Regression of a large gastric MALT lymphoma with antibiotic treatment to eradicate Helicobacter pylori.

We report a case of a large low-grade gastric lymphoma of mucosa-associated lymphoid tissue (MALT), which regressed following the eradication of concurrent Helicobacter pylori infection. This case lends further support to a policy of H pylori eradication as a first line therapy for all these lesions, irrespective of size. The long-term prognosis of tumours treated in this way, however, needs further evaluation.

Postexercise protein-carbohydrate and carbohydrate supplements increase muscle glycogen in men and women.

We have previously demonstrated that women did not increase intramuscular glycogen in response to an increased percent of dietary carbohydrate (CHO) (from 60 to 75% of energy intake) (M. A. Tarnopolsky, S. A. Atkinson, S. M. Phillips, and J. D. MacDougall. J. Appl. Physiol. 78: 1360-1368, 1995). CHO and CHO-protein (Pro) supplementation postexercise can potentiate glycogen resynthesis compared with placebo (K. M. Zawadzki, B. B. Yaspelkis, and J. L. Ivy. J. Appl. Physiol. 72: 1854-1859, 1992). We studied the effect of isoenergetic CHO and CHO-Pro-Fat supplements on muscle glycogen resynthesis in the first 4 h after endurance exercise (90 min at 65% peak O2 consumption) in trained endurance athletes (men, n = 8; women, tested in midfollicular phase, n = 8). Each subject completed three sequential trials separated by 3 wk; a supplement was provided immediately and 1-h postexercise: 1) CHO (0.75 g/kg) + Pro (0.1 g/kg) + Fat (0.02 g/kg), 2) CHO (1 g/kg), and 3) placebo (Pl; artificial sweetener). Subjects were given prepackaged, isoenergetic, isonitrogenous diets, individualized to their habitual diet, for the day before and during the exercise trial. During exercise, women oxidized more lipid than did men (P < 0.05). Both of the supplement trials resulted in greater postexercise glucose and insulin compared with Pl (P < 0.01), with no gender differences. Similarly, both of these trials resulted in increased glycogen resynthesis (37.2 vs. 24. 6 mmol . kg dry muscle-1 . h-1, CHO vs. CHO-Pro-Fat, respectively) compared with Pl (7.5 mmol . kg dry muscle-1 . h-1; P < 0.001) with no gender differences. We conclude that postexercise CHO and CHO-Pro-Fat nutritional supplements can increase glycogen resynthesis to a greater extent than Pl for both men and women.

Treating chronic pain.

Quantifiable and measurable methods accurately assess pain severity and successfully guide physicians in determining which medications and modalities are appropriate and necessary for the treatment of chronic pain. A variety of pain assessment scales and the World Health Organization's three-step analgesic ladder can be the starting point for successful pain management.

[An important step in cornea procurement: interview with the family of donors].

BACKGROUND: Since bioethic laws were promulgated in France in 1994, post-mortem cornea procurement can be achieved only after donor's family consent. In order to fulfil this task, we set up a group of individuals including ophthalmologists and members of the coordination team. The aim of this study was to establish the efficiency of such a group and to evaluate the content of donor's family interview. METHODS: During this 14-month prospective study, we analyzed all the interviews we performed with donor families. To report these interviews, we used a preestablished questionnaire. RESULTS: We met 60 families during this study. Procurement rate was 80%. This acceptance was mostly facilitated by advanced mean donor's age (65 years) and by the 12-hour period of time allowed after donor's death. The other important factors were the terms of the interviews, the commitment for a perfect anatomical restoration. We noticed that usually (83%), the family was not aware of donor's last will and had unfortunately to make a choice. Accepting to wait for a final decision helped getting a positive response (86%) and even sometimes helped to reconsider a previous negative position (10%). CONCLUSION: Because of a dramatic lack of corneal grafts in France, more than 7000 patients are on a waiting list. Our results show that positive response can be obtained from the donor's family when a trained group manages the donation request. We strongly suggest that each eye bank create such a group.

Variations in circadian rhythms of activity, sleep, and light exposure related to dementia in nursing-home patients.

