Although, happily, at the present time the actual tendency in mammary neoplastic disease is being increasingly orientated towards conservative treatment, a fairly large number of total mastectomies are still being performed in patients diagnosed at a late stage. The frequency, technical aspects and precise local and general prognosis of this affection are discussed in relation to 63 breast cancers treated surgically between 1975 and 1984. These so-called "trimming" mastectomies represent 4.22% of overall total mastectomies performed during the same period. The surgical technic employed was the Patey type total mastectomy with either primary suturing after-wide separation of borders or closure with a thoraco-epigastric fasciocutaneous flap with internal pedicle. Actuarial survival was satisfactory: 58% at 2 years, 34% at 5 years, survival being significantly better in patients with lack of glandular invasion. This surgical technic should be considered, in combination with other therapy, for these patients, who should not be classed as "unrecoverable".
The physical properties of totally implanted systems for long term chemotherapy were tested before and after in vitro and in vivo chemotherapy. The results show that the drugs used (Cisplatin, Vinblastine and Bleomycin) did not alter the mechanical and microscopic properties of the catheter walls. Although the results cannot be extrapolate to others antineoplastic agents, they confirm the usefulness of the systems for long-term chemotherapy.
The authors report 1,000 cases (357 cesarean sections, 230 hysterectomies for benign lesions, 157 conservative utero-adnexal procedures, 128 tubal plasties, 58 prolapse or incontinence procedures, 70 cancers) ,of laparotomies performed according to the technique described by Mouchel in 1980, i.e. strictly supra-pubic and transverse, from skin to peritoneum, including section of the rectus abdominis. This incision enables to perform in ideal technical conditions, with a minimal complication rate (3 hematomas, 2 incisional hernias for 1,000), and satisfactory esthetic results, almost all of the gynecological and obstetrical surgical procedures (90% of two among the authors' practice). The only contra-indications are, except for cases of previous median laparotomy, ovarian tumors. Neither the high risk of infection, nor obesity, extended hysterectomy, nor fetal distress, represent contra-indications, which is a definite advantage over the Pfannenstiel incision. As compared with the median incision which at some time offered similar results, the esthetics and mainly the strength of the abdominal wall are markedly superior.
Preliminary results are reported of the use of a surgical technique since April 1983 for excision of subclinical mammography-observed anomalies. The procedure involves the insertion of a harpoon-shaped metallic thread, after detection by mammography, to obtain guidance of surgical procedures. Preliminary results in 36 cases included the detection of 10 cancers, confirming documented data in this field, and it would therefore appear worthwhile continuing along this pathway of early diagnosis.
This is a prospective study conducted in order to evaluate totally implanted venous access systems. The report concerning 42 patients is presented. The follow-up is 5 to 17 months (median 9 months). A variety of antineoplastic agents were administered through the device. Complications rarely occur and after utilization all patients are still treated in this way. Physician-, nurse- and patient-acceptance were excellent and this system appears to improve the quality of life style of cancer patients.
21 cases of occult breast cancer were reviewed retrospectively. The patients were followed up from 1 to 9 years. Only one of these patients had partial axillary nodal disease and none had recurrence of mammary neoplastic disease. Thus, the prognosis of occult breast cancer is much better than that of clinical breast cancer and we think that all possible efforts must be made it order to detect breast cancer earlier. Mammography seems to be the best form of screening and in our opinion in every suspect disease surgical exploration must be performed.

This work is a retrospective study of the sarcomas of the uterus which were treated at the Oscar Lambret centre in the last ten years. The difficulty in diagnosing sarcomas of the uterus by the histologist can be appreciated when it is realised that classification of uterine sarcoma is complex. It is not possible to give any statistical interpretation of what is found because of the relative rarity of the cases (about 3% of all cancers of the body of the uterus). The clinical, radiological and histological aspects have been studied. Prognosis is very bad. This means that the type of pathological anatomy that has been found determines the treatment.
Telemetric transmission of fetal cardiac rhythm using a fetal electro-cardiographic signal which has been picked up from the mother's abdomen made it possible to monitor the fetus from a distance in 44 patients. This method of investigation, which is very demanding on the team, should be reserved for high-risk fetal cases. It makes it possible in some cases to pick up an early diagnosis of acute fetal distress, outside the times that are usually given for observations of fetal monitoring, namely multiples of ten minutes. The limitations in this technique are not to be found in the method of transmission itself, which does seem to be fairly easy to achieve successfully, but in the quality of the electro-cardiographic signals received from the abdominal wall, which are unreliable.

Since April 1983, harpoon technique was used for limited excision of subclinical mammography-observed anomalies. The procedure involves the insertion of a harpoon-shaped metallic thread, after detection by mammography, to obtain guidance of surgical procedures. The results of 155 procedures in 149 patients included detection of 38 cancers (26%), confirming documented data in this field.
