[Effect of variations in middle ear pressure on ruptures of the round window membrane of the inner ear of the guinea pig (Cavia porcellus)].

The electrical activity of the inner ear before and after rupture of the round window membrane was monitored in guinea pigs under different pressure conditions with the aid of electrocochleography. Following studies were conducted weekly over a period of 4 weeks. Findings showed that overpressure below the pressure needed to open the Eustachian tube caused strong temporary functional disturbance of the cochlea, especially at high frequencies. Irreversible changes that were pressure-dependent were observed and occurred mainly at high frequencies. Application of low pressure to the round window membrane caused a functional loss comparable to changes after overpressure. Animals with a predamaged cochlea reacted to overpressure that was below opening pressure of the tube and to corresponding low pressure with a longer lasting functional loss than did animals with an undamaged cochlea.

Conventionalization of germ-free rats reverses the disability of rhein anthrone to induce laxation.

This study shows that rhein anthrone has no laxative potency in germ-free rats because after intracaecal administration of a dose of 50 mg/kg the large intestine transit exceeded 240 min. The time course of the laxative potency of rhein anthrone injected intracaecally was evaluated after peroral inoculation of germ-free rats with the caecal contents of conventional rats. Large intestine transit was measured at consecutive periods, on days 0, 1, 2, 3 and 5 after peroral inoculation. It appeared that 1 day after peroral inoculation the laxative potency of rhein anthrone was already established (large intestinal transit < 10 min) and laxation remained on the following days (days 2, 3 and 5). We concluded that rhein anthrone is inactive in germ-free rats and acquires laxative potency after peroral inoculation of germ-free rats with caecal contents of conventional rats.

Comparison of in vitro binding properties of a series of dopamine antagonists and agonists for cloned human dopamine D2S and D2L receptors and for D2 receptors in rat striatal and mesolimbic tissues, using [125I] 2'-iodospiperone.

We investigated the ligand binding properties in vitro of two splice variants of the cloned human dopamine D2 receptor (the 443 and 414 amino acids long forms called D2L and D2S, respectively), expressed in 293 human kidney cells, in comparison with those of the dopamine D2 receptors in rat striatum, nucleus accumbens and tuberculum olfactorium. The new radioligand, [125I]2'-iodospiperone, showed a similar high binding affinity (KD:0.056-0.122 nM) for cloned human D2S and D2L receptors and for the D2 receptors in the three rat brain areas. Binding affinities of 25 dopamine antagonists and of 10 dopamine agonists belonging to different chemical classes were measured. The IC50 values of the antagonists were virtually identical in the five preparations: spiperone was the most potent compound (pIC50 approximately 9.9), remoxipride the least potent one (pIC50 approximately 5.7). The agonists showed similar IC50 values for the cloned human D2S and D2L receptors but their affinity for rat brain D2 receptors was 2- to 5-fold higher. Dopamine showed shallow inhibition curves, the high affinity binding was 10-fold lower for the cloned human D2 receptors than for the rat brain D2 receptors. Addition of stable guanosine-5'-triphosphate (GTP) analogues shifted the D2 receptors in the rat brain tissues to the "low" affinity state, the low affinity binding of dopamine was equal to the affinity for the cloned human receptor. None of the dopamine antagonists or agonists could differentiate between the two splice forms of the cloned human D2 receptors or between the D2 receptors in rat striatal and mesolimbic tissues. The lower apparent affinity of some agonists and of dopamine in the absence of stable GTP analogues suggests a less appropriate receptor G-protein coupling for the cloned human D2 receptors expressed in the 293 human kidney cells. Unexpectedly, guanosine-5'-O-(3-thiotriphosphate) (GTP-gamma-S) reduced the [125I]2'-iodospiperone binding to the D2 receptors by 20-35% in the rat brain tissues and the cloned human D2L receptor, and by 75% to the cloned human D2S receptor. The inhibition in the last case could be prevented partly by submicromolar concentrations of dopamine. The GTP-gamma-S effect is suggested to be due to reduction of disulphide bonds in the receptor. Recent molecular modelling studies indicated an important role of the disulphide bridge between Cys107 at the start of transmembrane domain three and Cys182 in the third extracellular loop, for the binding of dopamine to the D2 receptor.

