A variant of Noonan's syndrome: a case history.

The authors describe a variant of Noonan's syndrome hitherto undescribed. The special findings in this case were anomalous origin of the aortic arch vessels and focal glomerulonephritis.

Cardiovascular, skeletal and ophthalmologic manifestations in patients with Marfan's syndrome.

Cardiovascular, skeletal and ophthalmologic manifestations of Marfan's syndrome were evaluated in 36 patients with Marfan's syndrome, including 22 females and 14 males whose ages ranged from 3 to 70 years. The patients complained mainly of palpitations and chest pain. Mitral clicks and/or systolic murmurs were heard in 31 patients (86%); skeletal findings were found in 35 patients (97%) and ECG T-wave changes in 11 (30%). Chest X-rays showed cardiac enlargement in six patients (17%) and dilatation of the aorta in two (6%). Holter monitoring performed in 21 patients showed only a few premature atrial and ventricular beats in 4, although prolapse of the mitral valve was found in 18 patients. Echocardiography revealed mitral valve prolapse in 32 patients (89%), 21 females (95% of females) and 11 males (79% of males). Aortic root dilatation was found in 19 patients (53%). 11 males (79% of males) and 8 females (36% of females). This study demonstrates the variable clinical expression of Marfan's syndrome and the importance of clinical and noninvasive methods, mainly echocardiography, for its evaluation.

Ribosomal protein phosphorylation in vivo and in vitro by vaccinia virus.

Ribosomal protein phosphorylation was investigated in Ehrlich ascites tumor cells infected with vaccinia virus (Copenhagen strain). After 90 min of simultaneous infection and 32P-labelling, ribosomal proteins Sa, S2 and S13 appear specifically phosphorylated as well as Sb/La, P1 and S6, which are also phosphorylated in control cells. Sa is an acidic protein, whose phosphorylation has not been observed previously. A kinetic study showed that S2 is phosphorylated very rapidly within 10 min after the beginning of infection and it is complete 1 h later. The phosphorylation of S13 begins after a lag time of about 1 h and is completed after about 2.5 h of infection. Moreover only one phosphate is incorporated into S13 on a serine residue while up to four phosphates are incorporated into S2, the first on a serine and the three following on threonine residues. In vivo experiments, carried out in the presence of cycloheximide and cordycepin, suggest a viral origin for the kinase involved in the phosphorylation of S2 and S13. Moreover, in vitro experiments demonstrated that the kinase associated with the viral cores is capable of phosphorylating S2 on a serine residue only. In our cell/virus system, no significant difference in S6 phosphorylation was detected, when compared to uninfected cells. It is concluded that the specific and efficient phosphorylation of three ribosomal proteins from the 40S ribosomal subunit correlate well with possible translational mechanisms ensuring the efficient expression of early and late genes of vaccinia virus. In the light of these and previous results [Person, A. and Beaud, G. (1986) J. Biol. Chem. 261, 8283-8289], a mechanism is proposed for the shut-off of host protein synthesis and the selective translation of mRNAs of viral origin.

[Basophil degranulation test in suspected mould allergy].

Mould allergy is often linked with a pattern of signs and symptoms of the upper airways, especially of the nose. Since exact aetiopathological correlations are hard to determine by anamnesis only, further allergy diagnosis is very important. This often yields marked differences between intracutaneous and nasal provocation tests. The human basophil degranulation test (HBDT) now offers a further method of in-vitro diagnosis. This test can explore the condition of the basophils, which are of great importance for the allergic reaction besides the mast cells, and the ability of these cells to degranulate in the presence of a corresponding allergen. We investigated the correlations between the results of HBDT, intracutaneous skin test and nasal provocation test. 30 patients with perennial rhinitis suspected of being caused by moulds were tested using 4 mould mixtures. We found many more positive results with the nasal test and HBDT than with the skin test. The corrected contingency coefficient after Pearson showed a stronger correlation of HBDT to the incracutaneous skin test and to the nasal provocation test than between the two in-vivo methods. This is thought to be due to a wider sensitivity spectrum of the degranulation test which can possibly also measure other allergy reactions than type I after Commbs and Gell. We consider HBDT to be a valuable additional tool in allergy diagnosis. We should welcome a wider range of different allergenic test slides and a standardisation of allergenic extracts.

Treatment of human ulcers by application of macrophages prepared from a blood unit.

Decubital ulcers contribute to morbidity and mortality in elderly patients. Macrophages play a major role in the process of wound healing. We compared the efficacy of local treatment of decubital ulcers in elderly patients using macrophages prepared from a blood unit, vs. conventional treatments. Patients with decubital ulcers (n = 199) hospitalized during one year in a Geriatric Hospital in Israel, were included in the study. The ulcers of 72 patients (average age 82), who provided informed consent, by themselves or by family, were treated by local injection of macrophages prepared from a blood unit in a closed sterile system. The remaining 127 patients (average age 79) were treated conventionally and served as controls. No exclusion criteria were applied. Only a completely healed ulcer was considered a positive outcome of treatment. In the macrophage-treated group 27% (36 out of 131 ulcers) were healed compared to 6% (15 out of 248) in the control group (p < 0.001). There was also a significantly faster healing in the experimental group (p < 0.02). No side effects were noted. We conclude that Macrophages prepared from a blood unit, in cost-effective, closed, sterile system, are significantly more effective than conventional methods for the treatment of ulcers in elderly patients.
