohistochemical results are not always constant. A transition between well differentiated liposarcoma and its dedifferentiated structures are to be found in a single section for correct diagnosis. Prognosis of the dedifferentiated liposarcoma is presumed to be better than that of pleiomorphic sarcomas. The described patient was presented in terminal stage of disease and was monitored but for a month.

[Clinical characteristics of mycosis fungoides and Sézary syndrome].

Mycosis fungoides and Sézary syndrome are typical representatives of malignant lymphomas of the T-series affecting the skin. Both affections have a varied clinical picture, a different spectrum of histological and laboratory findings and lack a uniform prognosis. In the submitted review the authors discuss in more detail all mentioned aspects of the disease and draw attention to new therapeutic possibilities.

[Simultaneous occurrence of 2 thyroid gland carcinomas].

The concurrent finding of two thyroid carcinomas is rare. The authors present two patients with the concurrent occurrence of two tumours. In the first case--a 51-year-old man--the authors found a medullary carcinoma in the right lobe and a medullary papillary carcinoma in the left thyroid lobe. The medullary papillary carcinoma metastatized into the mediastinal lymph nodes and into the mediastinal tissues in the area above the tracheal bifurcation. From the morphological and immunohistological aspect in the primary tumour of the left lobe as well as in the mediastinum two different clones of tumour cells were found (the medullary component of the tumour responded positively to antibodies against calcitonin and the papillary component to serum against thyroglobulin. The patient was treated by radiotherapy. The tumour progressed however markedly in its medullary component and the patient died one and a half year after the first symptoms from generalization of the tumour. The second patient, a woman, had an occult papillary carcinoma in the right lobe and in the left lobe a of a medullary papillary carcinoma (follicular variant). This type of tumour also metastatized into a cervical lymph node in the area of the left lobe. The patient was treated by radioiodine. Nine months after bilateral thyroidectomy she has no signs of progression of the tumour.

[New findings on the classification, diagnosis and therapy of primary gastrointestinal lymphomas].

Based on available recent data from the literature the authors review recent findings pertaining to the etiopathogenesis, immunology, clinical manifestations and treatment of primary gastrointestinal lymphomas. From the pathogenetic aspect in particular the close association with infections and immune disorders in various portions of the gastrointestinal tract is important. In their review the authors draw attention to new aspects of the histological and immunological classification of these tumors, in particular as far as definition of lymphomas of the MALT-system is concerned. They evaluate also data from their own group of 83 patients with regard to the incidence and site of the disease. In the conclusion they draw attention to the importance of some new therapeutic approaches, incl. antibacterial treatment.

[Mixed carcinoid-carcinoma of the stomach].

Bioptical observation of a mixed carcinoid-carcinoma in a 75-year-old man's stomach was described. The subcardial tumour was 10-7 cm in diameter with secondaries in perigastric lymph nodes. A well differentiated adenocarcinoma with mucus production formed a minor marginal part of the carcinoid which expressed immunohistochemical markers common in gut cancers: carcinoembryonic antigen, epithelial membrane antigen and some cytokeratins. The well differentiated adenocarcinoma expressed different cytokeratins detected by AE1-AE3 antibody. The carcinoid was positive with Grimelius impregnation and strongly positive with chromogranin A and synaptophysin bound antibodies. Transition of both the tumour types was continuous.

Rectal adenocarcinoma with rhabdoid phenotype.

Tumour of the rectum was found in 50-year old man. Two different components were identified. The rectal wall was infiltrated by a tumour with rhabdoid phenotype which formed majority of tumour mass. This type of tumour grew under differentiated tubular and mucinous adenocarcinoma which was present in small extent. Ultrastructurally rhabdoid cells revealed clusters and whorls of intermediate filaments without myoblastic differentiation. Strong anti-vimentin positivity prevailed over the anti-cytokeratin positivity. The authors suggest that this is a case of an adenocarcinoma transforming into a poorly differentiated form with rhabdoid phenotype.

[Non-Hodgkin's B-cell lymphoma resembling Hodgkin's lymphoma].

Two bioptic cases of dual lymphomas in a single anatomic localization were described. In the first case supraclavicular lymph node and in the second case stomach were infiltrated. In both cases malignant non-Hodgkin's B cell lymphoma predominate. Hodgkin's-like lymphoma involved only part of organs and a few Reed-Sternberg-like cells were found. Immunophenotype of these cells belonged to B-cell line (CD15-, CD30-, CDw75+, CD74+, CD20+). Effect of lymphokins on the fibroproduction in both lymphomas was discussed.

[Löffler's endocarditis (with manifestations of restrictive cardiomyopathy and hypereosinophilia)].

Loeffler's endocarditis in a 47-year-old man was followed 5 years. He died of heart failure. His original diagnosis was eosinophilic leukaemia but cytogenetic investigation failed to show any chromosomal aberration and only mature eosinophilic leukocytes were present in peripheral blood. Relation between heart lesions and eosinophilic leukocyte factors (peroxidase, major basic protein, cationic protein, transforming growth factor beta) were discussed.

