Alprazolam compared to clobazam and placebo in anxious outpatients.

In this double-blind study we compared alprazolam, clobazam, and placebo for the treatment of outpatients suffering from generalized anxiety disorder. At the end of the treatment, the three groups were significantly improved without showing differences among them. However, both active-drug groups were much improved at the end of week 1 in contrast to the placebo group. We were unable to find any difference on efficacy evaluations between alprazolam and clobazam, but alprazolam reached a consistently better outcome on some ratings compared to placebo. Although benzodiazepines share many common properties, it is necessary to identify selected groups of patients that can be helped by specific compounds or even placebo.

[The borderline syndrome: conceptual problem, theoretical models, nosographic location].

The historical vicissitudes of the term "borderline" in psychiatric literature as well as its actual conceptual dimension are reviewed. The latter is exposed organized in categories or theoretical models as follows: as a peculiar or attenuate presentation of a psychosis, a grave form of a neurosis, a passage syndrome between a neurosis and a psychosis, or an independent nosographic picture. In mentioning pathochronic and generic aspects, an intent of unitary conceptualization is outlined which comprises: I) the clinical picture which translates ego pathology into behavioral terms with a "longitudinal" symptomatological hierarchization of functional levels which interact simultaneously sometimes (dynamic psychopathological space-time) and pointing out its dependence on the environment; II) the structural analysis or the dynamic formulation. The actual nosographic position of the borderline syndrome as well as the one established in the proposed Third Review of the American Psychiatric Classification and its heuristic value are examined. Finally, the presence of this syndrome in relation to modern time society characteristics is commented.

[Lofepramine: a comparative clinical study with amitriptyline].

Lofepramine, a new tricycle antidepressant, is compared with amitriptyline in a double-blind study. A brief pharmacological description of the drug is made emphasizing its low toxicity and anticholinergic peripheral effects, high plasmatic concentration levels and good tolerance and elimination in comparison with some other known tricycle antidepressants. Sixty depressive outpatients of a Mental Health Service in Lima, 5 male and 55 female, aging 16 to 65, 29 endogenous and 31 neurotic were studied with both drugs in a equimolar dosage. Through the chi square test, no statistical significance was found in maximal therapeutic response, Hamilton Depression Rating Scale scores, type of depression, and side-effects xerostomy which is lesser with lofepramine. A discussion of these results is made and it is concluded that in the present study lofepramine compared with amitriptyline has a similar therapeutic effect. Though not statistically significant, lofepramine seems to be better for neurotic depression and patients sensitive to anticholinergic side-effects.

[Ecology and psychiatry].

Psychiatry, with a recent history as to a pragmatic discipline and a progressive development from the individual to the social, finds a holistic perspective of singular heuristic value in the ecological proposition. Ecopsychiatry is a general approach in which the diverse sources of psychiatric knowledge are grouped in order to get a view of macro-reality emerging from a methodological integration of behavior sciences and a unified theory orientating to a realistic praxis in the normative. This paper reviews the rationale, notion and historical development of the ecological principle in psychiatry. After a superficial analysis of the first contributions in this field, it is intented to outline the real of the actual psychiatric ecology mentioning their methods and contents. Following Bateson's postulation it is emphasized that ecological principles cannot be isolated from their own ideological contents--the ecological system necessarily involves science's ideas for their concepts need a constant evaluation.

[Prospects of psychiatric research in Latin America].

The authors analyze the needs for research in Latin America and Assess the feasility of several implementations to meet the needs. A review is made of research work already completed or under ay, stressing the scarcity of full-time researchers, and part-time ones, the difficulties for establishing research teams and for creating the most suitable environmental conditions and training facilities. Lack of research planning is only aggrevating the general conditions, and impairing the performance of individual or group researchers, reducing their possibilities of integrating comprehensive programs.
