Isolation of the fifth component of the bovine complement system.

Bovine C5 has been isolated from fresh bovine serum by a five-step procedure: polyethylene glycol precipitation, sequential ion-exchange chromatography on DEAE-Sephacel and CM-Sephadex, hydroxylapatite chromatography, and affinity chromatography. The purified C5 was a protein of apparent molecular weight 202,000 +/- 9,000 composed of two chains: an alpha-chain of molecular weight 127,000 +/- 5,000 and a beta-chain of molecular weight 74,000 +/- 2,000. The alpha-chain was cleaved by Sepharose-CVF.Bb (a cobra venom factor (CVF)-induced C3/C5 alternative pathway convertase) in the absence of any C3 or C3b. The monocarboxylic acid form of K-76, a sesquiterpene compound isolated from the culture filtrates of Stachybotris complementi, inhibited the alternative pathway of bovine serum, and the inhibited hemolytic activity was restored, in a dose dependent manner, by bovine C5. This provided the basis for a C5 functional assay throughout the purification procedure. The purified C5 showed species specificity and was functionally distinct from bovine C3.

Static pressure seal of earmolds.

Four experiments were carried out to investigate the effect on the static pressure seal of earmolds made from currently used impression and earmold materials; the occasional practice of making more than one earmold from an impression; the earmoldmaker buildup of impressions; and the multistage buildup of impressions by the impression taker. Experiment 1 showed that the chance of sealing the ear with earmolds made by the ordinary two-stage technique with only general buildup of the impression was approximately 10 percent and that none of the impression or earmold materials significantly improved the chance of obtaining a seal. Experiment 2 showed that the chance of obtaining a seal with multiple earmolds made from the one impression was zero, or very close to zero. Experiment 3 revealed that the chance of obtaining a seal increased to 55 percent when the earmold was made from an impression which was specially built up by the earmoldmaker. Experiment 4 showed that the chance of sealing the ear with earmolds made from multistage impressions was significantly greater (87.5 percent) than sealing the ear with those made from impressions specially built up by the earmoldmaker.

Updated performance requirements for hearing aids.

This article updates the design specifications for a family of hearing aids required by the National Acoustic Laboratories (NAL) in order to meet the needs of its clients. The required range of coupler gains and frequency responses of behind-the-ear (BTE), in-the-ear (ITE) and body-level (BL) hearing aids has been recalculated on the assumption that the aids will be fitted using the new Byrne-Dillon procedure for selecting gain and frequency response. In the light of recently acquired data, the specification of maximum allowable internal noise has been changed slightly and no relaxation of the equivalent input noise criterion for high gain aids is allowed. Since the Byrne-Dillon procedure demands a somewhat smaller (though still quite substantial) range of hearing aid response shapes, the subdivision of the total response requirements has also been repeated. The number of hearing aid models specified is still 18, but the number of fitter-adjusted controls has been reduced from 6 to 4. The fitter-adjusted controls comprise a maximum power output (MPO) level control, an MPO shape control, a low-tone cut control and a high-tone cut control, with MPO control being achieved by means of low-distortion compression limiting.

Performance requirements for hearing aids.

The requirements for hearing aid frequency responses, SSPL characteristics, compression limiting, directional response, and internal noise are examined. On the basis of the assumptions made, aids would best serve the hearing impaired if they had flat very-low-frequency gains (and various mid- and high-frequency gains), SSPL responses adjustable in shape and level, directional responses, and internal noise levels below a recommended criterion level.

Diabetic nephropathy without hyperglycemia.

Three cases are presented in which biopsy-proven diabetic nephropathy was found in nephrotic patients with no prior history of diabetes. One patient presented with advanced retinopathy and end-stage renal disease (ESRD), another with retinopathy and mild renal insufficiency, and a third with mild renal insufficiency alone. In each case, biopsy showed diffuse and nodular glomerulosclerosis with no evidence of kappa or lambda light chains. The patients had normal fasting serum glucose values, and two had normal oral glucose tolerance tests. Twenty-three similar cases in the literature were reviewed, and in most, some evidence of diabetes or hyperglycemia was found; five cases remained in which there was no evidence of hyperglycemia at any time. The cases reviewed included a disproportionate number of men (65%) and blacks (42%). It is concluded that diabetic nephropathy may exist in the absence of detectable hyperglycemia in a small number of extraordinarily susceptible individuals. Men and blacks appear to be at increased risk.
