[Fecal incontinence. What to do?].

There are various functional or organic pathologies that can interfere with the defecation mechanism. The authors present 4 clinical cases with fecal incontinence: 1 false enconprese--2 myelomeningoceles--1 ileo-anal anastomose (total colectomy in Behçet disease). All cases were evaluated manometrically and operated with myorrhaphy of the rectum levators, plasty of gluteus muscles with approximation and median suture, (without rectum circumferential dissection), associating contiguous muscles of independent enervation. In incontinence due to ileo-anal anastomose, a prior ileostomy was made and an S ileal anal-pouch reconstructed. In other cases any intestinal derivation was made. In 3 cases, post operation treatment went well without incidents and there was a clear improvement in the continence. In the other case (myelomeningocele) a complication developed-an abscess with suture dehiscence-which most likely contributed to a less satisfactory result. After the incontinence cases being entirely studied the technique followed is easy to use, adopting neighboring muscular groups capable of improving the continence mechanism.

[Vasculitis associated with ANCA].

The authors present a case report of a 62-year old woman, with hypertension for many years. She suffered from weakness, anorexia and weight loss in the last 6 months. On admission, anemia, elevated ESR, haematuria, proteinuria and renal failure were present. Renal biopsy was compatible with chronic glomerulonephritis. The clinical picture and positivity for P-ANCA suggested systemic vasculitis. Later evidence of maxillary sinusitis and nasal mucosae ulcers as well as pneumonitis, although biopsy did not reveal granulomas, suggested the diagnosis of Wegener Vasculitis. Medicated with Cyclophosphamide and Prednisolone, for a year, with improvement. The authors make a brief discussion of the clinical criteria for classification of ANCA-associated systemic vasculitis.

[Ritual circumcision and the plastic repair of phimosis].

The AA. describe two variations of their personal technique of foreskin plasty for phimosis treatment. They utilize a variation of Duhamel procedure, with incision of foreskin dorsal face in italic S in cases where the striction ring is moderate and allows a slipping with glandular exposure. They resected ad minima the foreskin in the cases of very serious phimosis and make a YV plasty on the dorsal face, and if necessary a small excision à la demande of the remaining ventral skin. They achieve a good aesthetic result in both techniques and most of operated cases are hard to distinguish from a normal foreskin. The plasties with foreskin maintenance tend to be used in the modern European culture, where the rites disappeared and hygienic habits gained roots. With the exception of religious reasons or strict medical indications (diabetes, skin lesions, etc.) the phimosis repair by mutilation procedures tends to disappear.

Wuchereria bancrofti microfilarial density of autochthonous cases and natural Culex infectivity rates in northeast Brazil.

Autochthonous Wuchereria bancrofti microfilaraemic cases from three neighbouring cities of Northeast Brazil (Recife, Jaboatao and Olinda) were selected from thick smear or filtration positive individuals and classified by their microfilarial (mf) densities through polycarbonate membrane blood filtration. Individuals were subdivided into two groups: autochthonous cases without any previous specific drug administrations (group A) and those with one or more previous DEC treatments (group B). In Recife, unlike Jaboatao and Olinda (P less than 0.05), the majority of autochthonous cases were over 20 years old and no microfilaraemics were found under the age of 10. The majority of cases in Recife had lower levels of mf density (66.6%), while in Jaboatao and Olinda higher mf carriers (65.6%) prevailed when group A (n = 141) individuals were rearranged in mf density above and below 500 mf ml-1 (P less than 0.01). All subjects in group B (n = 110) had less than 501 mf ml-1 and the great majority less than 100 mf ml-1, but a significant difference (P less than 0.05) between Recife and the two other cities was observed. Captured mosquitoes in Recife, Jaboatao and Olinda totalled 7856, 8010 and 8003 respectively. The infectivity rate found in Recife (0.61%) was significantly lower than in Jaboatao (1.21%) and Olinda (1.31%) (P less than 0.001). The authors discuss the possible influences of previous disease control programmes (medicated salt and individual treatments) applied only to the city of Recife in relation to the outcome of this investigation.

[Torsion of the spleen].

The authors describe, a case of torsion of the splenic hilum complicated with a Salmonella typhimurium infection in a child aged 14 months. The existence of a defect of the spleen fixation to the parietal peritoneum, the wandering spleen, that occurs during foetal life is responsible for this situation. The etiology of this entity as well as its clinical presentation and differential diagnosis are analysed. the therapeutic indications os splenectomy and splenopexy are discussed.

