[Diagnosis-oriented diagnosis, therapy and fee schedules in oncology. A model trial].

In 1996 the project "operative oncology" was initiated by the Department of Surgery University of Kiel in order to guarantee a solid and longterm financing of surgical oncological interventions with a high quality standard. First step was to determine the detailed costs of the diagnostic and different surgical procedures in order to calculate flat rates per case. Covered by these comprehensive prices per case are also the psychooncological support, prospective evaluation of quality of life and molecular-oncological diagnostics (determination of micrometastasis). The development of project was primarily financed by the AOK health insurance company of Schleswig-Holstein. It was calculated by the GSbG and is supported since April, 1st, 1998 by all health insurance companies of the region.

Influence of microgravity on crystal formation in biomineralization.

Biomineralized tissues are widespread in animals. They are essential elements in skeletons and in statocysts. The function of both can only be understood with respect to gravitational force, which has always been present. Therefore, it is not astonishing to identify microgravity as a factor influencing biomineralization, normally resulting in the reduction of biomineralized materials. All known biominerals are composite materials, in which the organic matrix and the inorganic materials, organized in crystals, interact. If, during remodeling and turnover processes under microgravity, a defective organization of these crystals occurs, a reduction in biomineralized materials could be the result. To understand the influence of microgravity on the formation of biocrystals, we studied the shell-building process of the snail Biomphalaria glabrata as a model system. We show that, under microgravity (space shuttle flights STS-89 and STS-90), shell material is built in a regular way in both adult snails and snail embryos during the beginning of shell development. Microgravity does not influence crystal formation. Because gravity has constantly influenced evolution, the organization of biominerals with densities near 3 must have gained independence from gravitational forces, possibly early in evolution.

[Surgical oncology: possible methods for financing complex oncological therapy procedures].

The AOK Schleswig-Holstein actually finances the evaluation of treatment costs for the project "Operative Oncology". After the primary conceptual phase, the project will be financed for several years and will be accompanied by quality-control studies. In addition to the procedure of the BMG, the costs of psychological support/therapy as well as for new molecular biological tests for the detection of micrometastases are calculated. The reason for this calculation is that these additional costs might be included into a specific, oncological resource-based value system of payment.

T-lymphocytosis in bronchoalveolar lavage fluid of hypersensitivity pneumonitis. Changes in profile of T-cell subsets during the course of disease.

Recently, increased proportions of OKT 4+ helper T-lymphocytes have been reported in bronchoalveolar lavage (BAL) fluid of patients with active sarcoidosis. In this study we were interested in T-cell subsets of hypersensitivity pneumonitis, a disease characterized by a similar increase in BAL T-lymphocytes as active sarcoidosis. We applied an immunoperoxidase method performed on glass slides using the monoclonal antibodies OKT 3, 4, and 8 to study T-cell subsets in blood and BAL of eight patients with hypersensitivity pneumonitis, 11 patients with active sarcoidosis, and ten control subjects. OKT 8+ suppressor cells were found to be the predominant cell type in the BAL of patients with hypersensitivity pneumonitis and recent antigen exposure. After avoidance of further antigen exposure, suppressor cells decreased and helper cells increased. The results suggest that T-lymphocytosis in BAL of hypersensitivity pneumonitis and pulmonary sarcoidosis is mediated by different immunologic mechanisms.

Metabolism and formation of DNA adducts of benzo(a)pyrene in human diploid fibroblasts.

Cultured human diploid skin fibroblasts incubated with [G-3H]benzo(a)pyrene yielded about 10 times more H2O-=soluble benzo(a)pyrene metabolites and DNA adducts of stationary growth phase than did proliferating cultures. This increased formation could be blocked by alpha-naphthoflavone. Trichloropropenoxide and cyclohexenoxide, inhibitors of the epoxide hydratase, inhibited predominantly the formation of DNA adducts. Cultures from older individuals formed significantly more benzo(a)pyrene metabolites and DNA adducts, but control cultures from patients with either lung cancer or melanoma did not. The age influence was not apparent when the ratio of DNA adducts to H2O-soluble metabolites was determined for each individual cell line. However, the proportion of DNA-bound material in the cells from patients with lung cancer was significantly increased compared to cells from melanoma patients or healthy individuals.