[Intranasal provocation with lysine acetylsalicylic acid].

At present provocation tests are indispensible tools for diagnosing acetylsalicylic acid (ASA) intolerance, which is often associated with nasal polyps. Reliable oral and inhalation provocation tests are time-consuming procedures that often cause the patient discomfort because of accompanying reactions. These tests are therefore best done with the patient hospitalized. Studies concerning the sensitivity, specificity and tolerance of the new intranasal provocation procedure using lysine-ASA were based primarily on cases of bronchial asthma and yielded differing results. Our aim was to examine the above parameters in a large group of patients with nasal polyps (n = 153) and to compare this method with the oral ASA procedure. Intranasal provocation with 2 mg lysine-ASA revealed a sensitivity of 0.47 (n = 30) and a specificity of 0.91 (n = 105), with the test period lasting 40 min. A modified provocation test with 2, 4 and 10 mg lysine-ASA and a 100 min test period resulted in a sensitivity of 0.67 (n = 15) and a specificity of 0.84 (n = 37). All in all, only 2 patients reacted to the test with signs of bronchial asthma. Other reactions to the provocations were limited to the upper respiratory tract or were only found with rhinomanometry. These findings show that nasal provocation with lysine-ASA can be helpful in determining whether an oral provocation test is indicated. The procedure does not take very long, is well-tolerated and can be done in the outpatient department.

SPECT imaging of serotonin2 receptors in depression.

Changes of serotonin2 (5HT2) receptors have been described in depression, and long-term antidepressant treatment has been shown to decrease the number of 5HT2 receptors. In this study, single photon emission computed tomography (SPECT), with 2-123I-ketanserin as a ligand, was used to investigate 5HT2 receptors in vivo in the brain of depressed patients and normal volunteers. A higher uptake of the tracer was observed in the parietal cortex of the patients, and there was a right greater than left asymmetry in the infero-frontal region of the depressed subjects and not in that of the control subjects. These findings could indicate changes in 5HT2 receptors in major depression.

The reaction of the guinea pig cochlea to perforations of the round window membrane with and without perilymph aspiration.

The influence of simple opening of the round window (RW) membrane and the effect of aspiration of perilymph on the electrophysiological characteristics of the cochlea was tested in guinea pigs by measurement of the compound action potentials. We found that perforations of the RW membrane failed to lead to either short-term or long-term damage in cochlear function. There was only a slight spontaneous escape of perilymph but without measurable functional loss. Additional aspiration of perilymph led to entry of air into the basal turn and to an immediate loss of function of the cochlea. This regressed within 4 weeks in the middle- and low-frequency ranges. Measurable long-term damage persisted only in the high-frequency ranges. We attribute contradictory results of other authors to methodological errors which we avoided by a specific selection of healthy animals and the development of standardized operation, recording and measurement procedures.

The value of fentanyl/diazepam anesthesia for experimental operations and recordings of compound action potentials in the guinea pig cochlea.

Fentanyl/diazepam anesthesia is an appropriate combination for surgical operations on the guinea pig, since it ensures definitive anesthesia and analgesia without respiratory depression. Comparative investigations with pentobarbital and urethane were carried out to check their applicability for electrocochleographic recordings. We found that fentanyl/diazepam combination anesthesia is more suitable for electrocochleographic investigations than pentobarbital. We were thereby able to prove that pentobarbital has an attenuating effect on electrocochleographic recordings in contrast to the findings reported in the available literature. For this reason, and because the lowest rates of animal morbidity occurred with fentanyl/diazepam, this combination anesthesia should be used preferentially for electrophysiological experiments in guinea pigs.

[Welding spark injuries of the ear. Observations of personal case material].

25 cases of tympanic welding spark injuries of the years 1959-1989 are reported. Early results and late sequelae after conservative therapy, immediate and interval operation are described and discussed. Regarding the tendency of tympanic defects to enlarge after thermal injuries despite conservative and early operative therapy, secondary seal by tympanoplasty is recommended. Even then, however, the rate of residual and recurring perforations must be reckoned as being higher than after tympanoplasty in comparable defects of other origin, due to the inferior nutritive supply owing to the extended scar caused by burning. On the other hand, the rate of inner ear traumatisation with 12% directly after welding injury and with 4% remaining impairment of hearing, is low.

