Antiplatelet antibodies in childhood idiopathic thrombocytopenic purpura.

The serotonin release test using platelets from a panel of donors and autologous platelets after recovery was used to detect platelet antibodies in 42 children with idiopathic thrombocytopenic purpura. The test was done with sera obtained in the thrombocytopenic phase, in partial recovery, and after recovery. The test results for sera from 23 (55%) of 42 patients with thrombocytopenia were positive against platelets from the donors; 14 (61%) of 23 were positive against autologous platelets; and 28 (88%) of 32 were positive against either donor or autologous platelets. Complete recovery occurred in 17 (52%) of 33 patients whose sera had positive tests against the donor platelets at the time of diagnosis. Postrecovery sera from all patients had negative tests against donor platelets, and two (12%) had positive tests against autologous platelets. These results indicate that childhood idiopathic thrombocytopenic purpura is an autoimmune disease caused by platelet antibody that usually disappears with recovery from the thrombocytopenia.

Runaway youths in a southern community: four critical areas of inquiry.

PROBLEM: Significant life events in runaway youth include the experience of abuse, school/legal entanglements, and sexual behavior. SUBJECTS: 78 nonurban, runaway youths. METHODS: The Structural Clinical Interview Instrument and the Coddington Life Events Scale were used for data collection. FINDINGS: The majority of respondents reported life situations replete with experiences of abuse, school/legal entanglements, high-risk sexual behavior, and significant life events. Significant gender differences were reported regarding sexual activity (p = .002). Significant positive associations exist between the use of birth control and sexual activity (p = .002), and between school suspensions and sexual activity (p = .01). A significant relationship exists between the use of birth control and the occurrence of abuse (p = .01). CONCLUSIONS: Much research is needed to develop programs with a focus on prevention.

Enhancing quality critical care education: establishing a consortium.

The demand for expanded knowledge and accountability in critical care nursing has increased the need for qualified critical care nurses and is tempered by requirements that their preparation be cost-effective. The Emory University School of Nursing (EUSN) has pioneered in meeting this challenge. Since June 1988, a Critical Care Consortium consisting of the EUSN and Emory-affiliated hospitals has been operational, providing uniform critical care education to multiple hospital nursing staffs. Instructional resources are being economically utilized, eliminating duplication of efforts. This collaboration between academic and service settings enhances productivity, quality patient care, professional growth, and promotes significantly greater interinstitutional cohesiveness.
