[Asymptomatic endocarditis? Are consequent histological studies useful in valve surgery?].

After aortic valve replacement for endocarditis, follow-up treatment with antibiotics is imperative. However, the question of how reliable preoperative and intraoperative diagnosis of endocarditis is in cases involving aortic defects is unclear. Of the 187 patients who underwent aortic valve replacement with or without coronary bypass surgery between June 1992 and June 1994, 150 exhibited no indications of endocarditis during preoperative and intraoperative examinations. In 17 cases (Group A) histological findings indicated acute florid endocarditis in 7 patients and chronic lymphocytic endocarditis in 10. Contrarily, histological examinations of 133 patients (Group B) revealed myxoid and/or sclerotic valve degeneration. WBC and LDH activity, examined one day preoperatively and on the first and second days postoperatively, exhibited no significant differences between the two groups, with the exception of LDH activity on the first postoperative day (Group A: 490 +/- 114, Group B: 403 +/- 132, p = 0.04). Of the clinically asymptomatic patients requiring aortic valve replacement, 11.3% exhibited acute florid endocarditis upon histologic examinations. This subgroup cannot be identified based upon routine preoperative or postoperative laboratory tests or intraoperative observation. Histological examination of the aortic valve is useful for identifying the high percentage of otherwise nonidentifiable endocarditis. Further study will be required to determine therapeutic recommendations based upon such diagnosis.

[Heart operations in patients older than 75. Results and postoperative rehabilitation].

A total of 164 patients with a mean age of 78.6 +/- 2.7 years and often critical preoperative conditions [New York Heart Association (NYHA) class III + IV, 78.7%] underwent cardiac operations [coronary artery bypass grafting (CABG) 97; valve replacement, 33; CABG + valve replacement, 18; replacement of ascending aorta and others, 18] with an in-hospital mortality of 8.5% (n = 14). Follow-up was completed for 147 patients (98%) after 16.5 +/- 10.8 months. Most patients were in a good clinical condition (NYHA I + II, 79.6%; late deaths, 8 patients, 5.4%) and more than 95% of patients lived at home or with relatives. Charges for hospital treatment dropped to one third after the hospitalization period for surgery in the first postoperative year compared to the year immediately preceding the cardiac operation.

[CT-epidurography. A comparison of conventional and CT-epidurography with contrast medium injection through a thoracic peridural catheter].

Epidurography with contrast medium is used to verify the correct position of an epidural catheter and to detect malpositioning. There is great variability in the distribution of contrast medium according to the individual morphology of the epidural space and the way it is injected. Results of investigations of the anatomy of the spinal canal and epidural space performed with anatomic specimens, epiduroscopy, and conventional and computed tomographic (CT) epidurography are sometimes contradictory. We have performed CT epidurography in approximately 30 patients to date. Insights regarding the distribution of contrast medium in the epidural space can help to interpret conventional epidurographies and to explain special features, such as the "rail-road-track phenomenon" or the plica mediana dorsalis. METHODS. Patients scheduled for major abdominal operations were studied. Catheters were introduced into the thoracic epidural space on the day before the operation. Conventional epidurographies were performed routinely after insertion of the catheters with a mobile X-ray apparatus, usually in the recovery room, using 5 ml iopamidol (Solutrast 250 M). Selected patients (good mental state and stable psychic condition), after written consent and with approval of the local ethical committee, were investigated with CT epidurography using the same contrast medium (partly diluted). From among the CT scans performed until now, five characteristic images are presented and compared to conventional epidurographies. RESULTS. When small volumes (5 ml) of iopamidol were injected slowly, we regularly observed a railroad-track phenomenon in the AP images of conventional epidurographies. After rapid injection of larger volumes (10-15 ml), the spread was more homogeneous and sometimes outline a lighter zone of contrast distribution in the midline. CT epidurography in these cases (5 out of 30 patients) revealed a plica mediana dorsalis. In the majority of the CT scans of the thoracic epidural space its anterior (ventral) compartment was unfilled by contrast medium. According to the findings of several investigators, dura mater and ligamentum longitudinal posterius have grown together, so that an epidural space does not exist in ventral thoracic segments of the spinal canal. In lower thoracic and lumbar segments both structures are separate, and a wide anterior epidural space was present and filled with contrast medium. In other cases the dorsal and dorsolateral epidural space was completely filled, but the contrast medium stopped behind the spinal nerves and surrounding dural sheaths, as if these structures, together with the connective tissue strands between them, formed a membrane that spread in a frontal plane. A railroad-track phenomenon is interpreted in the literature as the X-ray correlate of contrast medium spread in the subdural space, indicating an incorrect catheter position. According to conventional epidurographies and confirmed by CT scans, this feature is also seen regularly in cases with correct catheter position. Small volumes of iopamidol spread preferentially into the wider parts of the epidural space between the gaps of the vertebral arcs, whereas the dura mater and periosteal layers touch at the inner side of the pedicules and laminae, allowing the existence of only a potential epidural space. This rhythmic widening and narrowing of the epidural space is the explanation of the "railroad track" in AP contrast epidurographies. CONCLUSIONS. CT epidurography is a valuable tool to provide better insight into the morphology of the epidural space when filled with fluid (contrast medium, local anaesthetics), complementing findings using epiduroscopy and anatomic specimens. It stands to reason that this time- and cost-expensive method can never replace conventional epidurographies, but can help to interpret them properly.