[Cholesteatoma and chronic tubal middle ear infection in children. A 10 year overview].

Over a 10-year period we observed 99 cholesteatomas. In 62% the cholesteatomas lay in the epitympanum, in 18% they lay behind an intact tympanic membrane, and in 18% in the posterior-superior portion of the pars tensa. There were 2 cases of iatrogenic cholesteatoma following insertion of ventilation tubes. In the same period we saw 1229 children suffering from chronic serous otitis media (SOM). A total of 1918 ventilation tubes were inserted in 856 children with SOM, so that the incidence of iatrogenic cholesteatomas is extremely low at 0.1%. Our findings indicate that cholesteatomas in the posterior-superior portion of the pars tensa are more often caused by disturbed ventilation due to SOM than are epitympanic cholesteatomas. Furthermore myringotomy and ventilation tube insertion is not only a means of treating SOM, but it can also reveal a hidden cholesteatoma. Long-term follow-up of children treated for SOM is necessary in order to recognise cholesteatomas and adhesive processes at an early stage.

[Small papular tumors of the perifollicular and perivascular connective tissue of the head and neck area].

The clinical picture of perifollicular benign tumors resemble that of juvenile warts, but the histopathology is completely different. Three different forms of perifollicular tumors, pure or mixed, have been observed. We report a fourth, previously unrecorded form, named perivascular fibroma from its histological appearance. The histological findings indicate a possible inflammatory etiology, as is also suggested by perifollicular fibroma, whereas postinflammatory signs are absent in trichodiscoma and fibrofolliculoma. The prognosis after removal of these tumors is favorable.

[Correlation between nasal polyposis and perennial allergy exemplified by house dust mite and house dust allergy].

A total of 139 patients with nasal polyps were investigated by a standardized history, intradermal skin test and nasal challenge to house dust mite allergy. The aim of the study was to assess the disputed role of perennial allergy in patients with nasal polyps using allergens to the house dust mite as an example. Using our diagnostic criteria we found allergy to house dust mite in 28% of all cases. We found this diagnosis in 44.8% of patients aged 10-39. We recommend, especially in this age group, an allergy test, if necessary with the appropriate therapy. Test correlations are illustrated in 174 patients who have been examined by the above methods. Good reproducibility was found by the nasal challenge, which led to the right diagnosis in 92%, compared with 81% by a standardized history. Therefore, it can be considered to be the key to the diagnosis of allergy.

[B-image sonography in lymph node staging of the head and neck area. A comparison with palpation, computerized and magnetic resonance tomography].

The results of palpation, CT and MRI and high resolution ultrasound were compared in 100 patients with malignancy of the head and neck. Ultrasound detected far more lymph nodes than the other methods. These diagnostic findings were compared with the operative and histological results in 62 patients. Ultrasound proved superior to the other methods: a lymph node metastasis was missed in only 2 patients by sonography, whereas in 20 patients metastases were overlooked by CT and MRI scans, and in 27 patients by palpation. High resolution ultrasound is at present the most reliable method for the detection of lymph nodes in the head and neck.

Influence of intestinal bacterial desulfation on the enterohepatic circulation of dehydroepiandrosterone sulfate.

Selective association of germ-free (GF) rats with dehydroepiandrosterone sulfate (DHEAS) desulfating bacteria allowed us to assess the exact impact of intestinal bacterial desulfation on the excretion and enterohepatic circulation of orally administered DHEAS. Germ-free rats selectively associated with the DHEAS-desulfating strain Peptococcus niger H4 (H4 rats) excreted 50% of the total label recovered within 17 h vs 21 h in GF rats and 13 h 23 min in conventional (CV) rats. Germ-free rats excreted 30% of the total label recovered via their urine. However, association of GF rats with the desulfating microorganism increased urinary excretion to 46%, comparable to the 45.5% found in CV rats. Fractionation of fecal label yielded 70% sulfoconjugated DHEAS and 2% unconjugated dehydroepiandrosterone in GF rats vs 5 and 77% in CV rats, and 55 and 14% in H4 rats, respectively. Our results demonstrate that the intestinal bacterial desulfation of DHEAS stimulated the enterohepatic circulation of DHEAS. This in turn increased the urinary excretion of label resulting in an accelerated elimination of labeled DHEAS from the body.

