[Skin and laboratory tests: comparison of the epicutaneous patch test with the TTL and LIF tests in the diagnosis of medicamentous allergic contact dermatitis].

We investigated 158 patients from 15 to 82 years of age with clinically evident contact dermatitis, diagnosed at the Department for Allergic Diseases Investigations-Clinic for Dermatovenerological Diseases, Medical Faculty in Novi Sad, during the period of 1 year. We performed patch-epicutaneous test, lymphocyte transformation test (TTL) and leukocyte migration inhibition test (LIF). Among the 130 (82.2%) patients suffering from contact dermatitis, with the positive patch tests to commercial or standard battery epicutaneous allergens, 26 (20%) had at least one positive patch test to the medicament. In these cases contact allergic medicamentous dermatitis (SAMD) was proved by positive clinical and allergic investigations. In only one case, patch test was negative, with booth the TTL and LIF test positive. Among the medicaments TTL and LIF tested, antibiotics were the most frequent in 9 (34.61%) cases, analgetics were found in 6 (23.08%). Professional contact allergy to medicaments has been estimated in 5 patients (19.23%). According to the obtained results and statistic findings, relationships between the two variables-TTL and patch test, and LIF and patch test, were estimated due to the contact allergy to medicaments. Both of them were low and negative without statistic significance. Patch test remains to be the irreplaceable test with regard to CAMD.

[Epicutaneous tests for standard allergens and the creation of specific batteries for occupations].

In the bounds of investigating the etiological factor in the onset of allergic contact dermatitis, with a special review of the professional etiology and the localization of face manifestations, we have placed a special emphasis on the forming of special batteries of allergens which would to a greater extent, cover a great number of specific professions and a wide diapason of unprofessional everyday activities of our climate. Our aim was to determine the incriminating contacts within a complex material such as a final product, in which the incriminated allergen is often and almost always present in a lesser concentration than the one that is needed to provoke a positive Patch test in sensitized individuals. In the testing with final products, cosmetic products take the first place (17.32%) in relation to the total number of positive tests, after that come textile products (14.53%), detergents and soaps (14.53%), medicaments (13.97%), rubber, plastics and glues (10.06%), oils and petrol derivatives (8.94%), protective agents in agriculture (2.79%), leather (1.12%), dentistic battery (0.56%) and other allergens (16.20%). During the testing with allergens by specific batteries, the greatest number of positive Patch tests was received within textile battery (23.01%), then with rubber, plastics and glues (15.49%), cosmetics (12.83%) and oils (12.83%). We especially point out the importance of this latter way of testing where we have determined its connection to the testing which is conducted with the final products which are brought to us by the worker.

[Justification for introducing specific patch test series in the diagnosis of contact allergies].

In contact allergy diagnostic procedures, beside the epicutaneous PATCH testing with commercial products, the authors form a standard as well as a certain number of specific epicutaneus allergen series, in order to prove the relationship between results in two different kinds of investigations, and the justification of testing with certain components of a complex product, in which the causative agent is often presented in a concentration which is lesser than the concentration needed for producing a positive PATCH test. The investigation was done including 158 patients with clinically evident contact dermatitis. TROLABR Hermal-Chemie Kurt Herrmann standard allergens were put in one standard and certain specific series such as: oil and petroleum derivatives and cutting fluids, detergents and soaps, leather, textile fabrics, glues, plastics, protective agents in agriculture, the dentist's battery, cosmetics, hair dressings and hair dyes. Allergic contact sensitivity was detected in 82.2%; in the textile battery (23.01%), in rubber, plastics and glues (15.49%), in cosmetics (12.83%), in oils, petrol derivatives and cutting fluids (12.83%). According to the results in two different kinds of testing and the statistical findings, the relationship between the two variables was estimated with a statistical significance on the level of 0.01.

Allergens

Serum immunoglobulin and complement levels in patients with psoriasis vulgaris.

In 102 patients with psoriasis vulgaris, divided into group with slight psoriatic form and the group with severe psoriatic form, the serum immunoglobulin and complement levels were determined. The quantitation of immunglobulin and complement was carried out by single radial immundiffusion. It was found that the IgM levels were significantly decreased in both investigated groups. The IgA and IgG levels were significantly increased only in patients with severe psoriatic form. While the beta1A (C'3) levels were significantly increased in both investigated groups the beta1E (C') levels were not significantly changed compared to control group. It is concluded that the immunoglobulin imbalance in patients with psoriasis, opposite to complement, is depended on the severity of clinical form.
