[Cardiac effects of dermatomyositis. A case report].

The cardiac effects of dermatomyositis and polymyositis are found frequently at autopsy, even though the clinical manifestations are rare. We report the observation of a patient with dermatomyositis, in whom ventricular tachycardia, and dilated hypokinetic cardiomyopathy were in the foreground of the cardiac anomalies. This rhythm disturbance responds to treatment, as opposed to the associated dilated cardiomyopathy. Electrocardiographic anomalies are frequent, notably conduction disturbances. Ventricular rhythm disturbances are of poorly defined frequency. Cardiac insufficiency is often stabilised with treatment. The other effects are much rare.

[Ischemic pseudo-normalization of T waves and ST segment].

The development of acute myocardial ischaemia in a territory with baseline repolarization abnormalities can be reflected by transient pseudo-normalisation of the T wave and/or ST segment. These repolarization abnormalities can occur spontaneously, during a stress test or during of an isoproterenol test. Clinicians should be familiar with these unusual electrocardiographic changes of acute ischaemia, which require appropriate surveillance and treatment.

[Late recurrence of a large post-infarction left ventricular pseudo-aneurysm. A case report].

Left ventricular pseudo-aneurysm is a neo-cavity characterised by the presence of a narrow neck and a wall consisting of fibrous tissue and thrombotic debris without any muscular cells. The diagnosis must be made as soon as possible by non-invasive methods (echocardiography, CT scan, MRI ...) or by angiography before early surgical treatment, in view of the high risk of secondary rupture. The authors report this original case of a post-infarction left ventricular pseudoaneurysm which recurred five years after initial surgical cure.

[False traumatic aneurysm of the internal mammary artery caused by a stab wound. Apropos of a case].

The authors report the case of embolism by a false aneurysm of the internal mammary artery: a 57 years old woman with no previous medical history was admitted to the emergency unit with a kitchen knife stuck in the 3rd left intercostal space. After withdrawing the knife, in the operating theatre, radiological controls revealed a blurred opacity in the left antero-superior mediastinum. The patient signed her own discharge from hospital against medical advice on the 7th day. She was readmitted 10 months later for left retrosternal chest pains. Chest X ray demonstrated a rounded opacity in the antero-superior mediastinum. Computed tomography with injection of contrast confirmed the vascular nature of the opacity. Arteriography in the aortic arch confirmed the diagnosis of false aneurysm of the left internal mammary artery. Selective angiography of the internal mammary artery was undertaken and the aneurysm was embolised with coils. The clinical outcome was immediately improved with relief from the chest pains.

[Compression of the right pulmonary artery by chronic aortic dissection].

The authors report the case of a 56-year old woman who had presented with clinical symptoms resembling those of pulmonary embolism and due to chronic dissection of the first aortic segment compressing the right branch of the pulmonary artery. Eighteen months after the acute episode, the diagnosis was suspected at echocardiography, but the other paraclinical examinations performed (scintigraphy of the lung, angiography, computerized tomography) failed to display the aortic dissection, although they enabled the diagnosis to be approached and the physiopathology to be determined. At surgery, a type II dissection was discovered, the compression was relieved and the pulmonary artery was recanalized. The post-operative period was uneventful, with return to normal of the scintigraphic and CT images. This case is exceptional in that it is extremely rare (this is the 6 th case reported) and the lesion was well tolerated, probably due to the presence of a systemic-pulmonary circulation.

[Silent aortic insufficiency mimicking dilated cardiomyopathy].

The authors report 6 cases of severe and silent aortic insufficiency having simulated in all aspects the picture of a dilated cardiomyopathy at the stage of cardiac insufficiency with primary manifestations. They insist on signs leading to the diagnosis of this clinical entity: past history of rheumatism, signs of electrical left ventricular hypertrophy in the absence of arterial hypertension, aortic calcifications and mostly presence of a discrete mitral diastolic fluttering during echocardiographic examination. Supra-sigmoid aortic angiography confirms the diagnosis of severe aortic regurgitation. In order to explain the non-perception of the murmur, they invoke the alteration of the transmission secondary to an air cushion (3 cases the association of another valvulopathy with murmur (1 case) and mainly the decrease of the leakage by increase of the left ventricular telediastolic pressure and the decrease of the diastolic aortic pressure, with diminution of the turbulences. The advantage of knowing this entity rests on the possibility of valve replacement, even at the stage of myocardial failure, since the satisfactory long-term post-operative evolution in 4 patients stands in contrast with the dangerous nature of a spontaneous evolution.

The ergometrine test: effects on esophageal motility in patients with chest pain and normal coronary arteries.