Expression of estrogen receptors and estrogen-induced proteins in tumors of "hormone non-dependent tissues".

Estrogen receptors (ER), progesterone receptors (PR) and some other estrogen-induced proteins (EIP) were found in the cells of salivary gland carcinomas, thyroid carcinomas, in gliomas and in various farther neoplasms as well as in some normal tissues. The positivity was demonstrated in both females and males. The results indicate that steroid hormones and their receptors could play, both in normal and tumorous tissues, more important biological role than expected so far.

Mixed medullary-follicular and medullary-papillary carcinoma of the thyroid: one or two entities?

Among 36 cases of medullary carcinoma of the thyroid we found two cases of mixed medullary-follicular carcinoma which in addition to the typical morphology contained foci of clear cells with cytological features of papillary carcinoma of the thyroid. The authors compare the two cases with the recently described entity called medullary-papillary carcinoma of thyroid.

[Evaluation of small-cell and spindle-cell thyroid tumors using immunohistochemical markers (retrospective study)].

Seven small-cell and spindle-cell tumours were found among 126 malignant thyroid tumours during 10 years. HE was irrelevant for their evaluation. Based on immunohistology their final classification was: 4 low grade small-B-lymphocyte malignant lymphomas, 1 medullary carcinoma, 1 follicular cell undifferentiated carcinoma, and 1 spindle cell anaplastic tumour, probably undifferentiated carcinoma.

Diffuse sclerosing variant of papillary thyroid carcinoma.

Five cases of the diffuse sclerosing variant of papillary thyroid carcinoma (DSPC) were presented. The fifth case was discovered in a statistically not defined series of carcinomas of the thyroid gland. DSPC were found in 5.7% of 70 cases of thyroid papillary carcinoma. All the cases described showed a similar histological picture. In the first case a "dominant nodule" was found. Immunohistologically B-cell markers were mostly proved on lymphocytes of the focal lymphocytic infiltration. Accumulation of Langerhans cells was present in the tumour foci. The finding of these cells is regarded as a sign of an enhanced immunological defense mechanism. The clinical follow-up interval of patients was 3 to 9 years. Less favourable prognosis of DSPC as compared with classical papillary carcinomas could not be confirmed in this series.

[Carcinoma of the thyroid with the so-called "tall cells"].

Two tall-cell tumours were found in a group of 70 papillary thyroid carcinomas. The tall cells were all over the first tumour whereas in the second case they represented more than 40% of tumour cells. Both patients were followed 8 and 7 years respectively. Cervical lymph node metastases was mentioned in the clinical history of the first case. Biological behaviour of both tumours was not different at large from classical papillary carcinomas.

[Present possibilities of prenatal diagnosis of Fabry's disease].

Fabry's disease (angiokeratoma corporis diffusum universale) is an inborn error of metabolism, which is based on a shortage in the cumulation of glycosphingolipids in endothelial and epithelial cells of glomeruli, vascular endothelia and in ganglion cells with subsequent severe organ damage. Heredity is X-linked recessive. In the submitted paper the authors present a case-history of a family where three men were affected and carriership was confirmed in five women. During the first pregnancy one of them a successful prenatal diagnosis was made from cultivated amniotic fluid cells. Chromosomal examination revealed female sex and the levels of the alpha-galactosidase A were established in cultivated cells. A healthy girl was delivered.

[Interfollicular Hodgkin's disease].

Two cases of so-called interfollicular Hodgkin's disease, one case lacking of all histological criteria and one case of lymphadenitis mimicking Hodgkin's disease, were identified among 76 patients with Hodgkin's disease. Case 1 and 2 rank among lymphocyte predominance type, case 3 showed mixed cellularity. They shared a progressive transformation of germinal centres which was proved by immunohistology. Leu M 1 antibody-positive cells were found in both cases of the so-called interfollicular Hodgkin's disease and not in lymphadenitis mimicking Hodgkin's disease.

Application of anticytokeratin antibodies to colon carcinomas.

Mouse monoclonal anticytokeratin antibodies (BA 16; BA 17; CO 8.2; C18.2; C 10; LE 41) were used for examination of 5 patients with large bowel carcinoma ("left colon") using the indirect immunoperoxidase technique. For each case cryostat sections were examined from three localizations: primary tumour of large bowel, mucosa in the vicinity of the tumour, and mucosa of distant part from the primary tumour. The antibodies BA 16 and BA 17 produced strongly positive results. Antibody C 18.2 seems to be less suitable for detection of cytokeratin expression in this part of large bowel. The obtained results indicate that no significant differences were found in expression of cytokeratins in the three localizations by histologically well or moderately differentiated adenocarcinoma.