Consolidating the role of *I-MIBG-scintigraphy in childhood neuroblastoma: five years of clinical experience.

In recent years, *I-MIBG (*I-metaiodobenzylguanidine), which is transported and stored in the chromaffin cells, has been shown to allow good visualization of neuroblastomas in children. This paper deals with 30 *I-MIBG-scans performed in 20 children: 16 with neuroblastoma, 3 with retinoblastoma, and 1 with a malignant paraganglioma. A high detection rate was found for both primary and secondary sites of neuroblastoma. *I-MIBG was generally superior to 99mTc-MDP bone scintigraphy in the detection of bone metastases. Our experience illustrates the unique place of *I-MIBG-scintigraphy compared with other imaging techniques: it makes it possible to define the nature of the tumour, particularly in cases with normal catecholamine levels; to establish how extensive the lesions are at the time of diagnosis; and to confirm complete remission. No abnormal *I-MIBG uptake was noted in the 3 cases of retinoblastoma.

[I-MIBG scintigraphy in the diagnosis of neuroblastoma in children].

Since the early eighties, *I-MIBG (= *I-metaiodobenzylguanidine), which is stored in the neurotransmitter storage granules of chromaffin cells, has been increasingly used for the detection of pheochromocytomas and allied sympathoadrenal pathologies. The aim of this paper is to illustrate, by means of clinical examples, the role of *I-MIBG-scintigraphy in child neuroblastoma and to underline its original place, compared with other imaging techniques, in determining the neuro-ectodermal origin of a tumor as well as in establishing the extension of the lesions at the time of diagnosis and during follow-up.

[Evaluation of the basic health care pilot project of the Turnhout area].

A primary health care project was started two years ago by the PIH in collaboration with the health councils of ten municipalities in the region of Turnhout. The objective was to promote actions on better health that were beyond the capacity of existing regional health care facilities. The project is managed by representatives of the province and of the ten municipalities. The first subject for coordinated action was healthy eating. Three practical projects were developed: a salt concentration analysis of bread from 150 bakeries; a quality evaluation of the deep fat from 110 french fry stands and a secondary school programme entitled "Eat Healthy". A new concept of project development was proposed after the first study. 4 steps have to be considered in the future, which are: first to quantify the amount of the suspected health care problem in the region; secondly to activate an intervention strategy which tends to limit to suggest and initiate long term policy actions. Findings of the to suggest and initiate long term policy actions. Findings of the first project show that to obtain the desired results a well conceived information campaign is essential.

[Antimicrobial effectiveness of azlocillin in cholesteatoma in parenteral administration].

Azlocillin has a broad spectrum particularly against pseudomonas and should therefore be useful for preoperative parenteral treatment of infected cholesteatomas in those cases which cannot be controlled by local therapy. To determine the efficacy in treatment of cholesteatoma the concentration of azlocillin in separate tissue fractions of cholesteatoma matrix, cholesteatoma debris and granulation tissue surrounding cholesteatoma was determined. In 9 cases 2 g azlocillin was injected and in 5 cases 4 g. The concentration drop between the different tissue fractions was equal in all cases. However, as clinical experience has shown, differing high levels of azlocillin were found in comparable tissue fractions of the 14 cholesteatomas. In some of the cases the minimal antibacterial activity was not attained in the cholesteatoma fractions.

New fast preparation of 123I labelled radiopharmaceuticals.

A new fast kit preparation of 123I labelled radiopharmaceuticals such as IMP, HIPDM, MIBG and Hippuran is proposed. A radiochemical yield greater than 99% is obtained at 100 degrees C within 10-30 min. The new labelling procedure is based on the nucleophilic exchange in presence of Cu(I) and an excess of reducing agents. The four kit prepared 123I-radiopharmaceuticals have been used with success in clinical studies involving 400 patients. The proposed method is also compared with earlier described methods which, yielding labelled side products and 123I2, do not fulfill the requirements for kit labelling.