[Radiation dosage during hysterosalpingography using a digital image intensifying technique].

Hysterosalpingography using a digital and conventional image intensification technique was carried out on two groups of 32 patients as part of investigation for infertility. Radiation dose was recorded as surface dose. In 51.7% of digital images and 54.8% of conventional images the findings were normal. Surface dose with conventional technique was 442 +/- 144 cGy/cm2, significantly higher than 207 +/- 113 cGy/cm2 with digital image intensification. The number of exposures was also significantly higher when using the conventional technique. The digital images showed no lack of diagnostic detail when compared with the conventional 100 mm films.

Congenital brain tumours: diagnostic and therapeutic approach. With a report of 3 cases.

Three cases of congenital brain tumours are reported, which presented with signs of increased intracranial pressure soon after birth, giving rise to the suspicion of a cerebral haemorrhage or of hydrocephalus. Correct diagnosis of tumour was established by computerised tomography, which additionally demonstrated a concomitant haematoma in each case. Two of the newborns had a primarily fatal course, with no specific treatment of the tumours being feasible. The remaining child underwent two operations, experiencing an unhindered neurologic and mental development thereafter. The histologic diagnoses were spongioblastoma and medulloblastoma in the first two cases, and ganglioneuroblastoma in the last. A conspicuous clinicopathologic feature of this neuroblastoma was the marked change in its growth pattern, revealing a higher degree of histologic differentiation and less malignant biological behaviour when tumour regrowth occurred. The diagnostic, therapeutic, and prognostic implications for this special clinicopathologic condition are discussed, with a review of the relevant literature.

[Transvenous DSA of the extracranial cerebral arteries. Causes of reduced image quality based on a critical analysis of 1000 patients].

In this study images of 1000 patients examined via transvenous DSA were analysed and the image quality scrutinised for each vascular region while determining the causes of image quality reduction. The results show that the diagnostic image quality was 98% (excellent/good 79%, fair: 19%) for a. carotis communis, 94% (69 and 25%, respectively) for carotid bifurcation, 84% (54%, 30%) for distal int. carotid arteries, 91% (65%, 26%) for vertebral arteries and 74% (45%, 29%) for basilar artery. Reasons for image quality reduction were artifacts due to movements by the patients, and were also due to vascular superposition. Reduced left ventricular function resulting in prolonged circulation time and dilution of contrast material was frequently combined with an advanced age of the patients (greater than 75 years). Our studies suggest that transvenous DSA is not useful in patients with significantly reduced cardiac output who do not cooperate properly and are of advanced age. It may, therefore, be omitted to avoid unnecessary stress and high cost. In such cases intraarterial DSA or conventional angiography is indicated.