Ergometrine can evoke coronary spasm in patients with variant angina. The cause of ergometrine-induced chest pain in the absence of coronary spasm is not clear. To determine whether ergometrine produced esophageal dysfunction and chest pain, we evaluated 28 patients by esophageal manometry. Six had chest pain in response to ergometrine during cardiac catheterization (group I) and 22 did not (group II). Results of cardiac catheterization were normal in all patients. Seven volunteers with no history of chest pain formed a control group (group III). Esophageal manometry was performed before and after ergometrine administration (0.4 mg I.V.). Ergometrine provocation during esophageal manometry caused significant deterioration in esophageal motility associated with familiar pain in 5/6 group 1 patients. The motility disorders were characterized by repetitive contracts of high amplitude and long duration in the distal esophagus. No patient from group II or III experienced chest pain after ergometrine and only 2 from group II developed long duration contractions. Thus, we conclude that in patients with normal coronary angiograms, ergometrine-induced chest pain without associated coronary spasm suggests that esophageal motility disorders originate chest pain.

Angina Pectoris↗

[Double superior vena cava with drainage of the right superior vena cava into the left auricle. Presentation as a cerebral abscess in an adult].

A 44 year old man presented with a cerebral abscess, the location of which suggested a septicaemic origin. Although the patient was not cyanosed, a cardiological work-up was requested to exclude a right-to-left shunt. This showed a double abnormality of the systemic venous drainage: presence of an abnormal left superior vena cava draining into the coronary sinus and of a right superior vena cava draining into the left atrium. These two vena cava intercommunicated by anastomoses. Angiography in the right superior vena cava after occlusion by balloon catheter at its junction with the left atrium showed flow from the right to the left superior vena cava and to the azygos system. Simple ligature of the right superior vena cava was therefore performed to prevent recurrence of cerebral abscess. This case is rare and of interest because of the presence of two superior vena cavae, one on the right draining into the left atrium and the other on the left draining into the coronary sinus, with anastomoses between the two superior vena cavae. This double abnormality of systemic venous drainage explains the absence of cyanosis and therefore the relatively late detection of this malformation.

[Comparison of the effects of bepridil and diltiazem in Prinzmetal's angina. Crossed, randomized, double-blind study. Apropos of 14 cases].

The efficacy of bepridil was compared with that of diltiazem in treatment of Prinzmetal's angina, in a crossed, randomized double-blind study of 14 observations using repeated Holter recordings (a total of 9 for each patient). Clinical and electrical monitoring showed that the efficacy of bepridil is comparable to that of diltiazem in treatment of spastic angina.

[Myocardiopathy disclosing a centronuclear myopathy in an adult].

The authors report the case of a primary myocardiopathy of the young adult, related in fact to a rare muscular congenital disease: centro-nuclear myopathy. The case is different from the classic picture in that it is revealed late, by its cardiac manifestations which dominate the clinical picture and lead to an irreducible cardiac insufficiency requiring a heart transplant. Only a muscular biopsy with histological and histo chemical study permits to make the diagnosis, while the lesions of the myocardium are non-specific.

[Value of esophageal manometry with the methylergometrin maleate test in angina with normal coronarography. The concept of esophageal angina].

22 patients presenting an "angina with normal coronarography" and 5 control subjects were subjected to a manometric exploration of the oesophagus in the basal state and under methylergometrine maleate (MEM) treatment. The MEM test is shown to be a good one for the induction of painful oesophageal dyskinesias, particularly in patients who had suffered in an inexplicable manner when that substance was administered for coronarography.

[Congenital agenesis of the inferior vena cava. Apropos of a case presenting as an ilio-femoral phlebitis. Contribution of echography and x-ray computed tomography].

The authors report the case of a young patient, 24 years old, hospitalized for a phlebitis of the right inferior member. The cause determined by venous angiography, echography and computed tomography, was a congenital agenesis of the inferior vena cava. This case allows to remember the embryology of the congenital anomalous inferior vena cava, which are rare and often unknown.

[Evaluation of the severity of coronary disease by thallium 201 myocardial scintigraphy and technetium 99m isotope ventriculography. Comparison of the 2 methods apropos of 30 cases].

The evaluation of the severity of coronary artery disease by non-invasive methods may involve the use of isotope techniques. The authors compared the results of technetium 99m cardiac functional angioscintigraphy and thallium 201 myocardial scintigraphy, performed at rest and on effort, in a group of 30 patients. The 30 patients consisted of 6 normal patients (control group) and 24 patients with coronary artery disease (8 cases of angina; 16 infarcts, including 12 inferior infarcts). Coronary angiography revealed 13 cases of one vessel disease, 8 cases of two vessel disease and 3 cases of three vessel disease. The results of angioscintigraphy revealed that the technique was reliable in terms of the evaluation of the abnormalities of contraction (sensitivity: 96%, compared with the results of haemodynamics) and that the study of the ejection fractions at rest and on effort was particularly valuable. The ejection fractions calculated by the isotope method were not statistically different from the ejection fractions calculated by the haemodynamic method. The patients with one vessel disease and the normal subjects increased their ejection fraction on effort by about 12% (p = 0.03), in contrast with the patients with multiple vessel disease, who increased their ejection fraction on effort by 2% (p greater than 0.05). In contrast, the results of myocardial scintigraphy at rest and on effort were disappointing. The method was specific (100%), but in our series, its sensitivity was only 72% for the group of patients with coronary artery disease. In cases with infarction and multiple vessel disease, the zone of the infarction was always detected, but no other abnormalities were observed in the other territories.(ABSTRACT TRUNCATED AT 250 WORDS)
