[Hemoglobin J Amiens beta 17 (A 14) Lys replaced by Asn. Coincidence of a functionally silent new abnormal hemoglobin and a polycythemia vera (author's transl)].

A new abnormal hemoglobin, Hb J Amiens beta 17 (A 14) Lys replaced by Asn, has been discovered during the exploration of a recent polycythemia in a 65-year-old patient of Spanish extraction. Oxygen affinity of washed red blood cells was found to be normal at pH 7.13 (P 50 = 30.0 mmHg, N = 29.5 +/- 1). Cooperativity is unchanged, and no instability was detected. From this study, it is concluded that there is no relation between this functionally silent hemoglobin and the polycythemia. In fact, the recent appearance of the polycythemia, the involvement of the other blood cell lines, particularly the thrombocytosis, the high score of leukocyte alkaline phosphatases, and the results of the bone marrow biopsy led to the diagnosis of polycythemia vera.

[Acute leukemia in 3 children with chronic juvenile arthritis treated with chlorambucil].

Three children out of a total of 40 who had been treated with chlorambucil for juvenile rheumatoid arthritis developed acute leukaemia. No malignancy was detected in 160 patients in those treated with steroids and/or other anti-inflammatory drugs. Chlorambucil may have induced the malignancies and its use should be avoided in rheumatoid arthritis.

[Comparison between acute myeloblastic leukemia in children and adults].

The clinical, hematological and therapeutic aspects of 36 cases of acute myeloblastic leukemia in children and 98 cases in adults observed in Algeria are discussed. In Algeria, acute myeloblastic leukemia is more frequent in children than in other populations. In our series there was a very unusual female predominance. Clinically, we observed in children only, 8 cases of chloroma of the orbit. From the hematological point of view, the only difference was a higher incidence of leukopenia and hyperleukocytosis after the age 30 years. The results of VAMP treatment (vincristin, amethopterin, 6-mercaptopurine, and prednisone) are better in children than in adults. But analysis of the results is more interesting if one places the patients in age groups of ten years. The percentage of complete remissions is maximum in the age group 0 to 10years and then falls regularly.

[Familial leukemia].

Two cases of acute myeloblastic leukemia were observed in a young 22 year old woman, then two years later, in her brother aged 16 years. The elder brother had died previously probably from the same disease. The very rare families in which more than two members had the same type of leukemia show the possible intervention of a hereditary factor in leukemogenesis.

Hypogammaglobulinemia with lymphoid nodular hyperplasia of the small bowel: endoscopic diagnosis of one case.

A 37-year-old man suffering from abdominal pain and diarrhea, was hospitalized because of abdominal discomfort. Gastroduodenal series showed multiple duodenal filling defects. Gastroduodenoscopic examination revealed numerous nodular lesions in the duodenum and biopsy specimen demonstrated nodular lymphoid hyperplasia. The serum immunoelectrophoretic pattern showed a virtual absence of IgA and IgM and a very decreased level of IgG. To our knowledge, it is the first case of hypogammaglobulinaemia with nodular hyperplasia of the small bowel, diagnosed by endoscopy and forceps biopsy.

[Study on Kveim's test. Positive tests in agammaglobulinemia. Comparison with TTL in presence of Kveim's antigen].

1. -Kveim's test (KT) was carried out on 104 patients for whom the diagnostic of sarcoidosis was suspectid. The positive results of KT correlate with other clinical, radiological and histological criteria. 2. - Two patients with idiopathic agammaglobulinemia (AGG) had positive KT. In two other cases of AGG, KT was negative. Two hypothesis are discussed: a) the association of AGG and infraclinical sarcoidosis favorized by the immunological deficit; b) false positive reactions, which raise the question of the specificity of KT. 3. - Lymphocyte transformation test in the presence of Kveim's antigen (LLT-K) was carried out simultaneously with KT 46 times. Among 30 positive KT only 20 LLT-K were positive. Among 16 negative KT 3LLT-K were positive. LLT-K was the only test carried out on 47 subjects. It was negative among 19 healthy control subjects, 19 allergic subjects and 7 patients suspected of sarcoidosis. LTT-K was positive among 3 patients suspected of sarcoidosis and 1 patient with Hodgkin's disease. 4.- LLT-K like other in vitro tests, presents some advantages over the in vivo tests, in particular the rapidity of response. We found that LTT-K was less often positive when compared with KT. The question of specificity is also raised for LTT-K.