[Arterial DSA of liver masses].

A total of 58 patients underwent intra-arterial hepatic DSA. 42 patients with hepatic masses were preselected by ultrasound or CT. In addition a conventional arteriography was performed in 34 of 58 cases. DSA without diagnostic quality occurred in 4 cases, in 4 other cases a diffuse metastatic disease of the liver was not detected in spite of good image quality. Comparing DSA with conventional angiograms our results demonstrated that DSA had a reduced imaging of intrahepatic small vessels but without negative influence on diagnostic value. Intra-hepatic DSA imaging could be improved using on-line image documentation, small size of image-intensifier and contrast material with an iodine concentration of 300 mg/ml. Advantages of DSA were observed in vascularized tumors and indirect splenoportograms. Contrast material with an iodine concentration of 200 mg/ml was sufficient for hepatic "vascular mapping" or demonstration of vascular diseases including main arteries.

[Arterial DSA in malignant hypervascularized bone tumors].

In order to evaluate intra-arterial DSA in malignant bone tumors (osteosarcoma n = 2, aggressive osteoblastoma n = 1) three patients were examined angiographically including conventional arteriography and DSA. Compared with conventional angiograms DSA imaging demonstrated a reduced peripheral vascular information due to reduced spatial resolution. Advantages of DSA were an improved visualization of tumor blush and atypical venous return and in addition a reduction of volume rsp. concentration of the contrast material using a iodine concentration of 200 mg/ml for preoperative vascular mapping and of 300 mg/ml for diagnostic tumor imaging.

[Complications at removal sites of autologous cancellous bone transplants].

1022 autologous bone transplants were performed at the University clinic of Ulm in the years 1971-1977. The cancellous bone deposits removed were localized in the anterior and posterior iliac crest, the trochanter major, the tibial head as well as the distal radius. Since some patients had postoperatively experienced difficulty in the healing of the cancellous bone removal sites, we decided to investigate these complications by means of questionnaires, clinical re-examinations as well as X-ray controls (3-9 years after removal). During the early postoperative phase (2 months), approximately one-fifth of the patients had complained about pain at the removal site. 3.1% had more or less pronounced hematomas in the scar regions, but surgical evacuation of the hematoma was necessary in only 0.28%. Additionally, 2 cases of infection as well as one femoral neck fracture following cancellous bone removal from the greater trochanter were observed. At the time of re-examination, performed 3-9 years after cancellous bone removal, only 3.9% of the patients complained of largely weather-influenced pains in the vicinity of the cutaneous scars. Sensibility disorders in the area innervated by the N. Cutaneous femoralis lateralis, were found in all those 7 patients who, in the course of cancellous bone removal from the iliac crest, had received skin incisions extending ventrally beyond the spina iliaca anterior superior. The X-ray controls showed more or less pronounced changes in the bone structure at the removal sites of cancellous bone deposits in nearly all patients, but correlation could only be determined between the radiological alterations at the anterior iliac crest donor site and the respective complaints of the patients.

[Changes in amino acid concentrations in plasma after oral ingestion of oligopeptides and analogous amino acid mixtures].

To what extent an oligopeptide mixture differs in equimolar dosage from an equivalent amino acid mixture after oral intake was tested on 11 subjects. The plotted course and absolute order of magnitude of the two mixtures revealed no differentiation in the total amino acid concentration of the peripheral blood plasma. The same was true of the individual free amino acids in the plasma. The only difference was found in the graphic representation of the maximum amino acid concentration of the individual test subjects, which was usually reached at an earlier date with oligopeptides than with free amino acids. The difference is even more obvious in the portal vein, as is shown in a test series not yet completed, where the plasma amino acid concentration increased at a clearly earlier date after oligopeptide substitution than after the supply of the equivalent amino acid mixture.