We measured 24-hour circadian-rhythm patterns of activity and sleep/wake activity in a group of nursing-home patients (58 women and 19 men with a mean age of 85.7 years). Severely demented patients were contrasted with a composite group of moderate y, mild, or not-demented patients. Sleep/wake activity and light exposure were recorded with the Actillume recorder. Cosinor analyses were computed to determine the mesor, amplitude, acrophase, and circadian quotient of the activity rhythms. The diagnosis of dementia was based on the Mini Mental Examination and on examination of medical records. Sleep was extremely fragmented in both groups of nursing-home patients. Severely demented patients slept more both at night and during the day, but there were no significant differences in the number of awakenings during the night or in the number of naps during the day when compared to the composite group of moderate, mild, or no-dementia patients. The severely demented group had lower activity mesor, more blunted amplitude, and were more phase delayed (i.e. had later acrophases) than the other group. In addition, the severely demented patients spent less time exposed to bright light. These results confirm that circadian rhythms in nursing-home patients are disturbed with more disturbance in the severely demented. Much of the disturbance may be related not just to age but to mental status.

Open access fundholding endoscopy in the private sector.

We conducted a comparative study of 150 patients referred for endoscopy to an NHS hospital and 150 patients referred to an open access scheme in the private sector for fundholding GPs. The concept of consultoscopy, whereby a consultant gastroenterologist gives an opinion at the same visit as the endoscopy is performed, is introduced. Results showed a similar number of normal studies (about 40%) were performed in each group, although the number of examinations showing serious pathology was significantly higher in the fundholding group (p < 0.01). Patients seen in the fundholding scheme had less sedation, required fewer outpatient appointments, and the GPs were given more advice on further management than those seen in the non-fundholding group. We conclude that open access endoscopy in the private sector is a workable option and may result in work being transferred from NHS hospitals to the private sector unless NHS hospitals review their working practices. The concept of consultoscopy is a useful development on traditional open access systems.

Family stress, perception of pregnancy, and age of first menarche among pregnant adolescents.

Family-of-origin stress, age of first menarche, and the perception of pregnancy as a life event were examined in 97 pregnant adolescents ages 13 to 18. This study investigated whether family-of-origin stress levels were significantly related to the pregnant adolescent's perception of her pregnancy as a life event. The study also examined whether age of first menarche was related to the subject's level of family stress. Family stress levels were assessed through the Adolescent-Family Inventory of Life-Events and Changes (A-FILE) developed by McCubbin & Thompson (1987). Perception of pregnancy as a life event was measured on a Likert scale developed by the researcher. Age of first menarche was obtained via self-report. Results indicated high levels of family stress among subjects with only a moderate level of impact or stress experienced from the pregnancy. As a group, the subjects experienced first menarche between 12 to 13 years of age, which is the average age supported by other studies within the United States.

Primary pancreatic non-Hodgkin's lymphoma in a renal transplant recipient.

Primary pancreatic non-Hodgkin's lymphoma (NHL) is rare and accounts for less than 1% of all lymphomas. The development of NHL following transplant is well recognized. However, there are no previous reports on localized lymphomatous pancreatic involvement in a transplant patient. We report on the first case of primary pancreatic NHL developing in a renal transplant recipient. The patient was treated without reducing immunosuppression and received combination chemotherapy and radiation. The patient is in complete remission at 10-month follow-up.

Effect of citrate concentration in specimen collection tubes on the International Normalized Ratio.

PROBLEM: Does citrate concentration in specimen collection tubes affect the International Normalized Ratio? METHODS: The International Normalized Ratio was determined on quadruplicate plasma specimens from 32 patients treated long term with oral anticoagulants-two from tubes with 3.2% citrate and two with 3.8% citrate. Two laboratories, using two different coagulometers, tested the specimens. RESULTS: International Normalized Ratios of plasma from tubes with 3.8% citrate were significantly higher than those from tubes with 3.2% citrate when tested with either coagulometer. Patients given adequate anticoagulation on the basis of the International Normalized Ratio at one concentration of citrate appeared either overanticoagulated and at risk of bleeding or underanticoagulated and at risk of thromboembolism at the other concentration of citrate. CONCLUSION: Results emphasize the need for using a single concentration of citrate for prothrombin time testing. We recommend 3.2% citrate.