[Reconstructive plastic surgery of the female perineum. A personal technic].

The authors present 8 cases of reconstructive plastic surgery of the female perineum carried out on patients aged between 9 months and 17 years old. As far as newborns are concerned only on five of them a "cut-back" was applied for low ano-rectal malformations, and one an abdomen perineal approach for high malformation, with post-operation complications. The others two have perineal vicious scars resulting from neo-natal phlegmon. They discuss the advantages and present results of the surgical technique applied: reconstruction of the cutaneous and muscular plans without previous colostomy.

The treatment of the painful hip in cerebral palsy by total hip replacement or hip arthrodesis.

The painful dislocated or subluxated hip in the patient with cerebral palsy presents a difficult problem in management. Twenty-three patients with cerebral palsy who had a painful subluxated or dislocated hip with degenerative changes were operated on at The Hospital for Special Surgery. Eight patients had a unilateral hip arthrodesis. Six had a successful arthrodesis initially, resulting in relief of pain and return to the preoperative functional level. A pseudarthrosis developed in two patients, but both had successful revision surgery, one by a second arthrodesis and the other by a total hip replacement. Thirteen of the fifteen patients with a total hip replacement were pain-free and functioning at a level consistent with their over-all involvement. One patient had migration of the greater trochanter and slight bending of the femoral component, with persistent pain. A second patient had progressive loosening of the femoral component over a period of seven years, but continued to walk with minimum pain. Two patients had a recurrent dislocation, one requiring revision of the femoral component and the other, of the acetabular component. We think that the presence of cerebral palsy, even in the patient with severe involvement, is not a contraindication for either a hip arthrodesis or a total hip replacement for a painful deformed or degenerated hip. These patients should be treated for the pain in the hip, and the presence of the underlying neuromuscular disorder should not deter the appropriate treatment. For unilateral hip disease in patients who are unable to walk and in young, active patients, we recommend hip fusion.(ABSTRACT TRUNCATED AT 250 WORDS)

Succession and abundance of Staphylinidae in cattle dung in Uberlândia, Brazil.

Fimicolous Staphylinidae prey on rearing dipterous in cattle dung, acting as their natural controllers, including pests such as horn fly. To survey the abundance and succession of these coleopterans in cattle dung deposited in pasture, six experiments were conducted from March to October 1995 in Uberlândia, State of Minas Gerais, Brazil. Cattle dung pats were exposed at a pasture for 1 hr, 8 hr, 24 hr, 2, 3, 7 and 10 days and were than taken to laboratory separate from each other, for Staphylinidae extraction. A total of 156 dung pats were exposed at pasture, from which 6225 Staphylinidae were recovered. Representing at least 30 species, Staphylinidae sp.1 (29.6%), Philonthus flavolimbatus (22.2%), Heterothops sp.1 (16.6%), Oxytelus sp.2 (7.6%), Aleochara sp.2 (7.6%) and Criptobium sp.1 (4.4%) were the most abundant, representing 87.8% from the total. The increased frequency of the majority of these species along the dung exposition time at pasture, indicated that they would be preying on at all the immature stages of the dipterous, or eggs and first instar larvae of species that lay eggs on the dung after its second exposition day at the pasture.

[Hypospadias. Review of a caseload and of the various techniques used--279 cases].

A review was made of 279 children with Hypospadias treated in Pediatric Surgery Department. Hypospadias was classified in accordance with existence of chordee and position of urethral meatus. Indications and the results are discussed according to Hypospadias type and surgical procedure performed over the years. Glanular Hypospadias (144 cases) were treated as follows: 117 by meatotomy or advancement meatoplasty and postectomy with 1.09 mean interventions per patient (M I/P). 24 by MAGPI procedure with 1,12 M I/P and 3 by other procedures. Distal penil Hypospadias without chordee (87 cases), 23 were treated by Denis Browne Crawford (DBC) procedure with 2,82 M I/P, 62 by Van Der Meulen procedure with 1,48 M I/P and 2 by other procedures. Hypospadias with chordee, penile and perineal (48 cases) 35 had chordee release followed by DBC procedure with 4,0 M I/P, 7 by Duckett procedure with 2,0 M I/P and 6 by other procedures. The present tendency towards the use of single stage procedures, namely MAGPI, Van Der Meulen and Duckett is analysed. Its advantages for patients and institutions--lower M I/P, decrease of hospitalization period, repair completed earlier in life and reduced emotional damage--are balanced with aesthetical results and frequency of complications.