In vitro comparison of cytochrome P450-mediated metabolic activities in human, dog, cat, and horse.

As domestic animals such as cat, horse, and dog increasingly become the clinical targets for drug discovery programs, the need to understand how these animals metabolize xenobiotics becomes more important. In the present study, substrates and inhibitors that were reported to be selective for particular P450 isozymes were used as probes to study in vitro metabolism in horse, dog, cat, and human liver microsomes. Seven selective catalytic activity markers for cytochrome P450-mediated reactions were measured: phenacetin O-deethylase (P4501A1/2), coumarin 7-hydroxylase (P4502A6), tolbutamide hydroxylase (P4502C8/9), S-mephenytoin 4'-hydroxylase (P4502C19), dextromethorphan O-demethylase (P4502D6), chlorzoxazone 6-hydroxylase (P4502E1), and testosterone 6beta-hydroxylase (P4503A4). Metabolic activity was found in every species with each substrate. Under the conditions of this study, it was observed that no one species was more active for any given substrate. However, rather large interspecies differences were observed. There was no marked sex difference in the way the various species metabolized the different substrates. The effect of selective P450 inhibitors on the various activities was tested with furafylline (P4501A2), mouse monoclonal antibody inhibitory to CYP2A6, sulfaphenazole (P4502C9), tranylcypromine (P4502C19), quinidine (P4502D6), diethyldithiocarbamate (P4502E1), and troleandomycin (P4503A4). In most cases, these inhibitors were effective to varying degrees against the activity seen in horse, dog, and cat liver microsomes. However, even at high concentrations, furafylline did not inhibit phenacetin O-deethylase activity in cat and troleandomycin did not affect testosterone 6beta-hydroxylase activity in horse. Sulfaphenazole was not tested in dog and cat because of the low tolbutamide hydroxylase activity. Overall, these results show that there are also large interspecies differences in the way the selective P450 inhibitors affect the in vitro metabolism of the various substrates in horse, dog, and cat liver microsomes.

The ESRC review of government social classifications.

In this article we first discuss the background to the ESRC Review of Government Social Classifications and the recommendations of the Review Committee's initial report to the Office for National Statistics in 1995. Second, we discuss the work of Phase 2 of the review, including the derivation of an interim version of a proposed revised government social classification schema and the criterion and construct validation work so far undertaken using this schema. Finally, we indicate the further work required to establish the schema in its ultimate recommended form for use by ONS and others. The main purpose of the paper is to introduce users of government social classifications both to the ideas underlying the review and to the revised version of the current classifications which we shall be recommending to ONS.

Prolonged use of coloured overlays for classroom reading.

Ninety-three children in a primary school and 59 children in two first-year classes of a secondary school were asked individually to observe a paragraph of random letters arranged to resemble text, and to compare the perceptual effects on its clarity of coloured plastic sheets overlaid on the text. A total of 29 colours were compared using 10 coloured plastic sheets and 19 pairwise combinations of sheets, one superimposed on another. The resulting colours sampled CIE 1976 hue angle (huv) and saturation (suv) systematically and efficiently. All the children who reported beneficial perceptual effects (53 per cent) were given their preferred overlay or combination of overlays to use as and when they wished. When the children were examined three months later the children tended to choose a colour similar to one they had chosen previously. Ten months later, 22 per cent of those offered the overlaps were still using them of their own volition. These children, but not those who had ceased to use their overlay(s), read randomly ordered simple words more quickly with their overlay than without. In a second independent group of children referred to the Norfolk Sensory Support Service, who used overlays routinely, the reading speed was similar with a grey or clear overlay; and slower than with the chosen coloured overlay, suggesting that reduction of contrast was not the critical factor. In a third independent group of children in a primary school in Kent, the increase in reading speed with the chosen overlay predicted the children who continued to use their overlay during the ensuing eight weeks.
